    In this video on thyrotoxicosis management and thyroid storm treatment i have talked in detail about how to manage thyrotoxicosis in emergency department.     Thank you. I will show my ER Dr your CZcams video if I have storm again. I laid in ER 4 hours with a raging heart rate. With IV, but no meds and the thyroid labs were back, Thyrotoxic Crisis. I was beyond anxiety, into a peaceful calm and accepted I would die.

    Sir , if patient is taking thyroxine for long time due to hypothyroidism and after long time checked tsh level which is very low ..so, should we stop thyroxine or start methimazole?

      Exogenous thyroxine causes TSH suppression. If patient was started on exogenous thyroxine and TSH has fallen below the normal ranges it means the dose of Thyroxine is high and it must tapered off(Lowered) till TSH comes back into the normal range.No need to give methimazole.

    Really commendable. You have wonderful knowledge and reply in very precise and professional manner.
    I have one question in this regard: My daughter age 21, perfectly alright, Alhadmulillah. Her weight is ok and almost stable (not increasing or decreasing). We got her routine blood tests like CBC, RFT, LFT, TSH, FT4, FT3 etc all are normal except very slight decrease in TSH, 0.39 instead of 0.4, In past (2019-2020) it was also normal. Kindly give your opinion:
    1. Is it significant i-e TSH...0.39 instead of 0.4 pls.
    2. Your advice in this regard. Hoping ur reply soon as usual please. Dr Muhammad Zareef Ph D

      Dr. M Zareef, if Patient is asymptomatic, then this is nothing to be concerned about. It's a very minor difference.

      Thnx a lot Dr sb for your reply. Some Endocrinologists say that if FT4 and FT3 are normal and patient is asymptomatic , as u pls said. TSH fluctuation doesn't matter a lot. Do you agree Pls.
