- Dr. Ebraheim’s educational animated video describes difference between the quadriceps avoidance gait and the active quadriceps test and the different indications and prognosis.
What is quadriceps avoidance gait? It is an ACL injury. Most people with an ACL tear will alter their gait to avoid anterior displacement of the tibia which occurs when the quadriceps contracts. The patient will walk with a slightly bent knee to avoid making the quadriceps work. This decreases the demand for the function of the quadriceps which minimizes the anterior pull on the tibia by helping the knee to be more stable. Quadriceps avoidance gait occurs by hip flexion, knee flexion or both hip and knee flexion. The patient leans forward so the quadriceps do not have to work (the hip is flexed) and the knee is bent.
What is active quadriceps test? It a PCL injury. The examiner stabilizes the leg of the patient while the patient is asked to actively contract the quadriceps.
Pivot Shift test: a palpable clunk is very specific of an ACL tear. The iliotibial band will reduce the tibia and create the clunk on the outside of the knee.
The hamstrings muscle are used in therapy for an ACL tear since the hamstrings pull the tibia backwards.
After surgery to the ACL, open chain quadriceps exercises such as seated leg extension should be avoided. After surgery to the ACL, open chain quadriceps exercises such as seated leg extension should be avoided.
Mechanism of Injury: Dashboard injury is a common cause of injury to the PCL is due to a bent knee hitting a dashboard during a car accident. Direct blow to the proximal tibia with the knee flexed.
PCL injury: the quadriceps muscle is strengthened during rehabilitation of a PCL injury. The hamstring will create a posterior pull on the tibia and cause increased stress on the posterior cruciate ligament reconstruction.

I was on a job accident at work April 30th 2018. I have been to 3 general doctors none of them seem to understand the pain I am going through. They only see the x ray. And the x ray does not show details. This video relates more to the symptoms of my injury
Thank you
Dr. Nabil thank you!
This really clears all doubts...thank you for informative videos.
Very simple explanation
Seated knee extension must be avoided at last 30 degrees only which cause strain at acl
But from 90-45 is completely safe
Thankyou so much Sir love all your videos they are best
AWESOME AS ALWAYS
Very interesting considering that my doctor prescribed me quadriceps strengthening physiotherapy for my sprained ACL. I wonder where did that go wrong because I have some pain and minor joint instability and weakness, lachman test was negative by the way.
Can we heal high grade tear pcl ligament without surgery and can we able to do leg workout after that
i love your all vedios its really very helpful for me thank you so much sir for giving good knowledge via you tube

What if both Acl and Pcl is injured?
Doctor, I am suffering from acute quadriceps muscle loss on both my thighs.
It may be due to disc problem. My left thigh lost 100% quadriceps muscles and recently started loosing my right thigh muslces also. It is diffuclt for me to climb stairs and walk. Kindly let me be helped as to there any solution to this problem? I am 62 years old working in Abu Dhabi. Is surgery advisable?
Good video....
dr. nabil ebraheim knee subluxations and closed realignment Are you willing to advise about rehab. my 2 patellae are subluxated laterally and i cannot walk. i am trying to realign them somehow myself but the rom is not great enough so i cannot hold my weight with the surgeried knees. i have been trying to increase rom and flexibility with yoga but i'm afraid i only caused more problems and should've strengthened the quads, hamstrings, gluts and adducters first. MCL injured/distended/sore also. thanks for any ideas of how to do it for yourself as i have white coat phobia...
1st time to comment in CZcams...... thank you

Sir Ji mera pcl tut gya h six month ho gye ab Dr n bola operation hoga two month plaster bhe rakha bher bhe pura mument nhi hua h
So once you recover from either pcl or acl injury, should you always avoid exercises that either doesn't like? Wouldn't that create imbalances that can further injure an atheletes?
I believe the point is when you're injured and/or fresh off surgery, you avoid certain exercises in the beginning. You don't avoid them ever/all your life.
Sir what is papillon ligament injury
thank you !! i love you
