Yeah the army camp even behind the wall is almost fully rezzed and complete. The Inn outside the Nilfgardian Army camp (the one you need special clothes to enter) was planned to be used for the quest at the camp. My best guess is you had to take out a Nilfgardian commander and steal his clothes, then sneak into the INN and get information for the ScoiaTeal leader. They didn't add in the Nilfgardian armor set to the game (the one that allows you to enter the "hidden" inn) until a much later DLC. The whole Inn is there, but completely empty of interaction and the NPCS never leave and there always looks like a vacant spot next to a guy that is waiting for you to talk to him.

And the apple part is just the commonest loot u could find on random dead villagers for some reason. Not sure what the apple is supposed to reference, but when I played, I found it one of the most common loot items on random dead bodies. Also, I don't think Master Mirror would have a body or a corpse, or at least he could change it as a demon or devil.

    - the ship might have sunk or only noticed he is missing way later when they had already moved away, so no rescue attempt could be made

I think they planned the beginning and end of Hearts of Stone when they made the base game (which is why you meet him way at the beginning). But they forgot that they put the corpse down, so they didn't edit it to only appear after quest completion.
The outfit and face change is probably just a design change between making the corpse and the final model which talks to Geralt. The code function which tells the loot to be an apple, or in a smaller chance, a few coins, is probably just labelled 'apple' so they picked it and the coins are unintentional.

Great videos. I've been having your "Missed Details" playlist on in the background as I go through my first playthrough of The Witcher 3. Since those videos are quite old, you might know these things already, but I thought I'd mention them on the off chance that you haven't.
First thing: You mention how the guy with the single monocle during the frying pan quest in White Orchard could be Thaler, but he's clearly and very much alive during the side quest to kill Radovid being helpful and making poor rock trolls some shoeses.
Second thing: I don't really know if this qualifies as interesting or not, but I finished the storyline with Triss and she left for.. somewhere, can't remember. But for some reason after that, quite some time after actually, I still found the lame duck guy and was able to save him. Geralt even still had the option of telling him to go see Triss. Dunno.
Third thing: this is the only actual interesting thing I've found imo. During a quest that's called "Wolf and Cat Play" or something like that, you go to help a village and the people are killed by a Witcher from the Cat School. One interesting thing I found was that once you track him down and find him by a tree, if you don't talk to him immediately and wait for him to have overworld dialogue, he will mention your gear. At the time, I was wearing all Cat School gear and he mentioned it asking if I was some kind of hybrid Witcher. Kinda cool. I don't know if he will have other dialogues if you're wearing the full gear for other schools though.

Well, you can learn about this quest when you go ask Palmerin to introduce you to the duchess. If you go from Corvo Bianco and around the arena at night (not sure if it's there during the day) and go to the tent in which Palmerin's asleep, you can eavesdrop on two guys on your right side, one of them is sitting on his horse and talks about some mission he has on that particular lake. However, there's also at least one in Velen. Can't get too specific unfortunately, but it's in the Southeast portion of the map, not too far from where a hag (water? grave?) Is guarding some treasure. It's fairly far in land considering its a whale...makes you wonder how it got there. Another appears during 'Through Time and Space ' in the world with the red weeds, though I believe that the devs were just trying to pass the assets off as the bones of a mysterious giant alien creature.

This corpse can be found in Velen in the Funeral Pyres quest. The one with the Niflgaardian Priest where he pays you to burn some bodies. It's used quite often even multiple times at the same pile.
