    Exceptional video! My dog was diagnosed with Cushing's but after going to a 2nd vet I've found out my 1st vet overprescribed Vetoryl and crushed my dogs ACTH levels. This video has clearly explained what is going on. Thanks so much! I feel much more informed!

    what about if there is surenal adenoma? though acth either from adenohypophysis or other tumor organs is down surrenal will continue to produce abundant of cortisol? what test we have to do?

    How much should dexamethasone suppress cortisol ? I got point 7 . It shows as low on MyChart but idk maybe this is normal

    Hi that was a really helpful video.
    Just a quick question; if I used a high dose dex test and cortisol levels was still high; instead of it being a tumour in the lungs causing an increase in ACTH, could it instead be an adrenal adenoma releasing excess cortisol; I'm assuming dex would not stop the adrenal glands from producing excess cortisol if they were the issue?

      you are right… high dose dex test dont supress the production of the cortizol by adrenal adenoma

    Is fasting recommended for dogs prior to the low dose dexamethasone test? My vet had us withhold all food for 24 hours. Her test came back negative, but I am afraid it was skewed from the extra long fasting.

    Probably late to the party. My cardiologist did blood test (I have POTS with orthostatic high blood pressure) and also a blood test of my hormones. Test results came back (I will have my cardiologist visit in 3 weeks) and my ACTH and cortisol are both over maximum limit. Can this might be Cushings?

      I only heard of POTS this past year (a woman with MG was misdiagnosed with panic disorder when she moved during a procedure!) I can't diagnose online - try to ask an endocrinologist!

    Why doesn’t the high amount of cortisol in body suppress ACTH release in first place?

      High ACTH caused higher than normal cortisol which by negative feedback suppressed further more increase in cortisol. Dexamethasone administration now have negative feedback which makes the reduction from high cortisol noted before the test

    What if a patient does shift work (11-7) or goes to sleep between 4 and 6AM? Can this interrupt the normal release of these hormones?

      Hey Jen, my guess is that it makes no difference because cortisol will be suppressed either way, but I would ask an endocrinologist. Please let me know if you find out!

      @@AJmonics I am here because my Endo doesn't know. I was referred to him because of low acth, dhea, and tumor on the adrenal. Last year he only did a baseline and 8mg suppression tests. Said everything is good. He said he'll do 8mg suppression test every year (no baseline) for 4 years to see if the tumor is active. This year, I questioned him about my sleep cycle and he said he'd research it.
      From you wonderful presenters on CZcams, I've learned that I need acth and cortisol done and 1mg, then 8mg if necessary, correct?

      Hello Qiana! You take 1 mg the night before (around 11pm), and your blood is drawn the next morning at around 8am. Please let me know if you need anything else! Good to hear from you!

    What does it mean if cortisol is very low after this test? Like under 1.20 is that normal?

      Great question Jennifer. I believe anything lower than 1.8 micrograms in the morning is considered low.
