    Thank you for this video. I went to Declan for a second opinion about treatment and left feeling so much better about the predicament I was in. Although I had my surgery done with my regular Urologist I was grateful to have had a really good open chat with Declan. I was fortunate in that the cancer was pretty much in the centre of my prostate so nerve sparing was done and was able to get erections after about 7 months (not as good quality or as easily as before but am grateful for whatever I get). Had to get used to orgasms without ejaculate, and that is something I will always miss. So grateful to have the level of care that is available to us in Melbourne. Thank you Victoria for the information you have been sharing on this subject. It is very much appreciated.

    Well done, Victoria and Professor Declan Murphy! I'm very happy you set the expectation that even if the erection nerves are spared, they may take several months or years to recover from the shock of the operation with the possibility they may never work again. For me, it took a good 17-months, although I need the help of viagra and they don't always work. Also, it might be worth mentioning that men do not need an erection to have an orgasm. That's something I learned as a kid going through puberty with a big collection of Playboy magazines.

    Excellent video - thanks to Declan for such a great, candid explanation of the "nerve sparing" process. Also noting his mention of the importance of early detection so that men can have the best outcomes possible - in terms of staying alive and having a better quality of life after treatment. We really appreciate the time and care taken by Declan and Victoria to make this video. Just attended a large Men's Shed gathering earlier in the week - approximately 200 men. A show of hands suggested that half of them had already lost their prostates!

      Thanks Fiona! We published a big paper last year confirming that over 90% of newly diagnosed men in Victoria have localised cancer (confined to the prostate). But a significant number of these will relapse and develop advanced cancer. We have a lot of interest in improving outcomes for these more threatening cancers

    Hello from the states. Im 55 and 13 months out from my nerve sparing RP. Ive had no luck with viagra and Cialis so far. I have used aprostadil injections to preserve the muscles and create artificial erections. I did have one spontaneous erection a few weeks back although I have not been able to recreate that again lol. I am grateful that my cancer has been removed and my PSA is currently

    This was a really useful video. Thanks. As someone with PC and likely to have surgery where unilateral nerve sparring is the best I can hope for it was really hard to find information online about the different grades of nerve sparring and the relationship to ED.

    Hi Victoria, great vid with lots of good info around expectations. I'm 2 months down the track from a prostate removal and know I'm not going to set the world on fire for for another 1 to 2 years if at all given the nature of this surgery and the complications around these nerves (fortunately there are the other great options out there to lead a relatively normal intimate life with our wife/partner). I have read some info on re-growing nerves to restore erectile dysfunction, this procedure as far as I know is performed by Professor Chris Coombs out of Melbourne. I asked my urologist and he has read the published paper but has some issues as to is effectiveness given the way the procedure is done and over time its actual success rate may decrease as the restored erectile function may be natural over time rather than due to the surgical intervention. Do you have any further insight into this procedure?
    On another topic I have heard of a relatively new prostate removal procedure performed by a Dr David Samadi's in New York that allows the patient to be relatively continent immediately after surgery if not within a couple weeks. Have you heard of this procedure before?

      These are fantastic questions Cam - I also want to get more expert opinions on these new procedures. Clearly there needs to be Part 2 with Declan about these new techniques and others in the space.

      @@ATouchySubject Good questions from Cam.The novel nerve grafting procedure led by Prof Chris Coombs and Dr David Dangerfield is definitely of interest (I have referred a few of my patients already). Nerve-grafting has been tried many times previously and has failed, but their adjustment to the technique is showing some promise. I was a reviewer on the paper and was impressed with it, but it is early days and will need to be further investigated. I am happy to refer patients from Cancer Specialists and Peter Mac
