सर,basalog twice daily कैसे लगाए??
पहले मैं सिर्फ रात को 10 बजे ही लेता था 24 यूनिट्स,उससे फास्टिंग 120 रहता था,लंच के बाद भी 160 के आसपास आता था लेकिन शाम 6 बजे के बाद मुझे थकावट लगती थी,नींद आये जैसा लगता था और रात डिनर से पहले 8 बजे शुगर 220 होता था, तो मैं novorapid 26 से बढाकर 30 लेता था,तब भी डिनर के बाद 240 ही रहता था,और 26 यूनिट क साथ तो 300 रहता था,, मुझे लगता है basalog (और lantus भी) मेरे अंदर 18 घण्टे ही काम करता है,तो बसालोग 12-12 किया तो फास्टिंग 180 आता था और लंच के बाद 140 आता था लेकिन शाम तक लो हो जाता था, हाथों में थरथराहट होने लगती थी,
अभी मैं सुबह 8 बजे 8 यूनिट और रात 8 बजे 20 यूनिट बसालोग ले रहा हु,,,फास्टिंग 118,लंच के बाद 155 और डिनर के पहले 120 आता है,डिनर के बाद भी 160 आता है,, क्या ये सही है??क्योंकि मेरे डॉक्टर ने "either you take 24 units at night or take 12-12" बोला था,,मेने खुद से मेरे डोज चेंज किये है,FDA ने lantus को सिर्फ once daily के लिए ही approve कर के रख है,क्या ये सही है कि मुझे टूजेओ या ट्रेसीबा पर शिफ्ट होने की जरूरत है?😢

Doctor, I don't fully agree with you the way you are telling them to increase the insulin dose but at the same time, you are not at all educating the patients that the real cause of elevated sugar is a high intake of carbohydrates in their diet. so you must also educate that they should reduce the carbs intake and eat healthy food. Secondly, you are saying that you start insulin only after the oral medication stop working on the patients, which is also incorrect. In my opinion, Insulin is the most natural way to treat diabetes if there is a shortage of insulin in the body of the patient because by giving tablets you push the pancreas to make more insulin and as a result, all the beta cells burn out and the pancreas stops producing any insulin and then you have no other option but to start the insulin doses. Instead of this, if you educate the patients to reduce their carbs intake at the beginning itself when their HbA1c crosses 5.7%. it's quite possible they don't become full diabetics. This kind of treatment which doctors educate is to make new patients from the people who have high Insulin resistance and then give them such drugs to burn out their beta cells and then push them to take high & higher doses of Insulin. This is not the right approach. Please think about the fate of the poor patients. Because your line of treatment forces a patients on a progressive disease which finally results in heart failure, dialysis & amputation etc. only the hospitals and doctors are benefitted but not patients. think about it.

None of the doctors is going to tell that, dairy is the biggest cause of type 1 diabetes, as the casein protein of milk have same structure as of beta cells of pancreas, our immune system kills the beta cells, thinking that its casein, and taking insulin can be said as natural but its no way less harmful, as it a hormone and a human can never judge accurately how much and at what time, we need particular amount of insulin (hormone). Turning vegan and eating a lot of raw veggies and fruits is the best option for survival without needing any medication,
And none of the doctor is going to tell the latest American diabetes association guidelines that if your blood sugar level is less than or equal to 250, you don't need to take medication, because without medication you'll live longer as compared to taking medicines to control it under 100, as taking medicines to control level under 100 increase the chances of death by many times

Yes you can take , I am also taking basalog in morning and human actrapid insulin before every meal but you have to adjust insulin dosage as per carbs intake

In pts not taking any oral hypoglycaemic :
0.5 unit/kg/ day is the minimum dose in non obese pts.
1 unit/kg/day. Is enough in 80% pts.
(Av dose 0.8 - 1.5 . For obese /stressed or one who does not exercise the dose is 1.5 units/kg/day)
However, if the pt is taking meds the Insulin requirement will be less :
0.2 units/kg/day.
Titrate with FBS in on alternate mornings :
Target FBS is 110.
