      In 2nd year if you have a chance to visit wards do take history of patients daily and learn basic physical examinations. Even some students in internship don't know the proper way of history taking so focus on it from day first. Best of luck

    This may be OK for Asian countries but in western setup we never ask for father or husband name. Also the ward sheet will have all the intro of the pt. Asking them means you haven't studied the pt case before hand and that sort of questions will annoy the pt. These type of questions are more for the first time pt visiting a GP.
    Usually intro , consent and then presenting complains onward.
    Rest is fine.

      @@theeasymedic5874 why not go for patient id wrist tags, reduces the likelihood of confusion. Rather than using their names, they should be referred to as Patient no. Genuinely helps in reducing the risk of wrong treatment.

      He's teaching history taking for third year medical students in a teaching hospital. You're not supposed to view the patient file prior to taking and presenting history. You take it the way it would have been taken when they first reported to the OPD and do your best to reach a diagnosis. The demographic data is important in identifying conditions affecting particular regions, in some genders or peculiar to certain occupations. When you present this history to the consultant or resident they assess if you've collected all the demographic data and if it's relevant to the diagnosis they ask you how so to assess if you understood it's importance.

      No PC should be simple words of patient , instead you can write yellow discolouration of skin eye

      PC is anything for which patient is seeking medical assistance . so PC can be anything and it is usually one or two main symptoms which patient tell you initially
