When I got off insulin, if I ate more than 4 days in a row, my bG rose enough to need insulin. So in order to STAY off insulin, I instituted a 3-day fast every week Mon-Wed, the other 4 days being OMAD, usually an RBS (ridiculously big salad). Yes, this meant I was eating all of 4 times a week, and yes, it sounds pretty extreme, but I was extremely ill and that seemed to be what I needed to do for many months. Today, I am currently still fasting 3 days a week, but not in a row anymore, rather Mon, Wed and Fri. And on my eating days, I usually do OMAD, but sometimes do TMAD. Most of the ex-diabetics I know didn't do anything nearly as extreme as I did; indeed one seemed to reverse diabetes in around 2 months just doing OMAD. But I also know one who had to fast *much* more than I did to get off insulin; she's been doing multi day fasts for over a year. It seems to me that what's required varies a whole heck of a lot. My last video, I made the point that what you are calling a 14 hour fast, which is how I grew up and was just how everyone ate back then (we called it: 3 meals a day!), is likely appropriate if you're not fat, sick and weak and just trying to avoid becoming so. It's likely "enough" for most people to retain their metabolic health. But for those who need to regain being trim, strong and healthy, likely much more fasting is needed. Somewhere in the equation of determining how much fasting is best, desperation is an important variable.

I'm looking forward to it, as my weight has now slowed down drastically after losing 58lbs. Also, I'm early into menopause and hope it helps with hormone issues.

Hello. I have read all of Dr. Fung's and your books. I absolutely believe in it. I tried but I run into trouble because I take long acting insulin and it is hard to figure out. My doctor is absolutely against anything to do with fasting. I cannot change doctors as they are rare in my neck of the woods in Canada. Thank you for the clear video. I don't know for certain that I have metabolic syndrome but chances are I do. I've definitely yo-yo dieted for the last 30 years. I weigh over 350 lbs. I'm a 50 yo woman. It sounds like my best course of action are the 36 and 42 hr fasts. Recently I read that these longer fasts raise cortisol levels though. Is this a concern you can speak on? Hi Sasha, 36 and 42 hour fasts seem like a great plan for you! As for the cortisol, it can definitely increase during a fast, but just stay hydrated, consume lots of sea salt, get good rest, and stay busy, then you should be alright.
I'm curious to know what is recommended for someone looking to lose 10lbs, and doesn't have any health conditions. Would two 24 hr fasts be sufficient? I switched over to intuitive eating instead of tracking calories and macros. My body generally goes for around 18-22h fasts before I get hungry/weak. Doing a keto/low carb diet, and I exercise daily as well. ^^ Every few months I do extended fasts as well. As aType 2 Insulin dependent diabetic with CGM and aiming to improve Insulin Resistance, I’ve started to trial 24 and 48 hr fasts once a week in advance of starting a full weekly program. I’ve read all of Jason’s and your books, trialed TheFastingMethod.com and am so inspired. Unfortunately though, my doctor and diabetic specialist nurse don’t support fasting or anything that resembles a Ketogenic diet. (by the way, I’m in the UK). For my 48hr fast I suspended all Insulin to avoid Hypo’s and this worked perfectly in that I had good steady blood glucose for the full period and improved BG in the days following the fast. I’m wondering what others are doing about Background Insulin when doing an extend fast ?

Today is my 4th day of 24 hour fasting, and its been great. Firstly eating 1 meal a day is very easy given i dont need to keep snacking and looking at things to eat and cook, secondly because the 1 meal is high fat and protein, it makes me full fast and I feel great the who 24 hours. I am a 67 yr. old male and have been doing the Keto WOE for over a year now and have lost just over 40 lbs., but have plateaued in spite of a few sporadic 20/4, 42 hour and 72 hour fasts (I currently weigh 195, but would like to get down to 180 lbs). I would like to do the 36/42 hour fasts 3 times per week as you suggest, but I am concerned with the total amount of protein for the entire week being too low to maintain muscle mass. On the other 4 "feasting" days would it be recommended that I maintain a normal macronutrient intake (i.e., 5/20/75) up to what might be my normal daily caloric intake minus 20% calorie deficit (2000 - 400)? I do not suffer much from hunger pangs and can easily eat too little during the non-fasting days, and I normally only eat twice per day. Sorry for the long comment. Thanks.

Extended fasts are great because you're in fat burning mode for longer, so your schedule should work. If you find yourself in a plateau, consider changing it up!
My concern is electrolytes. I get terrible leg cramps. Is there a broth and electrolytes protocol? I guess I am asking what can be consumed during the fasting times.

Does metabolic syndrome appear as flabby fat looking? I was always thin and tone and after menopause it’s as though my thighs and belly are all flab and fat especially if I eat something that I may be sensitive or react to. Thanks for your help
