Once she was settled an I was alone all I could think was "shit I couldn't raise a kid I didn't want to have in the first place" specially not while I was still in education
took till I was 30 for doctors to agree to do it, and tbf I didn't have any accidental pregnancies in that time...

The real issue in getting a male birth control is in the mechanics of it.
For female birth control you are essentially yoinking away the finishline for the sex cells. You prevent a fertilized egg from implanting into the womb. The egg and sperm cells are not directly effected.
While for males you have to find a way to make his sperm unable to get to the egg cell in the first place without permanently removing the ability from the man's sperm long term.

    • @harringt100
      @harringt100 Před 3 měsíci +27

Most "female birth control" does not affect implantation, or to the extent it does, it's a minor effect. (Like, statistically speaking, it's more likely for an egg to be fertilized and the embryo fail to implant in the uterus if a couple is just having sex randomly and no birth control is involved than if the woman is using hormonal birth control.)
Hormonal birth control (including plan B) is formulated with the goal of preventing _ovulation_ , not _implantation._ Preventing sperm production, although it might be more difficult, is pretty analogous to that. (Including the fact that women who stop using hormonal birth control usually need time to regain normal fertility.) Also, all types of hormonal birth control have side effects. But on the whole, the side effect weight is probably lower if you're not on them continuously. (i.e. if you take Plan B once after a condom failure versus being on a daily birth control pill.)
What this comment said about "yoinking away the finish line," is somewhat true in the case of IUDs, but all IUDs currently available have other mechanisms of action that come into play before implantation. (Primarily effecting the sperm or the cervical mucus, which is just as essential as the gametes.) And they're not as popular in the U.S. as hormonal birth control formulated to prevent ovulation.

    • @unlimon6382
      @unlimon6382 Před 3 měsíci

That's not how the pill works. It just stops ovulation???? 😅
how are you not wondering how it doesn't stop production. More agressive treatments like old plain chemical castration can be reversed. If you took the meds for a long time you may have more lasting effects but overall that is straight up castration and it can be reversed. Why is it so hard to believe this can be reversed too? And how come people are not informed on how female birth control works or how it has big effects on women's hormones? "No, it's fine cause it just prevents implantation and I suppose it harms nothing else but it would harm men!"... what?

Yes, there are SOME men who do not take responsibility, just like there are some women who don't, but the majority of men do.
Before I wanted to settle down with children, I wore a condom every time, without exception (literally). After we had the two children we wanted, I got a vasectomy. This was the norm in both my friend and family circles.
On a side note, have you looked at the statistics on unwanted pregnancies and how many result from female contraceptive failure due to improper use? There is a 10% difference between the efficacy of the pill when used correctly and for typical use. Can we make it not "normal" for women to screw up basic instructions. See the problem with generalizing now? It doesn't help anyone.

I don’t think there’s that many guys opposed to it. If there were something available that guaranteed it wouldn’t have any impact on regaining sterility, then I’d be for it.
The issue is that the testes atrophy if they aren’t producing sperm, and that atrophy can cause permanent sterilization. This happens in people on steroids or on testosterone. They have exogenous testosterone, so their body shuts down natural production, which also shuts off sperm production. In a lot of cases, these guys become permanently sterile or need months of treatment to regain some semblance of function. This is probably why researchers are focused on preventing sperm from being motile instead of reducing production altogether.

The goal is to go under the 1M sperm cells/threshold, which is considered by WHO to be 99% theoretically effective. Pearl index right now is 0.5 but again, on small scale studies, the best one having only 50 participants for 4 years if I recall correctly, which is still lots of pregnancy cycles successfully contracepted, you could also argue. In terms of number, only a survey interviewed 970 men using it and only 0.6% accounted for un unwanted pregnancy, only because they didn't follow the medical protocol correctly. There are a number of biases in this, and it's still a survey, but still it gives encouraging results and research about it should be pursued. Especially since lots of people are getting on this train every year. The silicon ring mannufacturer said he had its 20.000th client in early 2023. I would definitely take a pill myself. I had a girlfriend that was a really nice person but having sex with her was a bit stressful. She would refuse to let me wear a condom and she also can't take the pill because it causes her to become depressed. Like for real.

That's why men need more birth control options.
But they don't come because societal demand from men isn't there. We don't demand birth control, we're simply waiting for it. Women didn't wait for it in the 60-70's, and god knows the birth control options back then were shit compared to the new generations of pills and IUDs for example.
But right now, more and more people are trying to tackle this problem by using heat. Heat based method is efficient, reliable, little side effects and a very high chance or reversal.
It's not perfect, but at the same time waiting for a perfect mal birth control is what's been holding us the last 50 years, so maybe we should quit expecting the fantasy to become true and actually join women in the trenches.

The only reason that there isn't a pill for men yet is because they haven't found an effective option yet. The female pill if used correctly is 97% effective and mimics natural cycles that the female body already experiences. For men their is no natural cycle to take advantage of. From puberty till death the barracks keep producing commandos on the order of 200 million sperm per day. That is roughly 140,000 every 60 seconds. Compare that to women producing 1 egg per 28 days and still female pills are only 97% effective. If we assume that it takes 5 minutes from the time a man salutes until he releases the troops, he has already produced an additional 700,000 solders in that time and if the male pill is only 97% effective like the female version that is still 20,000 viable sperm snorkeling their way to the target. It only takes 1 sperm to make it to the egg. Or in other words there are legitimate reasons that science hasn't found a reliable male pill yet. It isn't some kind of conspiracy targeting women.
As a pharmacist I can state that there are drugs approved by the FDA on a regular basis solely for social and political reasons. The covid 19 vaccinations are a perfect example because they were not required to go through the usual review steps and we will not know for years longer if they are safe or not. Just to be clear I am not against the process used to bring them to market just stating that we don't have all the data yet and will not have it for years to come.

As far as I know, clinical trials for drugs are designed to weight the downsides of using the drug against not using it. In the case of a person who can get pregnant, it's weighted against the complications of a pregnancy. That's why the hormonal birth control we have available now has do many side effects and it's still considered safe, it's way safer than having a pregnancy.
However, since a person who would use a contraceptive method designed for men wouldn't get pregnant, they wouldn't have the preexisting risks of a pregnancy. In their case a medication that has side effects similar to the ones we have right now wouldn't be considered safe. It would be more "dangerous" to use the contraceptive method than to not use it.

The main reason why male contraceptives are not all that easy to make is not really touched here, and it is that to shut down the male reproductive system enough to stop fertility requires an unbelievable amount of hormones.
Female contraceptives work on tricking their bodies into thinking they are pregnant and thus the body does not need to produce the required hormones to allow you to be pregnant. In males it's the same, you need to introduce endogenous hormones to reduce the body's production of those hormones. The issue is is that males make a lot of these hormones. Like...a lot.
Professional male body builders often struggle with fertility issues, but only at the peak of steroid abuse. Those are the levels of hormones you have to introduce to suppress the male reproductive system.
So unless you want everyone to be hyper aggressive, die early from heart attacks, and look like the Hulk, the old method of overloading the body with hormones so that it doesn't produce them won't work for males.
Of course there are other options but that's why the classic male equivilant to the pill does not exist.

So much so that despite some contraceptive pills having similar side effects as the female birth control pill, they don't pass, because the female birth control pill wouldn't pass modern standards. and the only reason it hasn't been taken off the market with a push to reengineer it is because that would be a political nightmare.

Controlling the birthrate of any population (humans, animals, whatever) is significantly more effective when the methods are applied to the females of the population, not the males. Male contraceptives won't actually have a significant impact, particularly the more partners a woman has. Only one male not using contraceptives is all it would take, while the woman can be pretty sure she won't get pregnant if she uses contraceptives herself

If I remember right, female hormonal contraceptives are so common because female fertility works sorta like a hormonal clockwork. So it's a bit easier to mess with that. Just make it so that the hormone levels look like a different part of the cycle and fool your body into thinking it's not baby time yet (ofc, that has side effects).
With the male contraceptives, it was a bit harder to make it because it's super hard to block the mechanisms there and such, and it has harder side effects because you'd need to shoot down the levels of testosterone so much the guys would end up feeling weak. I don't think the guys were too happy at feeling like a grandpa due to the low testosterone.

The big issue, legally, with all male birth control, is that most drug approval processes require the drug to have RELATIVELY reasonable risk. Pregnancy is incredibly dangerous (0.2% mortality rate worldwide), so any female birth control just has to be less risky than pregnancy. Someone else getting pregnant, however, has no risk to the patient, so the risk of side effects is being compared relative to perfect health. If a male birth control pill has a 0.1% chance of giving you mild acne, it has a too high relative risk to be considered reasonable.

Considering what women go through, the aim should be at least _somewhat_ lesser side-effects than what they are going through. If they succeed at that, then there's potential for everyone to win. The emphasis will finally be off women when it comes to birth control AND they can perhaps finally stop taking those horrible meds too. And men will be _far_ more willing to take the birth control pill if it has fewer or less severe side-effects than the female version - otherwise it's going to be a huge struggle convincing men to give the pill a try. The main issue is that all drugs that actually work have side effects, and the FDA's approval process always weighs the risk of the drug vs. the risk of NOT taking the drug. For females, this means that the risks of birth control are weighed against the risks of pregnancy. Since pregnancy is very high risk, even severe side effects can get approved. But for males the risk of not using birth control is essentially non existent medically speaking, so approving anything for it never manages to pass the FDA's requirements.
Obviously a lot of males don't agree with the way the FDA is handling this, but this is in fact the reason. It's not just that some dudes in the clinical trials were babies about it, it's that ANY side-effect at all is likely to be too much for the FDA to accept when the alternative is side-effect free.
What we would probably like the FDA to do, is to look at the effects of a dug on public health in a more wholistic way. The drug may have side effects for users, but if those users choose to accept those risks it could lead to better outcomes for not just the users but for others. Furthermore, the risk of pregnancy may not directly effect the health of a male - but it can still impact their life in a lot of ways that can also effect their health. There's also the aspect of choice: Many things that the FDA doesn't approve of (like weed) are still desired by many people, and it's questionable whether or not the FDA should really be able to ban something that so many people want and would choose even knowing the side effects. It may be better if the FDA simple had other classifications for substances that it considers dangerous, but that provide a significant utility and could be used by an informed adult.
