Basically the story behind this song is that the parents of the children weren't around and it was only the big sister and the little brother that had a mental disease. For obvious reasons the sister couldn't pay for his medicine so she decided to sleep with men for money, ending up dead by one of her clients while the brother was watching through the keyhole of the bedroom.
yeah and the pupe (doll) could be a reference to an embryo from an unwanted pregnancy from the sis and a client..

This song is on their best, specially when you hear Lindemann's insanity breaking through in the song, this along what happened to his sister make this fucking mighty.
I am relieved, I thought that Puppe means puppy, not a doll. So i thought the song was more brutal
