How do you get this scene? Do you have to do this with Astarion only in the camp/party? I had Laz'el and Shadow Heart also in my camp and they always are there for the scene.

Yeah, you need to have only Astarion in the camp

Yes, if someone else is in the camp so it will be another dialogue with them
