  • Welcome to another episode of The Meat Medic Podcast. Today, I share a candid review of my 12-month journey with the carnivore diet. As a doctor who embarked on this diet to address chronic health issues, I found significant benefits but also faced unexpected challenges.
In the early stages, the diet resolved issues like joint pain, chest pain, and IBS, leading to a remarkable improvement in my overall health. However, around the six-month mark, the positive effects plateaued and new concerns arose, including nutritional deficiencies and a concerning increase in insulin resistance and chronic stress response.
This episode delves into the complexities of dieting, the importance of listening to your body, and the realisation that a one-size-fits-all approach doesn't apply to nutrition. I experimented with reintroducing certain foods, which led to insightful discoveries about my health and the impact of oxalates and carbohydrates.
I'll also discuss the implications of my blood test results throughout this journey and introduce the concept of the primal fusion diet, a balanced approach I've devised based on my experiences and patient data.
Whether you're a carnivore diet enthusiast, a skeptic, or somewhere in between, this episode offers a nuanced perspective on dietary choices and their long-term effects on health. Join me as I explore the lessons learned, the changes I've made, and why I believe in a more flexible dietary strategy moving forward.
For more insights and to join the discussion, visit primalfusiondiet.com.
Remember, these are my personal experiences and results. Your journey may differ. Always consult with a healthcare professional before making significant dietary changes.

I've spotted a potential problem with your theory regarding "long term ketosis" being an issue on the carnivore diet. Firstly, if you are not eating for weight loss and are in fact eating for maintenance or muscle building then the carnivore diet is NOT actually ketogenic, you should be eating enough protein that gluconeogenesis occurs and triggers an insulin response. The other is that everyone (on any diet) produces ketones whenever they fast, including every night, so if literally every human being produces ketones every night then I'm not convinced that being in ketosis a few hours longer per day is going to be massively detrimental.
Even if it is insulin that is helping, you don't need exogenous glucose to trigger insulin, you can do that by condensing 2 meals in to 1 and making sure you are eating enough meat in one sitting to get a decent insulin response. Have you tried a CGM?
I am still trying to lose some belly fat but I am also trying to build muscle, so I follow a bulk and cut pattern to my eating (and my cut usually involves a 48 hour fast once a month).
How many calories do you eat on carnivore? I'm 69kg and I find I need 3500-4000 calories a day to be gaining about 1kg per month and then in 48 fast hours I lose 2-3kg and go again.
Another consideration could be are you eating grass fed beef? I would also check vitamin D level.
The problem with eating fruit and veg now is that what exists in supermarkets now didn't exist a few hundred years ago let alone 1million plus years ago.

You were activating the randle cycle as would be expected. Your insulin levels increased, and triglyceride levels were nearly a one to one ratio with your hdl. Not good at all. You cannot mix high carb with high fat. If you want to eat that many carbs you must go very low fat. Prof Bart Kay should have been consulted. I bet he would have sorted things out for you on that front. I am strict carnivore for at least 4 years now. Dont have any of those issues. I do include dairy on occasions. Talking about a thick slice of brie or camembert 2 to 3 times a week. I consume sardines and mackerel as well. Remember some carnivore foods themselves can cause issues like eggs and dairy. Allergies and food intolerances do exist even on the carnivore diet. Its just a matter of balancing things out. Humans are you know were not designed to consume carbs at all. This is why they are not essential. You know this aleady. Did you eat more organs. They contain lots more b group vitamins which are important. Did you consume some of your meat raw? All these things do make a differences too many on this diet. Electrolytes as well play a huge factor to how one feels throughout the day, especially during summer or on hotter days.

I'm definitely happiest and healthiest when not consuming "carbs" - I do a little dairy but usually its like 10-20g a day of carbs, very rarely do I hit 40g, so I guess I'm more ketovore than strict carnivore

And you are absolutely right about strict carnivore being mostly an elimination diet for most people. It did give me the ability to quiet the noise in my body and brain and for the first time be able to listen to my body. But in the long run, I agree that some degree of flexibility with adding some whole food versions of non-toxic or low-tocix carbs is important and needed (at least from time to time).

I'm about 90% carnivore, Dr Sean Omara recommends (real nonpasteurised) saurkraut or ferments in general and they seem to settle with me ok. I came to carnivore from keto and worked out then that oxalates were messing me up so I'm oxalate free on carnivore. I like pate so I do eat some liver but not massive amounts, I also eat 4-6 eggs per day. I do also eat a mix of meats, not just beef and bacon, I also eat fish and chicken probably like once a week each. I also use quite a bit of heavy dairy (cheese, cream, sour cream), but my total carbs is typically less than 40g per day and usually less than 20g. 6 months on carnivore and I've not had any big dips in testosterone yet.
For cholesterol, look at Dave Feldman's LMHR study - they've found that just having low body mass when doing a keto type diet raises cholesterol, but those people also have CAC of 0 and match with "healthy" low cholesterol population in terms of atherosclerosis, so high cholesterol with a healhy body mass is "normal" for the human body (basically the theory being that the body is using fat as energy and needs a transport mechanism for that, its just an empty transport that needs to tidied up by the liver, its not atherosclerotic).
If you've been adding carbs then you will need more vitamin C.
The other thing to consider is that humans "in the wild" would sometimes have an abundance of food, would sometimes (but not daily) have access to carbohydrate sources and sometimes would have no food at all, so personally I think sometimes just eating meat is good, sometimes having a little fruit or veg is ok and sometimes fasting is good. Doing any one of these things every day is probably not optimal long term. Whatever you do I think cycling is going to be important, and not trying to think that if you do the exact same thing every day is going to be optimal.

Out in the wild we would also eat fermented stomach contents of our prey as well as partially fermented meat from carrion or burried meat. Hence our love for fermented foods and that's also why we cook our food, as it denatures it similarlly to ripening while being buried.

However I also eat a few cherry tomatoes, some courgette and the odd piece of fruit. I look for low oxalate. And not every day.
I think that cutting out processed seed oils, all grains, esp wheat, all processed foods and all sugary drinks would be an excellent baseline for someone to start from.
That eliminates everything in packets, all fast food, pub food, bakery foods, alcohol and sweetened beverages - these are the main culprits causing illness in people these days. Eating tomatoes, olive oil, fresh salad might not be optimal but it's a darn sight better than pizza and McDonalds!! I've heard it said it's not so much what you do eat but what you don't eat that makes the difference, and wheat, sugar etc are poisons - even more so when they are genetically modified and combined with chemical concoctions the human body reacts to as toxic waste, because it is!

Periods of high fat meat alone, periods of lean meats and berries, periods of more olives and avocados, some dairy, fatty fish, etc. If someone is metabolically healthy and can do this switching intuitively, perhaps seasonally as well as intuitively, you can get the best of all worlds.

Carnivore is not about ketosis. In fact that is not the objective. Did you eat all your protein in one sitting. The reason is because we will get a small spike in blood glucose when we do this. This is very important. The fruit that existed millions of years ago did not have the fructose levels that fruit we consume has today. Fructose will never show up in any hb1ac test. Just remember that. Also remember genetics is a factor. Again your ancestory plays a role in what you are best suited on a carnivore diet.

Have you ever thought about what else comes in fruits and milk? Potassium. We tend to over salt throwing off the sodium-potassium balance. Many people tend to have this imbalance and can trigger the symptoms you are describing. For my clients I recommend them to back off the salt and temporarily supplement potassium. Issues usually resolve. Classic carnivores like Amber O'Hearn speak about this and why she and others like her do not salt their meals. Just something to ponder on

I've come to the conclusion that anything that we do that is restrictive and repetitive will show problems over time. It doesn't matter if it's the same exercise routine over and over (jogging or at the gym), sitting all day typing at a keyboard (carpel tunnel), what we eat (carnivore, vegan, fruitarian), when we eat (OMAD or 10 meals a day), or how much we eat (gluttony or consistent calorie restriction), our bodies will adapt and initial benefits will decline and/or problems will develop over time. Chaos seems to be our friend. Things like: Random eating schedules often including periods of random duration without food; Seasonal changes in diet as we would have experienced in the wild; Random physical exercise as we would have experienced hunting and gathering food: walking, running, dragging and carrying things, etc. Constant change in both diet and physical activity, as long as it fits within the framework of our evolution, seems to provide the best outcomes for humans over the long haul. It's trying to create or adhere to a single unvarying protocol that violates our evolutionary heritage.

We do add in veggies and I'm glad to have the low oxalate information. We both lost 50 pounds and my husband felt better than he had in years (we're 72 now). But, as happened with you, that did not last.

  • @stanleysheppard8464

I was on a strict carnivore diet for about 6 months. I felt great on it at about 3 months mark, when I finally got fat adapted and no longer had daily diarrhea. But slowly but surely after that peak I started to feel that my energy levels started to go down. This was really the biggest issue for me when after my first meal of the day at noon I'd feel tired and needed a nap. I started doing more research on that and it turned out I was not the only one who had issues with strict carnivore diet after a few months on it.
At about 6 months I decided to introduce some carbs, less than 50 grams per day. Now in addition to animal products I eat garlic, ginger, some lemons, some onions, parsley and cilantro, one medium size apple per day and a tomato or two depending on size, plus shiitake and oyster mushrooms After making these changes I do feel more balanced and more energetic. After initial period of diarrhea followed by a period of constipation on strict carnivore, now I have regular bowel movements daily. Also since on strict carnivore diet my stomach and esophagus got actually healed, after decades of taking antacids and PPIs, I feel now that I get much more out of plants that I eat as now I digest them completely. It may sound to many here counterintuitive, but lower stomach acid was causing a lot of issues for me, from GERD to indigestion and gastritis, and with carnivore elimination diet my stomach acidity level did increase significantly and that allowed body to extract all the nutrients out of food I consume, plus it forced esophageal sphincter to work properly that stopped acid reflux.
To summarize, strict carnivore diet is not for everyone. Though I always considered myself to be a big meat eater and actually enjoy it, for me it turned out to be not an entirely balanced diet for the optimal well-being. Some veggies do make me feel better. I didn't do "big starches" yet, like potatoes and rice (love them both too), not sure yet if I'll be coming back to it since feeling good and healthy beats the pleasure of eating heavier carbs. After introducing some veggies back, my weight went up at first by a few pounds but now it is back to what I had on strict carnivore and stays steady there. Another good video on this subject is by Rob Stuart titled "Is carnivore diet dead?". There too, he went from a complete carnivore to a diet with some fruits and veggies for a positive change in his health and wellness.

Excellent video. I am against all dogma and there is a lot of things we need to learn about physiology in different metabolic states.
In my opinion there are genetic differences between people and one way of eating may be great for some but not others. Some people thrive on meat only and some not.
It would be mad not to adjust nutrition and not to experiment if you start feeling worse.
I think people and other animals used to cycle - you could not get berries in the winter...I think some times that keeping too strictly to one way of eating may not bealthy in long term - unless this is what one's genes want.
Myself - as carb addict I try to be pure carnivore but once in a while fell off the wagon so I "carb up" . And still have great results.

I tried carnivore and could feel the shock my body went into so I switched to keto but I didn't have the knowledge I do now and I was finding myself in a calorie deficit far too often as well, my electrolytes were all out of whack. Through all of that I kept researching/learning and finally decided to just eat as responsible as I could. I eat generally healthy. Nutrient dense type foods like grass only pasture raised beef, pasture raised pork and chicken. I also eat lots of eggs (pasture raised) and I take things like bee pollen, unfortified nutritional yeast, Chlorella and sea moss to bump up my vitamins and minerals. I drink plenty of coconut water to balance my ekectrolytes. I eat unpasteurized honey, organic fruits and veggies, make my own bone broth and render my own fat.
And I drink reverse osmosis water so as to avoid all the poison in my tap water.
We just need to do what we can.
It won't be perfect, but if we put in the effort, the benefits will be worth it.

I feel like this is a common problem. People like to talk alot about what kind of meat they have and like "all I ate is ribeyyye" but nobody actually bothers to sit down and talk about the actual fat content in their food. Your ribeye might _start_ and "probably 20:20" fat-protein but after cooking and oxidizing you might have less than 5-10%. The only people who really fall out of the category are the people who gorge on tallow, and butter, and cream, almost entirely on its own. Fasting in general, people seem have had success with, even on carnivore. Which is interesting.
To be honest I strongly suspect humans are probably meant to eat entirely liquid meals some days. There are numerous references to various soups and broths where half the time the non-working or less sick would basically just get the meat broth and the hunters and workers get the actual meat... And that was enough to sustain you. Sometimes depending on the meat they would cook it down and just throw away the meat, or give it to the dogs. Particularly with organs

I get so jealous when I read the testimonies of ppl who are healing and enjoying great energy and great sleep while on carnivore. But I have noticed that on the days I have made my special fatty concoction (consisting of 250grams of melted butter mixed with 250ml of cream) I have felt great and satiated; not needing to eat anything until the next day. Stephanie keto person discusses this a lot on her channel. She says that the point of a carnivore diet is to get into ketosis and if that's not why u're doing it then forget about it. I will try the 3 day sardine fast and afterwards I will stick to more of a ketovore plan with plenty of fat from butter, cream, cheese and egg yolks. Eggs make me puke when I eat them but I've been told to just try the yolks on their own and that shouldn't give me any issues.

Wondering whether the volatility allows as much signal to be gleaned from the noise. My (non-medical) guess is that if you were to do blood tests over a week you would see at least as much volatility in many (most) of these figures. Something I do wonder about when it comes to blood work in general.

I guarantee that most of hyper carnivores like myself watched this with an open mind. But I must say, why is it that the FIRST thing people do when they have issues on carnivore is run back to carbohydrates? There are zero other reasons you may have experienced issues, aye?!

I am about 90 percent carnivore. I take loads of vitamins along with my diet. I have no aches and pains. What I do plan to do is eat more often. Every few hours I will eat a hard boiled egg and I still eat apples. Moderation. I avoid flour, sugar and I just eat wholefoods.

6 and half years carnivore at 53 years old. My libido is still insane but I do body weight training which I think helps. Fruit just wrecked my digestion especially at night l, did not give me more energy. I find that the more active I am, the more energy I have. I have not done blood work for years.

This is why I don't eat the Lion Diet. I eat The Wolf Diet. Wolves eat some berries now and then but nobody is revoking their carnivore certification. There is a big difference however between a handful of berries now and then and going face first into a huge bowl of tropical fruit and honey as Paul does.
When men have a partner who has recently given birth, their T levels go down regardless of their diet. That's just nature's way of spacing the kids out a bit. Because you had fixed your T on carnivore, I think this natural post baby dip may have been more noticeable for you. I was trying to figure out, in the chronology of your carnivore journey, when the newest member of your family came along.

Out in the wild we would also eat fermented stomach contents of our prey as well as partially fermented meat from carrion or burried meat. Hence our love for fermented foods and that's also why we cook our food, as it denatures it similarly to ripening while being buried.

Question? How many years of bad diet before going carnivore? And you really believe all your health issues will fix itself in a year? Yes, I've noticed ups and downs as well during my carnivore journey, but every time after a low, I started feeling better days or months later. I believe that what is happening is that the body repairs itself and that it needs to store energy and elements before the next cycle. I compare it to a child growing up - it happens in spurts.
Also, where do you live? If you're not anywhere near a sea I'd advise looking at iodine (I didn't see you list a thyroid panel or talk about eating fish)

Appreciate the open minded approach. I’m partial to carnivore + various fermented foods sprinkled in throughout the year.
Seems to me to be the closest ancestral foods for most, especially considering all of our modern plant foods are alien to what was eaten then.

I eat ketovore most days and keto once in awhile. I cut out almost all oxalate foods. My labs are almost all excellent. The ones I personally struggle with are low ferritin, high LDL and high homocysteine. I do have MTHFR. I’ve fixed ferritin with boron and copper. Fixed homocysteine with tmg and folate. Now working on lowering LDL a little without side effects (joint pain, IBS) from carbs. I sleep nonstop for 6-7hrs and feel great!

I wonder if most people starting out on "strict" carnivore are actually consuming enough fat. It is easy to overlook this while thinking a fatty rib-eye or whatever is providing the right metrics. Hence the term 'high-fat'.....we need upwards of 200g per.day, which is actually way more than anyone coming from a carb-centric diet could possible conceive of.

My thoughts also. A little bit of denial. People who are used to being 'strong' and 'tough' and 'pushing through' have a hard time recognising stressors sometimes.
