Idea based off of your warped humanoid idea: the uvuudaum doesn't see itself as being hostile to the material realm. It got stuck in the Far Realm, learned of how the Far Realm can corrupt other planes, and came back to warn everyone. However, it was too warped already, and it's warning accidentally contained the eldritch corrupting truths that it was trying to avoid in the first place.

I read a sci-fi short story once about a space voyage to a distant planet where the sheer boredom started making the crew suffer from time dilation effects, which might work well with your idea. Just have the afflicted party blink and all of a sudden, they're in the middle of a fight, you just say "It's round five and you've already taken 15 hp damage." Or, heck, even give them extra random turns in combat, the fact that you're not telling them _why_ that's happening will drive them mad.

A fun detail from the Epic Level Handbook for the Uvuudaum:
Epic Level monsters also have access to epic level feats. One of these feats is Spell Stowaway. This feat allows the feat-holder to be affected by a certain spell if it was cast by someone within 300ft. The idea being that if a wizard cast Haste on themself and someone with Spell Stowaway for the Haste spell was within 300ft, then the feat-holder would also gain the effect of the Haste spell.
So picture a wizard, seeing their friends driven mad by this creature's aura of confusion. The ranger just got launched into space. The barbarian has taken so much Wisdom drain they might become an NPC by next round. The cleric is blocked off by a Wall of Force. The wizard knows time is running out and casts Time Stop to try and cast some buffs.
The Uvuudaum has Spell Stowaway for Time Stop.
    Time cannot protect you.
    Embrace the madness.

Given my experience with this monster, your explanation of them being former humans is really oddly on point. Me personally, I see the clothes and the Uvuudaum really reminds me of Nyartholotep in how it is shaped, one of the most "human" figures in the Lovecraft mythos.
What did I mean by "oddly" on point? This is the most memorable monster in the game for me. The second to last major encounter in a 7 year campaign happened like this: We were betrayed, surrounded, overwhelmed, and our keep burned. Our barbarian pulled out a flask of something we got in someplace like Carceri, a flask of "Blood of the Ancient Gods," and drank it, transforming instantly into an Uvuudaum (who our barbarian player got to play). It dominated our ranger, and smoked all the enemies with its aura of confusion while we escaped. A party of 3 17th level characters now, we didnt think we had a chance except that one of those party members was a wizard. We called in a favor with an epic level wizard mentor, who cast some powerful ritual spells while we tried frantically to lock down the area of the approaching monster. We ended up literally glassing an area to get rid of the thing after we trapped it, breathing a big sigh of relief, stopping the far realm incursion and getting our full party back.

Not what they wound up going with (I built an undead ship mech partially constructed from the body of an ancient white dragon), but I still was really rooting for the "launch Hoondarrh The Red Rage of Mintarn into fucking orbit" strategy.

The point about building up to an encounter and the cold damage turning off its regeneration made me think you could do some The Thing scenarios with it. The party finding itself in the laboratory of some ancient wizard or the ruins of a citadel that once belonged to an order that rooted out cults. There would be clues around and then a sample of flesh kept extremely cold or encased in ice.
If the party just defrosts it without having investigated the area first then good luck to them as it begins to regenerate again.

I really love the concept of a monster from a plane of "madness" and chaos that just wants to get home again. It isn't even necessarily malicious, even if it's causing untold horrors and destruction. It's just breaking things in desperation to escape a plane that feels stifling and constricting. Possibly even painful.
I've toyed with the idea a little in one of my original stories and I really have to do something more involved with that sometime...
For a TTRPG group, they could even end up helping the thing, figuring that if they send it home that might be the faster and easier way to stop it from breaking everything in its attempts to get out on its own.

After a short calculation, "Nailed to the Sky" teleports it's target to 74 000 feet above. As it's outer space in most case, you cannot move in a normal way, but in an Elemental Plane it's just a distance (if you are in the Elemental Plane of Water, and deep enough, you doesn't need to fly to go back, it's enough if you can swim), But anyway, you would have more than enough time even if you would be falling, to cast a teleport spell. Except, in that case your momentum would be kept after the teleport, so you would likely die from the moment you catch up with the ground...

The far realm is crazy and would be really hard to portray in any setting. One of the descriptions of the far realm that I once read (I think the 3rd edition planar handbook) stated the far realm was made up of innumerable layers. Each layer had its own rules, physics, ecosystems and so forth. Yet the distance between layers may only be inches or feet apart. It was possible to take a single step and cross between three or four layers. And since it was impossible to determine the properties of one layer from the next. Simply walking could tear you apart and put you back together in weird ways. Their is literally no way to map or figure out how the far realm worked because it keeps changing how it works with each step you take.

this is just a concept idea i had while learning about the uuvudam, but one of these aforementioned creatures manages to break the barrier between the far and material realms but with everything there's a cost. In fumbling it's way into the material plane the uuvudam lost most if not all of it's reality breaking spell casting abilities which manifest themselves as shards of madness that warp anything and everything around them, they can even find there way into people. The goal of this uuvudam is to reclaim all it's power, obviously sewing chaos along the way and eventually turning the planet into it's own pocket realm of madness. one of these shards can find there way into a player character or an npc allowing them to learn great magics but at the cost of their sanity and the risk of turning into a lumbering pile of flesh, the shards also have a compulsion to be drawn near one another so if someone has it within them they will desire almost to a maddening degree the other shard no matter where they might be. sorry for the crap grammar i didn't feel like going back and correcting all this.

I like this, and am currently attempting to make an Eldritch Fae/Squid type thing that makes an "Avatar" to walk the worlds of Mortals so as not to accidentally drive the mortals Mad, and twist them. The idea is a spawn of sorts ended up growing a semblance of a "Soul" and a fondness for the fragile mortals of the worlds and actually thought to protect them from the encroaching Madness, and thus spawned more of themselves to help keep open portals closed and stuff. They also do not allow themselves into the Mortal World properly because their mere presence, like this Uvuudaum, warps minds and physically twists their environments. Most of them have similarities, but do not necessarily look like themselves, and their "Avatars" are quite diverse, and usually half warped and twisted, forcing themselves to have to cover the warped parts in one way or another... Like a mask over a face of endless teeth or eyeballs, for one example.
Have little idea how to do the stats, but with this I may be able to get a good starting point. Physically they have many chimeric traits and can appear squid/octo-like, with maybe shells, some might have spider legs, are more serpentine, or whatever mad mixture of things you could think of. The two main ones I have are like a Nautilus with a thick, bony Draconic-like skull, and Cuttlefish tendrils/fins. Her Mother is more of a ball of tendrils with Spider legs and too many eyes, and their "Avatars" have no eyes (But can see fine anyways)... It's all still a work in progress. They're my "Sweet little Nightmares", and when pushed... can be all kinds of maddening nightmare material in combat if they have to be. Otherwise they are somewhat open to have all kinds of abilities and appearances to fit a campaign as needed.

I think, canonically, the uvuudam was first created when the ancient elves, described in the manual of the planes, breached the boundaries between the great wheel and the far realm. Most of the elves were transformed into the first Kaorti, but the archmage leader of the cabal was transformed into something far, far stranger. This is why it is depicted in the manual of planes artwork, even though the stats do not appear and they are not mentioned specifically, for some reason they didn't plug the epic level handbook in their other splats nearly as much as they did with their other splats, because a 2sentance explanation would have saved me a lot of headache trying to figure out what the hell that thing was back in the day. I never had the epic book until I was much older and got it as a PDF lol
