Hyperglycemic Hyperosmolar Nonketotic Syndrome HHS | HHNS Nursing & Pathophysiology

  • Hyperglycemic hyperosmolar nonketotic syndrome (HHNS or HHS) nursing diabetes & pathophysiology NCLEX review on endocrine disorders of the body. HHNS vs DKA are two complications of diabetes mellitus. HHNS presents with extreme hyperglycemia (blood glucose greater than 600 mg/dL) and dehyrdation. Ketones and acidosis are not present in this condition as with diabetic ketoacidosis. The hyperglycemia hyperosmolar state can lead to a coma and even death if not treated promptly. HHNS treatment includes intravenous fluids, insulin therapy,and electrolyte replacement. This video will highlight the patho, causes, signs and symptoms, and nursing interventions for HHS.     1:25 HHS > 600
    2:32 Body resistant to glucose
    2:54 Hyperosmolarity --> pulls glucose and water out of cells to the blood
    3:35 Kidney has too much glucose to reabsorb so it leaks to urine.
    4:12 Na, K, Cl is excreted more along with water = Dehydration.
    5:46 HHS happens gradually
    7:10 Mental status change (coma, seizures )
    8:55 Administer Insulin drip (only Regular)
    Insulin causes Potassium to move into the cell , so make sure that Potassium is above 3.5

    Thanks for the helpful information - I'm a recently diagnosed (2 months ago) Type 2, and I've been soaking up as much info as I can find. My numbers are back in very normal range after a complete reversal of my bad diet. I was dx'ed with glucose at 398 (but didn't feel bad, just had polyuria and slightly blurry vision), and A1c at 10.0%.
    I found your DKA video first, since I *have* heard of that, and was interested to find out more about it (I have a really good friend who's a Type 1.) You mentioned HHNS in that video, which brought me here. It's good to know about this - If I ever get polyuria again, I'll know to at *least* get it checked out asap.
    I didn't see blurry vision mentioned anywhere in these videos - is that not a common symptom of DKA or HHNS?

Polyuria is a big thing with it, and yes blurry vision can be a symptom....however, I tried to hit on the major symptoms (because there are a lot of them) nursing students must know for NCLEX. It is very awesome that you are educating yourself on these conditions because that is the key in preventing them. Thanks again for taking the time to watch them and for your kind words!

    Hi Sarah,
    If HHNS is a complication of Type II (i.e. have insulin resistance) why would you administer IV insulin in HHNS patient's where their tissues do not recognize / be able to utilize the insulin?

      Great question! The main reason HHNS is so severe on the body is because of dehydration. So, really when you correct the dehydration by giving fluids this will cause the body to respond more appropriately to the insulin you will give them....because remember there is an extremely HIGH glucose level in HHNS. Therefore, insulin isn't the only thing that will help this condition but fluids will as well....it will all work together. The insulin will help take the glucose into the cell to be used. I hope that helps :)
