maybe its how the doc call her every time he talk about her, maybe he just repeated what the doc said, maybe the writters were drunk who knows lol

If he did it was only brieifly, I get the impression he became more involved with Shaun after she died
Glassman literally shouts 'Maddy please' and shaun stops and turns around. Maybe that's why Shaun said Maddy too? 🤷♀️

she played angels really well. she also played one in Manifest

She OD'd. I don't recall if they ever actually say what she was using, but she died from overdose.
