Nothing about it is supposed to be realistic, however the main idea of it has a lot to do with real life. Many of the blocks are from real life. It’s a combination of both realism and fantasy. We’re talking about ‘SURVIVAL’ Mode here.

And you still got somewhat lucky, you have no idea how many times I've been left with 1-2 hearts and start running only to be no scoped by a skeleton that just spawned in the dark…
Something like this happened to me, I was returning to my home from a mining session (my home was over 1000 blocks away and I didn't have a respawn point sent), and I accidentally fell down an extremely deep hole and it took 7 hearts away and I was so scared that I accidently slipped again and I accidently lost 2.5 hearts. Thankfully it wasn't night time, but it was so unexpected I immediately ate bread to recover and my heart was pounding.
