thank you for the videos.... when you do the pre endo build up.... why do u seal the pulp exposure with composite , before proximal wall build up?? is it to prevent blockage of access ?? do u need to fill the whole cavity or just building the proximal wall will suffice??? what if there is bleeding or pus discharge from the from the pulp chamber, how then would you manage the pre endo build up
Thank you for watching the video. Pulp exposure was sealed with composite to prevent bonding agent or composite blocking the canal orifice. If there is bleeding, we can stop the bleeding with local haemostatic agents and temporarily seal with composite or even with temporary filling materials. after buldup, while doing access, we can remove this temporary material. Soon i will make a video on different ways of pre-endodontic buildups, different ways to block the orifice, buildup for grossly decayed teeth, canal projection technique and so on... Building only the proximal wall is adequate.

if we take a probe and try to move in modelling wax, it will get stuck right.... the same feeling will happen once the probe tip get caught in an orifice.
Using composite may look more precautious, but sir.... Pt has to afford RCT cost plus composite cost...!!! And often the RCT's r due to Class 2 caries!!!
Yes. Composites are expensive. But pre endodontic restorations are must. We can try the pre endodontic restorations with GIC or zinc polycarboxylate whenever feasible...
1.Needle burs can penetrate deep inside and cause perforation 2. Create irregularities in pulpal floor 3. Some dentist enlarge the orifice with such burs...which can lead to ledge formation I personally find, it can be used to punch a small opening before giving intra-pulpal anesthesia

Sir which is the best bur for beginners if not surgical round bur or needle bur?
I recommend TR 14 bur. It is a round end tapered fissure bur. Single bur is enough to prepare the access in most cases.
