We may have heard of dental problems. When we think about alignment for children and teenagers but what you need to be aware as parents or family is that certain problems are also skeletal in nature not merely dental. Now skeletal problems means that there is a mismatch in the growth of the jaws. So sometimes the upper jaw may be growing forward faster than the lower. Sometimes the lower jaw may be growing faster than the upper and sometimes there may be a deficiency in the width of the jaw. Now these problems need not just braces but they may need something called as Phase 1 Orthodontics. Phase 1 Orthodontics is initiated at at a younger age when the child is still growing and that allows us to use the flexibility and softness of the bones before growth is completed to our advantage. This allows us to do things like widen the jaw to fit the teeth better it allows us to create a stretch effect to improve the growth where it is lagging such as in appliances like the twin block and it also allows us to put a restraining effect on growth where the growth might be excessive this involves using things like a head gear or a chin cup. Let's talk about twin block in detail. A twin block appliance is basically an appliance that allows the lower jaw to catch up in growth with the upper. So commonly what is called as buck teeth where you feel the upper teeth sticking out is often caused by a lower jaw lagging in growth a twin block is a series of plates that the child can wear which allows us to create a stretch or a pull on the lower jaw and that creates a secondary lengthening effect to overall improve the face profile now what happens by doing phase one it reduces the severity of the mismatch that may remain when they are older. So the final definitive treatment would probably be done when the child is around 13 and has lost all their baby teeth. But doing phase one at a younger age between say 8 and 11 allows the secondary problem to be lesser.
