  • In this episode the surgeons discuss how long to do physical therapy after a total hip or total knee replacement. Generally, more physical therapy is required after a total knee replacement compared to a total hip replacement.

    “Let pain be your guide,” said the orthopedic surgeon to my frustrated teen basketball player son. So son’s reaction was to become a PT. He loves it, just wishes more folks would keep up with the exercises he uses with them. Thanks for this video - I’m going to send it to him and hopefully he’ll become one of your new subscribers.     Following what the surgeon and therapist says to do gives everyone the best chance for the best possible outcome. Doing so was one of the best choices I've made in my life. If you have a good surgeon and a good therapist, give them your trust. What they say to do may not always make sense, but give them your trust about those things. Another great video

    Thanks Doctors. I am 3 weeks away from my TKR-right side and am doing the exercises that therapy will want me to do after the surgery. I hope that these and some moderate treadmill work will ease my recovery!

    Omg. Priceless. Physical therapy is a must or your TKR or HR will be a bust. In my case , I did two zoom videos before I got in to see my P T in person. I luckily did the exercises from the book pre surgery and was pretty much on the mark by watching your videos on what exercises to do etc. After my 6 week mark I was given green light to ride my Harley. Then 5 days later I fell and split it open , that landed me 22 more staples and had to wait for them to come out before getting back into the swing again. Even with the set back, I was able to get my flexion and extension back to a fantastic degree and subsequently released from therapy. I continue to keep active, do exercises and happy with my progress at 4 months post op. My knee looks fantastic and so does my scar. Thanks for all the videos you have put out and I was so well prepared because of your knowledge and honesty about surgery, I no doubt would be scrambling for help had I not found your stuff yrs ago. Keep up the great content. And I am in charge of my own health. Cheers

    Thanks docs, for your time, skills and generosity. I'm almost 80 yrs old and no knee or hip problems. I'm very active, 50+ year vegetarian, recent vegan convert. Lots of fibre, so pretty good microbiome I guess. Anyhoo, a dearth of auto-immune problems. But I watch you docs every time because curiosity and learning are always good ideas.

    Another very informative video!
    I started having jaw surgeries in 1982 and had a total of 10 bilateral procedures in 11 years. After my first surgery, my jaws were not mobile. By the 3rd procedure, mobility was deemed necessary to prevent atrophy. The amount of scar tissue that forms after surgery is astounding. Then sometimes the next surgery is just to remove scar tissue. But the post operative treatment changed completely just a couple of years after the first surgery.
    In the past two are a half years I've had a fusion in my back and then neck. After each of these surgeries, I was transferred from the hospital to an inpatient rehabilitation hospital. I spent about a week in rehab each time. I've learned over the years that physical therapy is my best home away from home.
    Physical therapy is a crucial part of the healing process, especially after invasive procedures. Some people just don't get that. They think if they can get around okay that it's enough.
    But physical therapists treat you and teach you how to do the right movement. Like you keep telling us, we are in charge of our own health. Therefore, if we don't continue to apply what we learn in physical therapy, everything we did and learned will be lost. And thinking that you can go to physical therapy for 6 weeks and be done is a good way to become immobile.
    Physical therapy is one of the most important parts of the process because mobility is the desired end result.
    People shouldn't think they can do rehab without physical therapy. Medical procedures are always changing and advancing so the outcome is always improving. Therapy prescribed for a procedure done 5 years ago is likely better and more in tune today because of what's been learned in that 5 years.
    Nothing is written in stone because we're always learning better ways to treat the body.
    Physical therapists are some of the kindest and most passionate people I have ever met! They are well trained professionals who know the best treatment for each patient. And they know what to do to make your body to work as well as possible. And they should become your best friend because their objective is to make you better!

    100% PT is so needed for Total Knee Replacement! I not only went for 8 weeks but also did at home religiously. I also live in a house with steps. I continue to do some form of PT 4-5 x week. I have 2 friends who got TKR and didn’t do their home PT after they were done with going to PT and live in ranch homes. They both still have a hard time on stairs, their ROM is not great and still limp. JUST DO IT! if you go through the whole surgery it’s insane not to follow through. Yes, it’s hard! But not having the best knee you can I’ll eventually be harder!

    Hi Docs - I am so glad I found your channel. I have started PT to strengthen my knees (both) muscles to help me until I can get the surgery (Weight Problem). So I will be hopefully ready for the surgery and after surgery exercises. I can see that the more you do, the more rewards you reap. Thanks for sharing and continued success. Love PegEgg

    So true! I’m currently almost 7 months out from a TKR, and although I’m pretty much back to my normal activities - I find that if I stop doing my specific quad and hamstring strengthening exercises for several days (sometimes longer when life gets busy), my knee starts to stiffen and my ROM drops.
    If I go back to daily stretches and even just short exercise sessions, things loosen back up and I feel much better. Riding my stationary bike every couple of days is also a good way to get those muscles and tendons warmed up.

    I had open heart surgery to replace my aortic valve back in April. I started out with light yoga stretches after about 3 weeks at home, and recently I’m working out with a 7.5lb dumbbell doing Curls, Side Laterals, Bent Over Rows, Bench Presses, & Triceps Extensions. Then I do about 10 minutes on an exercise bicycle. I perform this circuit every 4 days. I still perform my yoga stretches at night every night before bed. Yoga loosens me up so that I get a good nights rest. I’m 61 years old and a former natural bodybuilder.

    Physical Therapy is a must have TKR. I also did it for a while after 2 hip replacements in same year. Abductor muscle just doesn’t want to get strong. Please ALL do what the docs say!

    Good guidelines
    They often reassess your progress and give you options to return.
    Unfortunately unless ancient, OHIP won't cover any of it. That changed before 2003.
    Some will provide a reduction in rates which keeps you improving.

    You guys are great. I had my 1st TKR on Dec 31, 21 and my 2nd on June 9,22. My physio is my job so that I can return to work. My life is returning to a functional level now and I'm enjoying being able to walk now haha Ice and physio are my 2 best friends.

    You docs always sound so upbeat. I totally agree with this advice. Four years ago I was staying in a really nice house right on the beach and the last day which was a beautiful vacation. But the last day leaving I had suitcases in both hands in the pouring rain walking down the stairs. Guess what happened? I slipped fell down four steps straight onto a pavement with both of my knees and only the suitcases blocked my arms. I swore I broke my legs or kneecaps. Upon arriving at the urgent care they took an x-ray and apparently I didn’t break any bones. But ligaments and muscles were involved and they ordered me to be on crutches.
    I really don’t like staying down so I’m walking all around with those crutches as much as possible people thought I was crazy. And let me tell you the pain was excruciating at first. Physical therapy and lots of exercise and alternating with cold packs. Having to ride around in the grocery store in a cart that I didn’t have a drivers license for and was constantly beeping at people because I really didn’t know how to control it. And I wanted to let people know please move because I might crash. Lol
    Anyways I didn’t know this was gonna be such a long story needless to say years of working on it and using different types of appointments both herbal and medicinal creams and then finally started using a vibe machine. Which strengthens all your muscles your bones and your balance. I am now about 99% don’t have any issues with my knees.
    I am also not responsible for any misspelled words because I am voice texting and I don’t really go back and do a bunch of editing. So I hope everybody understands what I’m saying.
    Suggestion can you guys maybe once in a while grab a church and or some sort of a model or skeleton or something and move it around pointing and explaining things that might be funny and entertaining and informative.

    I am 5 months post op TKR do my PT daily at home in pool and full weight bearing 3 times a day since working remotely. Bend bend bend had no issue with straightening but rom a bit more issues. Pool helps tremendously! Enjoy your videos been knowledgeable Thank you 😀

    I had my 2nd knee replaced end of April. Surgery pain was awful, worse then ever. Took me by surprise. But now I can ride a bike and walk normally. PT was and is the key to stay active as possible. No pain no gain…

    How it worked for me with both knees is, I started PT 6 weeks before the surgeries. I got to 120 degrees in both knees at the one week mark. The pre surgery PT I feel helped me with post surgery. One was 10 years ago the other 8 years ago, Since surgery I've done PT (weight training with them both) twice a week. I don't often miss. I still have the 120 degrees and for the most part my knee function is as close to normal as I could expect. Good topic.

    Broke my ankle almost 6 months ago. Had metal plate and screws added. After cast removal, went to Physio for 2 months. Still doing Physio at home. Still hurts like HE double hockey sticks, but it feels better and has more flexibility after Physio. I guess I’m in it for life!?!

    I attended physio for a good 10 months after my TKR. I also had to have a MUA because despite physio 2-3 times/week and being diligent with my home exercises, my knee bend seemed to hit a wall. After the MUA, I still continued with physio and home exercises as well as personal training (which I did for a good 1-2 years prior to surgery) and Aquafit 4-5 times a week. Everything combined helped me finally achieve a suitable knee bend (I had no problem with extension)! It is most important to remain active.

    Thanks even for a partial total knee replacement went to my usual p.t. person that I have been seeing over 20 years for my knees. One day she told me that I did better than her high school patient for an acl in less than a month besides doing my home exercises too. Which in a couple years know will be a brand new replacement for me. Wish I can have it now but still to young for it have to wait till 50 years old. I'm 43 years old

    Hi Docs - I’ve done PT for a new hip & for a messed up elbow and outcome has been great. My husband is coming home from the hospital tmw after a mitrial valve repair and also a pacemaker. I’m interested in the kind of PT he may be receiving? And also want to keep him motivated !
    Whatever it is will be focused on cardiological recovery of course.

    After my total knee replacement I went to a therapist twice a week for 2 months and had "homework".
    We did machines with the therapist and simple chair and standing exercises at home.

    I had my TKN on 10th March. And kept on having knock back in progress. 9 weeks back my physiotherapist returned me to my GP as I was back on crutches again and I must have looked bad. GP arranged an appointment at my local hospital Urgent treatment center UTC. Where I was admitted in to hospital. Consultants washed out my knee and carried out an abstraction. They thought
    i had an infection in my TKR. The microbiologist did not find any thing. But my CRP infection level was 140 (should be between 0-9) I was given IV antibiotics. for 12 days then oral antibiotics for 6 weeks. In this time my CRP went up and down multiple times. Last week I had my discharge appointment with orthopaedics. I was on crutches again. Previous week I had walked a mile each day.
    Monday morning I had great difficulty bending my hips and knees. The appointment was following tuesday.
    Speaking with the Orthopedic consultant I express my opinion the antibiotics were not working as I finished them that morning.
    A light bulb went on in the consultant head. And I was taken away for more blood test. After waiting an hour MY CRP was 160. I was admitted again into hospital.
    Junior Consultants prepd me for another abstraction.
    The following morning the Senior Orthopedic Consultant (SOC) did the ward round. The SOC said what is this. To their excuses SOC stated you found nothing last time it is not going to happen. Then one of Juniors stated how about antibiotics to which the SOC repled for what infection!!.
    SOC next statement was we are discharging him tomorrow find out what is wrong with him. I have never been moved so much and been examined so rapidly. I was taken to a department there was no joining the queue I was seen instantly.
    Following day at 15:00 hrs they found I had Crystal monoarthropathy - Pseudogout. At 16:00 hrs I was discharged with 500mcg Colchicine BO. For a minimum of 3 weeks 2 daily.
    on discharge my CRP was 180.
    Follow morning it was like waking up in someone else body after taking 1 tablet the evening before.

    I want to REALLY thank you both for all your video's. I'm scheduled for total knee replacement in September and am 71 years old. It's great to know what I'll be expecting both concerning the surgery and most if all the care and physiotherapy afterwards. I've been doing my own physio home to strengthen my quads etc and even bought ankle weights and are using them as I progress.
    Just one question. Is there such a thing as too much strength training? Will built up muscle hinder the actual surgery?

    Hi, Brad. I have a question about a TKR procedure called the "minimally invasive" knee replacement, or, the " quadriceps sparing" total knee replacement. As l understand this the quadriceps muscle is not cut for the procedure, resulting in faster healing, less pain management, and less rehab. Do you have any experience with this procedure? Are these things really true? If this surgical procedure is superior to the traditional TKR, why isn't every orthopedist doing it? Thanks in advance. Your videos are very informative. Keep up the good work!

    I went to physical therapy for months with my Left TKL and that knee feels good. With the Right TKL I was into long enough to get my leg straight and then covid hit and I stopped going. I can diffenently feel the difference, more pain in R knee.

    I’m in the uk after my THR I was only given self guided physio at 12 weeks post op I requested some further help and started swimming every day physios now discharged me but I’m still doing the pool work everyday and im still improving in my mobility

    I’m lucky so far, my joints are healthy! I’m having vascular issues in the legs. Have just been referred to surgeon to go on wait list for 2nd surgery. 😊 (Mind you, heat is doing me in! 🤢). Stay cool 😎

    I love your videos- vey helpful. I am 18 days post op for a hip replacement ( anterior) and have started the home portion of exercises based on exercises given to me by a home PT . 2 things: I do have a clear leg length discrepancy. PT measured this at a bit over 1cm. It is really weird to walk and PT has me doing deliberate slow walking with walker and says I will adjust. 2) Thigh numbness, burning and pain . After the simple PT exercises ( not with walking) the muscles down to knee and in front of thigh & groin just burn and ache. I'm in considerable pain hours later so it is hard to know during excesses. Can you two offer a video on leg length and pain after exercises ? I have more pain in terms of muscles that I did before surgery. In fact there was no pain like this before. Thank you.

    I had a hip fracture and slow recovery, so PT started late. My surgeon said I had improved as much as I was going to, but I’ve continued with PT and am still getting better, though slowly. I realize this post is a little off topic, but how long should one do PT after a fractured hip where you can’t put any weight in the leg for more than two months? I’d love to see a video discussion of that. Thanks!
    Oh, and a discussion of Osteogenesis Imperfecta would be welcome also!

    I remember with my knee op having physio x3 weekly. Helped but really hurt. Best thing I did after a while was get a physio to write me an aqua routine. I kept it up for two years and even now years later, I can remember most exercises and enjoy doing them at the pool before or after aqua disco class. I now do physio lead pilates weekly, as a preventative to every ache n pain. I call my class "Move it or lose it!"

    Hey guy’s, I throughly enjoy your channel. I live in 🇦🇺 And I recently had some surgery to remove a plexiform neurofibroma, I suffered some rather dangerous complications in recovery and required a second bout of surgery to subside the bleeding that lasted 6 hours. Is there any chance you could chat about the tumours, how they work and complication they cause. Thanks

    I absoutely agree, I had a TKR 5 years ago went for 3 month therapy and continued doing on my own but did stop and thought my everyday would help well it didn't i'm still stiff after 5 years what should I continue to do to get the motion back in my knee. thanks Docs

    Thanks for your imput on the subject and I'm thinking 6 weeks is way to short a period without losing some of if not all whatever amount of bend we have.
    I'm 3 months bilateral post op and the left knee has been very slow at bending and has 120 degrees and 125 degrees for the right knee on a good day and am happy with those figures.
    Both knees although still a little stiff (hey its only month 3) allow me to put my motorbike boots on and get out on my classic 500 Royal Enfield motorbike and do all the normal living day to day stuff.
    I'm 60 years young and retired early because of the knees and plan to include my daily at home water based physio and upright exercise bike sessions for ever and ever, amen.

    I went for preop for left hip surgery. I mentioned therapy and told no therapy. We just want you to walk. Better you feel the more you should walk. Therapy place called and told me I have six appointments approved. So what do I do? Opthopedics tells me no don't go to therapy. Therapy place says it is important and scheduled four appointments.

    Hi guys you are great!! Would you be able to explain your ferritin levels and B12. My levels are quite low ( ferritin) and I believe the use of PPI’s have caused this. Can you explain how this works. I’m so tired all the time! Thank you!! Love the shows.

    I'm in my 70's and dealing with painful arthritic knees with right knee bone on bone. I've been told I need Total Knee Replacement on
    both knees. I'm trying to keep from having the surgery for as long as possible. Have had cortisone, and orthovisc injections which have really helped with the arthritis pain. It's been almost 5 months since my last injection and it's now hard to walk or stand for long periods of time without my knees becoming stiff and painful.
    I also have "Wind Swept Knees." Could you do a video on this condition and how best to treat it?
    And could you explain why my knees hurt at night while in bed?

    Hey docs, I'm a pretty young guy in his late 20s. What are the best things I can do to avoid a joint replacement when I'm older? Is drinking a lot of water good for your joints? Btw, I have ligament issues but not too severe at the moment.

    Hi Docs - Thanks very much for your terrific videos! I had a bi-lateral hip replacement (anterior method) approx. 2 years ago. I am doing well. I do AquaFit approx. 3 times/week - but would also like to include lap swimming. Is it okay for me to do the Breaststroke? I am primarily concerned about the Breaststroke kick. I am now 70+ but sometimes forget that I am - and tend to overdo. Are there other motions which I should take care with - or avoid - to minimize wear & tear of the protheses' etc. Again - many thanks! Your videos are very appreciated. g in T.O.

    I was doing pretty well after PT..but I thought working outside and chores were going to help..nope..

    How common is it for your hip to click ( not audible ) after TRHR ? It does this when doing a marching movement.. There isn’t pain but started at about week 16 after surgery

    Would you please talk about Intrapatellar Constructur Syndrome after a total knee replacement. I’ve been taking cortisone for 5 weeks now and I still feel stiffness even though I do my exercices twice a day. The patellar is mobile (sorry I speak french 😉) The surgery took place on October 4th 2022. 😢

    Thank you for this video! I found it helpful, as my HTO was 10 years ago, and know replacement is inevitable. I bought a new bicycle, a contraption to be able to use it inside my home, and after forcing myself to ride only a half hour a day, my knees feel better. Two days of crunching while peddling, they seem loosened up, and I wish I would have done this years ago. I Will remember both of your words as I continue towards eventual replacement, and keep up the PT’s exercises, as I know knees need to keep moving. Thank you for your advice, Docs!

    I've had a few occasions that required PT. My experience has been very bad in each case with respect to the following. Week 1, they evaluate me and give me a list of exercises. Week 2, they do absolutely nothing. I come in and they point to the rubber bands and walk away. This is expensive and I don't see any benefit in this type of care. Am I going to bad PT or are all PT this way?

    I went 3 times, once a week at week 2.5 and 3.5, and one last one at 6 weeks. I pushed myself hard after surgery, doing all the exercises 3 times a day. I'm coming up on 8 months post surgery. I'm doing ok, the only problem I'm working on is I cant bend completely and I can't put my foot on my opposite knee to put my socks on. But I walk fine, leg is straight and the muscles are in good shape. The other knee will get replaced in 6 months so I'll have a matched pair. 🤣

    hi , i had a surgey 3 months ago beacause i got a humerus fracture on an arm wrestilng and i have a plate with 7 screus in there my doctor witch is a very good doctor said i dont have to take them out but but i am 17 and i have heard that i have to please answer me what should i do?

    Surgeon isn’t starting me with physical therapy on my total knee replacement until two weeks after why is there an answer to this why he’s waiting two weeks or do some doctors just wait two weeks

    Given this is probably best decided by a physio then maybe give them the professional autonomy to decide. With all respect to you guys…Surgeons generally do not make good functional decisions and why should they? It’s not really their bag. Similarly, in the same way as I have no clue for example what is the most optimal prosthesis to use in a hip replacement.. for me if you’re using a physio who is driven by cash, dragging patients back week after week with no decent strategy and making zero progress then move to one who does.. for most of us these days, optimising education is key here. It’s a goal oriented process and not time dependent.. In the same way as we dose medication, stay in therapy for the shortest period needed to achieve optimal outcome. No patient is ever the same and some need more hand holding than others.

      Agreed Simon. But like you said therapists are different as are their goals. We don’t tell Physios how long to treat our patients.     Don't forget, physio is not free.
    A coworker was going 3 times a week and each appointment had a $40 copay. So its not a cheap thing.
