    Epidermolytic hyperkeratosis (EHK) is a rare form of ichthyosis due to mutations in keratin 1 and 10 genes, but we also see it almost daily in dermpath as tiny incidental foci on the skin of many different patients without ichthyosis. We also sometimes see it as a solitary lesion that can be seen clinically (epidermolytic acanthoma). Excerpted from my dermpath basics 4 video for my 5-Minute Pathology Pearls video series.

      My family has four generations of EHK (my late mom, myself, my late daughter, and her oldest son, my grandson) so this is good information for educating doctor's.

    I was diagnosed with EHK about 3years ago I was treated with accutane for about a year it helped decrease the amount of skin I was producing not much of a change in appearance though hoping to get approved for a new medication soon. Thanks for this video :)

    I’m 66 I’ve had this since 2009 it drives me crazy and I had to make a scene in the doctor office for them to take a biopsy

    Very cool. I have mostly right-side mosaic EHK and always wondered why every new spot I develop starts off looking like a bubble under my skin.

      Thank you for sharing your story! I met a young girl once with pretty severe EHK and she said kids at school made fun of her and said she had “rock skin”. She told them “no I don’t! I have ichthyosis! Educate yourself!” Or something sassy like that. Gave me huge respect for that little girl and others who struggle with medical and social side effects of ichthyosis!

    Just noticed dark spots on my areolas, I didn’t notice them a few days ago. Should I be worried? Any treatments? Help is greatly appreciated

    PLEASE I really need some insight! I can pay out of pocket if needed for your opinion. I have a very severe skin condition and I think this may be related. It’s every where and recently has been showing in my face more.

    If there is epidermolysis, why hyperkeratosis exists? Why does the epidermis with epidermolysis responds with hyperkeratosis?

      Great question. I have no idea why it produces hyperkeratosis. The “lysis” of the epidermis is an illusion. It is not truly lysis as the keratinocytes are not falling apart. But rather they have cleared out cytoplasm because the keratin filaments that usually evenly fill the cytoplasm are all tangled up into those pink globules instead of evenly spread out.

      I think it's reactional hyperkeratosis to compensate for the markedly disrupted barrier

    I was born with this condition EHK and I'm now 41 and have developed allergies to most lotions... I just want relief without the accutane or soriatane.
