Note: I played an early demo build of this a year ago (which had the first two areas in it). The full game has now been released though and it's one hell of a ride. Lots of freaky characters to meet and a clever time-loping story where your failures can lead you to totally different locations!
Does the NHS have hospitals that have adjoining prisons, like in this game?
@GregoryTheGr8ster Haha, Sure! And massive swimming pools! According to the lore, the Heilwald Klinikum was a renowned hospital/asylum hybrid medical facility. So I guess the prison area would be in the asylum section. Either way, you really don't want to go there for any treatment! :)

My heart sunk for nurse Astrid who was now trapped within the Iron lung-like contraption. She grabbed the meds from the desk so she can give them to you.
OMG, I didn't know that it was nurse Astrid. I was puzzled at first as to why she said sorry when he took the meds.
So that's why she seemed so different from the other crazy patients (except Dokhu) who had received the treatment.

Normally in a lot of medical horror, it's usually the patients that are treated as the real threat. A lot of media, especially if its in a psychological asylum/medical setting, depicts sick people as inhuman monsters that care about nothing else but to kill and maim.
I'm really glad this game did not do that. Although I admire doctors and nurses, the medical abuse stories I've heard are actually horrifying and gut wrenching. Vunerable people, from physically disabled to mentally unwell, are the most likely to be abused, not the other way around.
It's wonderful to see a slice of humanity in something so disturbing and grimdark. To extend sympathy to the ill and even to some of the staff to a degree.
I think this game satirizes the underbelly of medical care world, in which the game serves you with some sort of Hippocratic Oath to gauge the doctors and nurses misbehaviour by representing it thru their "madness"

To be fair, I think that the sequence is meant to be really surreal and absurd.
It feels like the game is going for comedy by juxtaposing the horror with purposefully ridiculous situations.
It's impressive how well the game builds legitimate tension and unease while the actual situations and characters are so silly.
The developer must have had a great time plotting out these sequences 😂

"Trash Collector Alive" Wait, what? You could get him killed? Gross as he may be, he is one of the nicest people here! A bit of a Dampé (functionally similar to him too).
Along with Dokhu, Helene, and technically, Astrid. None of them cause you harm or betray you. Helene seems to be the only one of the staff who retains full humanity even, everyone else becomes some kind of mutant or amalgamation. Even the trash collector, friendly as he is, has become partially trash.
I like how the Heinwald Klinikum is basically Silent Hill- location where people's inner demons become visible as creatures.
The thing that I still want to know- what is the deal with the character we play as, who is he in the story

Helene remains human but she seems to have broken from the trauma of the facility and from being overworked. She's completely apathetic now and only does her deeds out of duty. She's become a bitter, cynical shell of who she once was. At least, that was how I interpreted her.
That said, her being one of the few people to actually and properly care for the patients puts her in my good book.

@@lesigh3410 I don't think she's apathetic. I think she's exhausted and overworked, and she's trying to pick up the slack left over from the others that she realized have gone absolutely psycho. She has kind of turned into a monster in her own right, but instead of changing into a beast to torture the residents, she flies around to try and keep up with the work at an inhuman pace.

Just sat through all two hours, that was pretty awesome! The character designs are creepy af and pixelated enough to where you can’t fully see all the details of their faces, which makes it better imo.
Its also cool how the only time you can actually understand the voices is almost always when something bad is about to happen, so no matter what when someone is speaking it’s unsettling in one way or another. Lot of creative design choices that add up in a big way.

The name of the clinic (Klinikum means hospital/clinic in German, but in that case its more a clinic than a hospital) can have a different meaning in English, since the two words can be translated into English in different ways.
The name "Heilwald" consists of the words "Heil" (safe, heal, unhurt,... ) and "Wald" (forest, wood) . With that you can translate it in different ways. Like Healwood or Safewood as example.
In any case, based on the name of the clinic, as well as the names of the staff and objects (Fleischsuppe=meatsoup, Küche=Kitchen, Kantine= Cafeteria ),it seems that everything takes place in Germany.

I LOVE this game. What an amazing concept, thrilling story, and abit was too funnysolutely fleshed out, unique, and creepy characters with interesting wants, dialogue, and unique problems to solve for them. Also, the character designs were absolutely flawless and creepy and the graphics fit right in perfectly to the whole game. One of my fav parts was how Anne, the electric shock therapy lady, kicked open the doors, lmao. It was too funny and a great detail added, among others. An art, but certainly not least, thank you ABG for showing every nook and cranny of the game and completing it at 100 percent s we could enjoy every juicy and creepy detail. Best gaming review channel ever. You don't talk but still have more personality than the rest of these obnoxious reviewers, and show everything, even what it is like dying from the antagonist(s) and all the possible endings.
PS - shoutout for all the tea baggers out there, lmfao. Those monsters deserved it.

everyone in this game is kinda creepy (at least among those chasing you) but they're all weirdly endearing in a strange way. but the only ones that really unsettle me are the uh... Things inside the bathhouse, and wolfram himself. i sort of took it that the things in the bathhouse *are* wolfram, showing his true colors. ie. that he's only a Monster in private

So the loophole is that the game ends just like it starts, with a Dr. Randolph and/or you falling in the hole and starting the cycle again? I noticed that the monster version of Randolph has his teeth destroyed and bleeding, just like you at the end... are you him? Or a part or him? Also I take it that the ominous voice is the old chief doctor who "resigned" just like everyone else thought Astrid had resigned. I'm also curious about how this affects different people, like the nurses who didn't get it so bad, but Astrid who was also a nurse (and a good perso too apparently) had it the worse... also the whole thing comes back to the golden rule of "no one ever dies" of dr. Randolph.
Edit: also everyone seems to have a theme or medley at least, and I think Randolph's theme when he chases you at the beginning is a distorted version of the ending song, really cool! I felt kinda sad at the end, like pity, and sad for Astrid too. Damn what a great little game
I do t think that we play as Dr. Randolph, but maybe it’s a different kind of cycle. One where the last chief doctor fall into the loophole, and when everything is complete, they join the pipe guy or escape. Or the doctor dies and that’s that. But the ending does leave something to be desired.

I can’t believe one person made this.! He is super talented. The creativity and quality of the whole thing is just fantastic. One of the best games I’ve seen on your channel for sure.

I really love how this game turned out. It was super fun to watch, it was just the right amount of tense moments. All the doctors were really unique and I love that they all have their little story. At first it was extremely eerie, but at the end it became sad. Specially for people like Astrid, who had good intentions and ended up the worse... All that being said I still have some questions about the sealed plugs. I was almost sure you would had to do something with them but apparently not. Still I really loved it, I think I prefer it this way, with not everything 100% clear. I really really liked it!

Finding the plugs on the wall, I believe, were the "secrets" you put into the statue w the number five on it that led to you open up the other plug, where the ominous voice came from, that looked similar to the others, like a corkscrew. It even has the pop sound.

A "Resident Evil" style game based on abusing the Hippocratic oath. Interesting concept and execution.
This is my newest favourite game now. The spoken dialogues are truly captivating deep into my soul. Can't get enough of the way the nurses (except that kindhearted nurse) walking. And the Trash Collector & Dokhu boy is lovely, so cute.
That Neglected Patient really brings the "Exploding the toilet" to another level with his new entry in the bathhouse.
This game truly is an impressive work of art. I love all kinds of horror games and this one has breached my fear threshold and got me good. The way the characters talk in creepy sounds and the dark pixelated graphics adds such a unique and frightening aspect to the atmosphere. It’s the right amount of creepy, mystifying, intriguing, and goofy. I won’t be able to sleep tonight

The pixel style really adds to the horror. The atmosphere is great and I love how the hospital staff are designed. Like how Nurse Anne walks and occasionally shocks herself or how doctor Hauser casually teleports when chasing you. The freakier monsters also deserve special mention such as the ones you encounter in the showers.
In most games, not being able to die would take away some of the tension but I like how you can use that to your advantage here to open new paths.
And I love how the story isn’t totally spelled out and still leaves you with questions. Like how exactly did you get there? What or who is that mysterious voice you free? Are they some type of spirit? A god? A demon? Why did they speak in English while every else didn’t until they got mad? And what exactly caused Dr Randolph and the rest of the staff to go mad anyway? I’ll admit I would’ve liked some more of these questions answered but I’m not too disappointed if at all.
I like to think that the Ominus voice is the last chief doctor or part of him left in the building.
I'm guessing the Ominous Voice is the previous Chief Doctor.
Another possibility is that it's the Heilwald Klinikum itself.
I think it might be death itself. If we assume the story has some sense of literal-ness and isn't one big metaphor, the chief doctor wanted to stop death at the hospital at any cost. So what better way to do so than to literally stop death and trap it.
That's what I feel as well...
The thoughts of the Trash Collector helps a lot to find more of the story of the game, pretty much starts been Randolph a self-proclaim visioner that wanted to all his personnel to experiment his fields in all ways possible for improvement and become the best hospital around, turning all the hospital in a decay spin to hell without moral and dicipline; some keep good hearts, but the others gone mad and devoured on their darkness; when this happen, Randolph felt he failed and couldn´t stop what he just did, feeling more and more anger on himself without showing it to others.
I'm loving it! Getting Echo Night vibes, but the mechanic of "death" not being a game over, but rather waking you up in an "out of state" location where you can get more lore, have a wacky adventure, makes the pain of the trial and error of the game much more fun- even when you die, you'll see something cool.
I'm pretty desensitized and immune to majority of survival horror/horror games yet this one still somehow managed to make me feel very unsettled and uneasy.... Very eerie

I wonder what the nature of this place is. Is it a construct of the main character's trauma with a bad Clinic? Or a sort of "Hell" for a bunch of quacks?
A doctor managed to make a loophole in time-space so no one would die in the clinic and it worked! But no one got better either so most just went mad, especially the staff.
I'm still confused on the plot of this story. What exactly happened to drive all these people crazy?
Might be connected to the chief doctor madness. Runs the place like it was an asylum and did horrible things to the patients
Remember what was said at the beginning? "Nobody dies at Heilwald". The new chief physician managed it by violating time and space -- so nobody ever died, but nobody got better, either. Over time, reality began to break down, resulting in insanities and mutations.
The one voice of sanity was Astrid. The one person who dared to question the doctor and point out flaws in his practices. So she was quietly "silenced" before she could show her evidence. (That's her in the iron lung, if I'm not mistaken.)

Messing with time and mortality in combined with extreme but not unheard of medical abuse and neglect turned the nurses and doctors into monsters and the patients into barely alive shells basically
The broken dialog speech on this game makes the character I'm sure wonder if he or she has lost it. Can't trust doctors or nurses here at this facility

1:50:50 How beautifully it describes the bittersweet ending that starts with the music and towards the end of the music. No matter how bizarre it describes, it also highlights a bitter truth.
This game is amazingly good. It is creepy, tragic, and funny in equal measures. It feels like Silent Hill at times with the sounds and design. The characters are fascinating, the story is intriguing, and game play seems fun. The sound is off the chart for how good it is. The music is great and used at perfect times to highlight a scene. The voices are haunting and creepy, often sad. The low rez look just adds to the ambience. I am very surprised this isn't more popular. This game needs more love!
ABG I tried watching another silent Let's Player and they just don't have the personality you have. Your presence shines through and makes watching fun. The nodding, the tea bagging (XD), etc really make the gameplay a joy to watch. You are a delight to vicariously play games through.

I think there's a lot to be said on the delivery of the dental clinic scene. That is the one area where there's the straight fixed path, and you can't use any loopholes. No tricks will work here. That's why Sabine doesn't trigger anything. The tension continues to build as you want to know what's so bad that the game forces you to continue. And this is the sole part of the game where the protag expresses fear: you hear your heart pound. So if it's bad enough to make a character who's expressed no real fear to this point express fear, then we know it's bad. We have been forced to let this tense aura fester, to keep going. So we know it's gotta be something.
I just noticed that the shit texture wraps around Nurse Anne as she walks away to check the generators.
it only happens for a second but it stood out enough for me to notice and rewind.

I bought this on Steam last night, am yet to play it but dropped by here cause of being curious if German was actually spoken, and Dr. Randolph caught immediate and undivided attention. It's like if you took an already broken/unfinished AI, fed it the Hippocratic Oath and asked it to implement it according to it's own understanding, Dr. Randolph is what would come out the other end.
So, I'm going to get committed after dinner in a couple of hours, and was wondering if I could book an ECT session with Dr. Randolph himself for tonight? Or if at all possible, a tour of the treatment facilities, so that I get an overview of treatment offers? I simply cannot wait!

what happened to Nurse Astrid absolutely crushes my soul. an innocent nurse who wanted nothing more but to expose the dark truths about the conditions the hospital put patients through had to go through something so horrible. bad things happen to the best people, and it shows in this game
Such a fascinating game. Very imaginative and intriguing. Love the PS1 graphics style (glad to see more developers experimenting with that aesthetic.) Well thought out game, really cool ideas at play and implemented well.
"Don't fucking SEASON me!!" I think one of my favorite characters Sauer the Cafeteria chef. Hilarious and creepy at the same time.
The game has this great ability to make you laugh and make you uncomfortable all at once. Not an easy feat to pull off. Especially, with veteran gamers who have seen damn near everything possible up to this point.
Uniqueness and imagination goes a long way with those of us who have been around a while.
Looking forward to playing this game myself, as well as future projects from this developer.

i like how everything in this game moves in real time, there's no fade black cutouts when characters are supposed to move around- adds a bit to the immersion!
This really is an amazing game on so many levels but the ending felt like there was still more to go. I mean we barely see the dentist but the game is over when he is gone but everyone else is still doing their thing? Feels like the ending only covered a fragment, it just does not feel like the end.
The dentist was the first doctor you meet. He transformed into that... THING at the very end, succumbing to the same fate of the other doctors.
That dentist part was intense
