How to: Measure Blood Pressure

  • An introductory video explaining how to manually measure blood pressure. In this video, we are sharing with you the auscultatory method of determining systolic and diastolic arterial pressures. The cuff is placed above the antecubital artery and the cuff is above the upper arm. The stethoscope is placed near the antecubital artery and no sound will be hear when there is enough pressure to close off the artery. When the pressure is slowly released, the sound of blood jetting through the vessel by the vibrations of the wall is called the Korotkoff sounds. The first sound is also the systolic pressure. The diastolic pressure is when a muffled quality occurs. Please note that the sound is often overestimated because sound can still be heard even after deflation of the cuff. This method is not 100% accurate but will give values within the 10% range.

    I literally came here because I was just curious on how to read blood pressure, I'm not a doctor, nurse or in any medical field whatsoever and yet this was so easy to understand. You explained everything so clearly.

      There is a few other videos they show actually how the cuff is suppose to be aligned he placed the Stethoscope right under that spot there didn't say that. The reason lot of people get wildly inaccurate numbers at home with a machine is because they put the cuff on in correctly not allowing for a 1/2 inch space between the elbow bend also not aligning the cuff correctly. Also cuffs say the arm range if your outside of that range it won't read correctly.

    Then i watched the video and notced i had my stethoscope "off" , i had no idea those could be " off" !

Now I am 19 and I am a Registered Medical Assistant, with hopes of becoming a PA later on. When I started out young, I realized I had a passion for this field.

    For people arguing about the proper pressure to pump the cuff up to. This is a neat trick I learned being an EMT, where all we do is take manual blood pressures. Palpate the patient's radial pulse while pumping the cuff. Once you feel the pulse disappear pump the cuff around 20mmhg more and you will know around where their systolic pressure is. This stops the chance of pumping the cuff too low or too high for your patients.

      Just learned that trick today. My preceptor said its a neat trick but never use that method unless you need to find the systolic. Because that method lacks getting the diastolic and thats important too. Still cool but not that useful

      Sam Damon more important indeed but diastolic can indicate heart disease which is also important as an underlying factor or for patients general awareness. Important to do it properly as there is no point asking/answering questions later about why it wasn’t done if you had the opportunity but weren’t bothered. At least important for baseline

    Thank you so much for explaining this. In my pre nursing class, I was told that the first beat was the systolic pressure, and the second beat was the diastolic. I was so confused in lab wondering why I was getting 120/110 lol NOW it makes sense. Diastolic is the last heart thump! Awesome! Thank you! 😀

    A few too many inaccuracies for my liking and many other commenters have picked up on one or two...
    1. The part that wraps around the arm is a CUFF. A Cusp is a segment of heart valve, or when your birthdate is close to borderline between two astrological signs(!!)
    2. The stethoscope is not turned off or on with rotation. It has 2 sides, each specialised for particular sounds. The "bell" is better for hearing low frequency sounds and the "diaphragm" for higher frequency sounds. For brachial blood pressure, use the diaphragm side.
    3. The bell must not be under the cuff. The edge of the diaphragm housing can be. Likewise, don't apply a lot of pressure to hold the stethoscope in place. That can result in inaccurate readings.
    4. Open the release valve more gently, letting the air pressure down more slowly to get accurate readings. The needle is moving so far between each pulse in your video, that you'll only get rough readings.
    5. Systolic pressure reading is not because of the heart contracting. When the cuff pressure is so high that it squeezes the artery shut, no blood passes to the point where the stethoscope is. As you release the pressure in the cuff, the first sounds indicate that blood is just getting past the cuff for a moment. The heart's maximum output pressure has just exceeded the pressure in the cuff. This happens to occur in early systole.
    6. Diastolic pressure is when the cuff pressure has dropped to the lowest pressure that the heart still drives arterial circulation, just before the heart goes into diastole (where the atria begin to refill the ventricles)

    Today was my first day volunteering in my local hospital. As soon as I went in, the nurse called me and taught me how to take blood pressure, heart rate, oxygen saturation... I was nervous for my first one, but later I got the hang of it and did many after that- it felt amazing doing that!

    Small tip for those that still stumble upon this video. When testing the stethoscope PLEASE do not tap because some are much more capable on picking up sounds and if you tap with them on, then it will definitely hurt your ears. A better way is to just gently rub you finger over it and listen to which side is louder. Which ever side is louder is the side you should use! Hope this helps to those that need it!

    Marie Wallace, the purpose for going up to 200mm Hg is that it is a starting point for checking blood pressure.
    Since you slowly let out air while listening to the brachial artery, you want to allow yourself enough time to hear the first rush of blood through that artery. For some people experiencing hypertension, that first rush of blood may be at 180mm Hg or even higher, which is why the standard starting point is 200mm Hg.

    Wow, there's an amazing amount of critical comments on here!
    Just wanted to thank you for mentioning about turning the stethoscope "on" and "off". I found that out the hard way when I was taking my Nursing program. And when I was asking other Nursing students about it, I found out that ALL the Nursing students I asked, were "faking" their readings. They didn't know how to take a B.P. - I should have clued in when all the Nursing students kept saying my B.P. was 120 / 80. I thought it was because I was walking everywhere.

      You actually do not turn the stethoscope 'on' or 'off.' The larger diaphragm side is normally used for adult patients. The smaller-diameter bell is used for pediatric patients, for maneuvering around bandages, and for carotid assessment. you switch between the two by twisting the connection. This video bugged me to no end.

    One of the biggest problems with so many "how to" videos is that the instructor often assumes too much knowledge on the part of the listener. He or she has forgotten what it was like to be a total newbie.
    Occasionally it's even worse. The instructor is more interested in showing off than actually helping newbies understand.
    One question though. How would one go about taking one's own BP using the same equipment? Any special dos and don'ts?

      Merissa if you are a high b.p patient then never use digital apparatus as i always shows 5 to 10 error.Manual apparatus is better and b.p can be controled by taking certain supplements too without medication

    Interesting video. I have been in EMS for 26 years and have never inflated the "Cuff" to an arbitrary number like 200. Your inflate the cuff until you can no longer hear the pulse, depending on the individual that could be 60 for an athlete or 180 or more for someone with CHF. Inflating the cuff more that what is needed in uncomfortable for the patient and could potentially give a false reading.

    Good video....if you don't know your patients history you do need to go up to 200...there are many undiagnosed hypertensive patients walking around..you may actually save a life by not taking short cuts.....sure take your dial down just a tad bit slower but once you do this many times you can actually move that speed and get an accurate reading...good job...

    This is a well made video, but there is an error in your procedure. You release the pressure at too fast a rate, and so there is an inaccuracy in your readings. The error is because you can hear the heart only when it beats, not when it's idle. Here's an example. Let's say the subject's heart beat rate is 60 per minute, or one a second. With the falling needle measuring mm pressure, the needle will fall past so many mm in a second's time. It looks to me that your needle moved at least 10 mm in one second around the systolic value. If the heart beats by chance just at the time the needle passes the true systolic value, you get an accurate measurement, but the heart is not likely to cooperate so well. Thus, your reading could be off by as much as 10 mm. For a subject with a slower heart beat rate, the error could be larger, and for a subject with a faster heart beat rate, the error could be lower.

      Don't get why he would bother to make an instructional video and then release the air too fast. The instruction book that came with mine said to release it at 2-3 mm Hg per second. And also not to go over 180.

    Remember, the diastolic bloodpressure is the pressure between every heartbeat. When the cuff is tightly around your arm at 70, and let's say your diastolic is 60 mmhg - the cuff is creating a "bounce" on the blood during resting. When the cuff pressure go lower then your diastolic, it is no longer affecting the pressure and the blood can flow as usual without being disturbed, and that's why you dont hear anything

    Not sure why he doesn't palpate the radial to get a systolic estimate. That way you inflate the cuff to 30mmHg higher than their estimation, as opposed to going right up to 200. Also the cuff should be around 5cm above the antecubital fossa, it's too low and their should be no contact with the bell of the stethoscope and the cuff.

    1. It's ok to put a small portion of the diaphragm (bigger part) under the bladder (the cusp) but don't put the whole thing with the bell (the smaller part) under the bladder, your pt is gonna end up with a huge circle mark on their arm, especially if your pumping to 200.
    2. If you really want to get good at taking bp, take some in the back of an ambulance, the low rumble of a diesel engine sounds extremely similar to the low frequency of korotkoff sounds, throw in a siren and you become an expert real fast.
    3. Don't ever take a bp using the needle bounce instead of sound, it's not the same thing.
    4. Don't fake a bp, just retake it.
    5. If you're gonna fake a bp, please don't say its 120/80, it's a dead giveaway.
    6. Please don't give an odd number with a manual bp, it's impossible to obtain, round up or down. Especially if your faking it.
    7. If your a new EMT riding with a medic, TAKE MANUAL vitals, It's easy to cop out and use the lifepack but your selling yourself short, practice!

    The stethoscope should not go between the cuff and arm, the cuff should be placed higher up on the arm right up near the armpit and the stethoscope should be placed proximally and medially to the antecubital fossa. Also personally I go down a lot slower, 2-4mmHg per second. This allows for accuracy. I was taught this as if you're going down too fast you could easily mistake someone from hypertensive, e.g. you said about 118, you want to be confident in your reading. Happy measuring :)

    Great video, thanks a million. I just purchased am OMRON self taking manual blood pressure kit for $21 and the instructions are not clear. Thanks to this video I can now go ahead and use it with more confidence. I passed on all the automatic blood pressure monitors because it seems no matter what auto monitor, people complain about their inconsistency in providing accurate readings.

      maxy dany I’m a Vascular Scientist. You pump up to approx 20mmHg above where you first occlude the artery not 180. Itbisbdefferent for everyone. You deflate much slower than he did on the video otherwise you can miss the signal returning particularly if there is irregular heart rate.

    I'm a senior in high school talking an EMT certification course and unfortunately, my teacher says that its incorrect when you base it off of the first and last beat that you hear (systolic/diastolic).The correct way to do this is to look for the first and last time the hand jumps because even as you let pressure go in the cuff you can still hear the heart beat and not get a true b/p reading.

      Most of us go to 180 but in an emergency situation, which EMTs face daily, going up to 200 is best.

      I do too. What I like to do when I close it is not twist it too hard. That way, you can easily loosen it.

    This is a good video for those who are beginners, one thing though, he pumped it all the way up to 200, on a regular BP reading you should pump the BP cuff up 160 or 180 no higher than that unless the patient is having a heart problem than this is when we pump it up to 200 and so forth to see how fast their blood is going.

      +Michael Matta Yeah when he said 200 i was like whaaat. We we're taught to got 30 above their normal systolic if it's known, or to palpate the radial pulse and pump the cuff up slowly until the radial pulse is occluded. And then go about 30 above that number.

    Nice attempt to explain procedure, but this video includes several technical mistakes that will likely result in inaccurate readings. It would be better to revise this video to reflect proper technique or to delete it altogether. The primary mistakes are generalized over-inflation and way too rapid a release of pressure. Teaching people to take their blood pressure reading in this way can result in harmful inaccuracy potentially leading to moral liability associated with complications resulting from misdiagnosis and incorrect treatment based on incorrect readings.
