The air bubbles that consistently escape throughout the trick is kind of a give away. If you hold you breath in a swimming pool there will be air bubbles from your clothes for like 10 seconds. but here you can see them consistently being released probably a lung implant with a two way valve and clear tubes. Also turning upside down allows for connectin to an additional hose that is connected to a two way valve breathing apparatus this can be subtle and implanted into the nose or the side of his mouth with an invisible small tube. The last part of the effect is where he disconnects all tubes from either the oxygen supply he has on him secretly hidden or an extraneous source. The last 3-4 minutes of the effect are him actually holding his breath which is correct as all air bubbles have now dissapeared. Many ways to achieve the effect probably a smart combination of all of them delivered in a modern way. great effect

what's crazy is that the crowd and commentator and the kids were huge distractions. you have to semi-meditate underwater for maximum efficiency. usually achieved through staying completely still, quiet, relaxation/tranquility and slowing your heart rate down. won't be totally surprised if he one day hits 20 minutes under said circumstances. my PB is 3:13 btw

Thinking like a magician:
There are plastics you can get that are nearly invisible underwater (their index of refraction is almost identical). We know that this trick can and has been done with the use of oxygen tanks and tubes. As a magician, a lot of the little things that people don't think should matter, actually matter. That lighting looks dramatic and dynamic and wonderful, but it can also be used to disguise and obfuscate certain things as well.
Blaine is wearing black clothing that, while not baggy, is also not skintight. There are tiny containers of compressed O2 that can be had for well under Blaine's budget. But if he wanted to keep it cheap, run tubes up either his legs (so that when he's upside down he can breathe) or up the back of his arm (notice a bandage on his arm).
I've never done a "hold your breath for an insane amount of time" trick, but these would at least be the avenues I'd look down if I wanted to replicate this effect (and it could be that Blaine has used none of these methods, but I don't see a good reason that they wouldn't be open to him if he had access to those polymers.

Credit to David - he can actually hold his breath for 4 mins after breathing oxygen and 3 mins after breathing air. Turning upside down in the middle "helps him keep air in his lungs"....sure does, and stick his feet out of the water :). Drop a glass marble in a glass of water and watch what happens. No more clues
