Human beings evolved to live in matriarchal tribes: families made up of multiple generations, families that stay together for life.
When Mark asked Jake about Jake's biggest regret, there was a long pause. Mark, Dr. Syl and Dr. Barnes were attentively quiet while Jake took the time he needed and it felt like a really beautiful moment of care and respect.
It would be nice if he could get some healthy friends who treasure him. He's interesting and well spoken. I hope so hard that he can overcome his drug dependency.💗

Yes, and the stigma around it only makes it harder to get help. I’ve known people in my life who were struggling with addiction who wouldn’t even think about getting professional help because of how much they worried about being judged, let alone what general society would think. People do need to know they aren’t alone and help is out there for them!
Very happy that Jake is with us. He knows that pants can be a billboard. That understanding, speaking, and spreading the word can do so, so much - for those who do know, for those that don't, and for those who eventually might.
I wish more were as insightful and well-spoken as him, especially at 21. I love to see the new generation of educated addicts. The tables are turning

god...i relate to this kid. Im 34 and have ADHD, Social Anxiety and poly drug abuse starting at 14. I have been on ONE of 2 in East Tennessee (USA) ...the first time I was on the clinic I had to come off of it (the clinic that is) because I couldn't afford 160 a week. 3 years later the methadone clinic started taking "state insurance" and I got back on it..but the 3 years in between i got methadone off the street from other addicts that would drive down the Georgia.

I am assuming that proper mental health assessment and treatment is not financially available for Jake. Even in Australia assessment for ADHD as an adult is thousands of dollars.

yo, i feel like i'm in some alternate universe hearing him talk about adderall like that. it's been a positive life changing drug for me. the only downside is it doesn't last very long. no anger (well, not NEW anger lol). no sexual side effects. just helps me focus a little better. i hope his comments don't scare anybody off trying (doctor approved!) medication for their ADHD. i went from being a useless sack of shit to a somewhat useful sack of shit with potential. if i had known i had adhd sooner i probably would have been much better off.
*edit* trying to say... adderall wasn't good for him and i respect that, but that isn't going to be everyone's experience so don't let that scare you off.

Do you think drug addiction ever exists on its own? What I mean is that the model in the US is that drug/alcohol addiction is a disease (which is sure better than the willpower model) - but I worked in a drug treatment center for homeless people and what I saw was not that it was a disease in itself but a symptom of a mental illness … my example is that someone who has good mental health isn’t going to think ‘I need to go find some heroin’ and so down that road … I have gotten some heat for thinking that way but it sure seems to make sense to me (I have Autism so I also could be being literal as well)
Honestly I agree that I don't think drug addiction exists on its own. Most people who use have childhood trauma/trauma or have some sort of mentally illness. And because they don't know any healthy ways to cope with those issues they result to drugs. It's really sad, I wish mental health resources were more accessible to people
I agree with you. I was very close to going down that road because of undiagnosed BPD, bipolar, adhd, autism, the works. What saved me was going on psych meds and getting therapy. I somehow managed to hang on until my 30s when I started really really prioritizing my mental health and now with the right tools I can cope (relatively) well.
Fantastic video. Small side note: methadone clinics are what he’s talking about and they’re the main treatment for addiction in the US. I don’t personally have experience with illicit substances but I’m from a small southern city with a lot of substance abuse. I’ve known of loads of people who had to use methadone.
Okay, something I genuinely think you should talk about more is porn addiction, especially how kids on ADHD meds can get pretty damn hooked.

I was over prescribed benzos and relied on them for 11-12 ish years and omfg my withdrawals were horrific. I’m clean now but it took a long time to get off them and I went to a rehab here in Canada that’s just amazing but it was horrific to get off those things. I know opioid withdrawals are shorter than benzo withdrawals but they are PAINFUL too I saw ppl experiencing them! I hope Jake gets help and has what he needs. If anyone knows his handle I would love to reach out to him bc he deserves sooo much respect for talking about this and for trying to get sober.
he wants so much to get sober, and he is such a nice guy it seems. i pray for him. he just needs a good friend that will help him.

Without better social supports (though videos such as this give me hope for the future in that regard)…how else can addiction to substances NOT happen or be be treated? When basic social needs and conditions are not met there is nowhere else to turn to regulate. So nice to see one more than one person, a more team approach than common, analyzing this…though Dr. Syl does quite a good job when on his own of not acting like things are definitive including his understanding of things. Mark of SWU doesn’t have a therapist there for people after they talk about such difficult things…sad. And wonderful that his work is being picked up and expounded upon. Did better when he had a girl friend …ie connection…also wary of “Borderline Personality” label…his finger was an accident he as much responsible for it as the girlfriend but he doesn’t acknowledge that at all. “One of the worse mental disorders” is nice Dr Sy and Dr Barnes correct and likely this young man is having issues with women reflective of watching his parents.
I don't know about other states, but I live in Massachusetts and we have lots of methadone clinics. We are also a pretty liberal state though. I actually work admissions at an inpatient drug detox program. I think Jake is in a much better place than most of the clients I see at my job. But his story is not an uncommon one, unfortunately. I see a lot of people that have gone down a similar path. I'm proud of him because a lot of the people who come in their early 20's leave the program AMA (against medical advice or voluntarily) because they just aren't ready to take that step to say I'm ready to commit to going off the drugs 100%.
I would give up THC, but I would cut a few years off my life in lost sleep. I genuinely could never sleep as a kid, now I can.

I like, well, "like" is not the right word, to watch videos and movies about addiction because my sons are addicts and my husband is an alcoholic, more than 12 years sober.
It just tears me up how my boys are so messed up. At least my older son is now working at a good job. But neither graduated from high school and have no interest in getting their GEDs or furthering their education.

There is a ton of methadone clinics in the us there's 2 in my town 2 in the next town there all over now....

Sadly lots of good stuff here and sticking with “Addict” label as a mysterious condition without looking at social conditions . So dark ages. Anyone notice how alone the guy is? How about addressing that? Mark doesn’t ask enough about kid’s family life etc. and good because it isn’t the right place for him to open up about such things AND all these video skip over social influences in addiction in terms of support. AA is not enough…for god’s sake its been around for a long time and things just getting worse and that is the only social support even these two otherwise excellent psychiatrists can point to? So angry at mark for asking the poor kid what his biggest regret has been…agian putting it on the individual…mark’s major bias.
It was intensely sad.

@@madelainemadelaine2934 oh yes he was definitely insightful and i found he settled into it as the video went on. the dynamic was a bit awkward at first.
one piece I really struggle with is the idea that some of the behavior associated with addiction is considered a symptom. For example I just can't accept that lying about it isnt a choice, especially when the addiction is known.
Like if you had OCD and went to elaborate lengths to hide that, would _that_ be considered a symptom? I see it as distinct from the repetitive actions that are commonly associated with it.
I'm curious what others think.
I don't think the lying is always a choice. I was raised by an opioid addict and she is adamant that she has a legitimate medical reason to use opioids every day. 25 years ago it was easy to get pills prescribed. When it got harder to get prescriptions she moved us to Florida and had a steady supply for years until she got cut off about 8 years ago. Now she moves state to state using palliative care through hospices telling them she has terminal muscular dystrophy, she has a mild neurological condition but not muscular dystrophy. This keeps her supplied until her unpleasantness causes the hospice to actually look into her records. She wholeheartedly believes she needs opioids because of her neurological condition but I think the pain she experiences is due to withdrawal. Four of her siblings have the same condition and all of them lead successful happy lives, something she's never been. I don't think she'll ever quit pills because she truly thinks she's not an addict and needs the pills. I haven't spoken to her in years but last I heard the palliative care wasn't working out very well anymore and she couldn't afford to move states again and she's a known drug seeker at all the hospitals around her. It's so obvious to me when she's lying but I believe she genuinely doesn't think she's lying so I don't see it as a choice
maybe if the consequences for being found to have or seeking help for for the OCD were jail this would be a more accurate comparison, but it's complicated.
27:20 Yes he had a formal diagnose, he was talking about that at the beginning or at least I understood it like such. He told that still being in school he had a medication that helped him focus but he was feeling like a zombie and didnt remember much of it and so he stopped taking that, then he experimented with self medication @DrSyl

Fentanyl is stronger, yes. But it's also dosed at 75-100ths of the morphine. 10mg = 100mcg. I have cancer, my sublingual tablets (1200mcg tabs) don't feel any better or worse than 60mg of oxycodone (equivalent) dose. By better I mean the normal mild sedation and calmness you get from opiates at that dose for cancer pain. Fentanyl isn't the problem. It would never be in the drug supply in the first place if we'd legalised things years ago.
it's not like the pills on the street are coming from pharmacies, they're being manufactured by the drug dealers. pressing pills and making them look legit is easy.
It's also being mixed in with other drugs because it's so cheap. A lot of people don't even know they're taking it. they think they are just buying oxy or whatever.

But how does he get off fentanyl? If the withdrawals are so intense, what is the recommendation on getting off this insane drug? 🤔
By honest, realistic medicine. If the person doesn't want to stop using yet. it's cheaper and safer to prescribe them vials of fentanyl or fentanyl sprays so they don't end up in the ER costing the system money. If they do want to quit you can use fentanyl patches and SLOWLY reduce the dose or use subutex or methadone and slowly reduce too. We need an honest discussion about drugs, fentanyl isn't evil, humans will always take drugs and prohibition has never worked.
some people need medically assisted detox and be hospitalized. suboxone/subutex can help, but that's really more for controlling cravings. that's a huge part why it's so hard to quit. especially if you can't afford rehab, and fentanyl is sooo cheap

Why did he get a 100 pil bottle? In our country If your almost dying from severe pain then you get Tramadol but not one package but five cut of that strip 😂 and here goodluck , be painful but adiction is not going to happen.
If you're a parent, please don't put your kids on ADHD meds.
