    Depression has been a thing you don't talk about in public unless it's someone you really trust and come on if you're paying someone to trust you it gives off inauthenticity to your audience of people kind of like Selena's "gossip session of confession" the reason these are private things is because the real truth is grotesque or ugly and not good to talk about

    this, people hear pedophilia and instantly think criminal, you have to think that the inmense majority of people with pedophilia arent criminals and have never commited any act, they treat them as "high risk" as if having those feelings would make them violent but they dont think about the fact that 99,99% of them are just people that were born with something they didnt choose and they cant take it away from themselves. but they have the same values as you or me, they dont come from the viking era or 2000 eyars ago. those are not criminals, those are not risky, those are real people with a problem that need help. Stop demonizing them, they are people. And the same goes for murderers, if you have a brain that is unable to live in society without killing people, then you have a problem that needs to be solved and you need help. Nobody deserves to be a murderer, even if they wanted to in the first place. Even malice can be cured. Everyone needs help. Perpetrators dont exist. Everyone is a victim of this world

    After watching this, yes I agree. The gentleman on the right has late stage schizophrenia. He seems pretty responsive at times. You'll noticed the catatonic state more at the intro with his increased rubbing of his eye. Being a doctor myself these are all hallmark traits of Schizophrenia. His pupils are quite dilated which could be from a couple things A ) They are still working on getting his a combination of olanzapine and samidorphan, sold as Lybalvi balanced with other medications B ) [ Which I find to be the most fascinating ] Having him watch videos of people and talk about them. Notice at 7:07 we see the ridge of both knuckles. We call this fidget several things as it is an indicator of his frontal lobe being overloaded with information. Very interesting case, amazing case study. We are making great strides in this field. It also appears they let him stich his name into his clothing. We see this a lot. Notice how he stitched his name Phill as " Fil " and the last letter not being legible at all. Love this case.

    All of the research and treatment is into adult men but we've seen a recent substantial increase in female sexual offenders especially female school teachers targeting male students maybe it's time we stopped gendering these things and did more to stop ALL perpetrators and protect ALL victims there just isn't as much research because of the feminist slant that men and male biology is always the problem

    You, Dr Syl, are not the typical guy I would be attracted to (physically), but your calm demeanour, humble intelligence and genuine kindness has always made me think of you as a "catch"! Ive always thought that my attraction to personality and intelligence/education makes sense on an evolutionary level, but I think I caught a bit of hysteria for a minute there because I swear my uterus did a little dance when you said you knew French. I don't have any connections to France or their language. Why the heck did your attractiveness skyrocket when you said you knew French!? I'm trilingual, so I shouldn't have been nearly as impressed as I was 😂😂

    When you were talking about the gray area of paraphilia you focused on not harming the other (or self), but I was wondering why you didn’t address consent (with safeguards in place) of people that might engage in such activities? The other thing I was curious about was where does the psychotherapy come in? It seemed the option put forth was a pharmacological one to bring about behavioral cessation; it kind of feels like going for a quick fix for society’s benefit (compliance to norms) versus the hard work of working with a client to bring about change to where they can still experience the totality of being a human. I understand that both can work in conjunction, but stopping after the pharmacological option it seems that the person is determined to be irredeemable (locked in a drug induced prison without the option of rehabilitation). I might have it wrong, I’m sure your original presentation was much more involved, but this was the questions and thoughts I had after watching this video. Thanks of doing it, not everyone is as brave. I’ve learned a lot watching your videos, look forward to the next one.

    I noticed how you seem to be talking as if all sufferers of these paraphilias are men (e.g. orchidectomy as a treatment for a dangerous sexual behaviour). It’s clear men perpetrate these actions most, but there are female p*dophiles too. I’d be interested to hear more about women who have or act on these paraphilias. Also the description of the treatments for these men sounds similar to some gender affirming care for trans women, so it’s worth highlighting that these are two VERY separate things (given that trans people are currently suffering a lot of abuse & being called “p*do” & “groomer”, etc).

    Hey Dr. Syl. I'm a men with many fetishs and I really appreciate the eay you describe the healthy side of it and the problematic side. BDSM and fetish are often seen as an disorder by other people. They usually think of the hard core BDSM, such as wipping. But there can be many fetishs that people have and dont even know is falls under de fetish espectrum. Liking man with beard for example, is considered fetish just like liking feet. The point you made, and should be reinforced here is that the illness should be view separate from the healthy fetish. Thank you very much to talk about this so professionally and wirhout judgmente like you do.

    I dunno if you would respond but me being a gay man, I find it very erotic to see heterosexual men being "pegged" by women. It get a high from it as well as seeing "top" sex performers to do bottoming roles. Is this sadism? I donno how would this be as an analysis for a behavior? Am I normal or borderline something on the other scale? Weirdly, most psychiatrists I met with in India were okay with just everything, except for me being gay, where some even wanted me to be ashamed of myself as an Indian to be gay. One famous psychiatrist got me in trouble by calling a mob of Hindu extremists who wanted to h∆ck me with machetes in a mob justice aka the infamous mob lynching that happens in India by a group of people and they are never charged with it under any law. One female doctor was very kind and she understood my issue and comforted me. I feel shocked to be in a country where child marriages are still accepted but homosexuality is not even by major psychologists and psychiatrists.

      I fall in the first and the second disorder. Third is not acceptable to me and I condemn it. Fourth is again, that is disturbing. Five is just me being aggresive but in a mutual understanding and consent that would be just to enact passive and pretend to be controlled by me nothing beyond that. Sixth is wrong if two people are not consenting, though I am not attracted to younger people. So, I can't say more. Seventh, not attracted but I think I find older men attractive. Eight, not at all in that category. But i know of heterosexual people in the dark web space where men cross-dress. It was unusual for me to know

    Why are there always only social supports in place for the predator and not the survivors of these kinds of people? Also what kinds of therapy helps? Because talk therapy isn't useful nor is EMDR. If you're always feeling like you're going to get revictimized you can't ever rest its like you're frozen in the danger state

      There are a lot of variables in therapy, and because people are unique, no one size fits all. A person needs the right therapist for a start. That might mean seeking out different ones before you find a therapeutic relationship that works. I agree talk therapy only goes so far. People need actionable steps they can practice in their lives. Healing is very hard work. and needs much persistence and resilience. Those things arent so valued in today's world, so that makes acquiring those skills harder. It can take a long time and you may not be seeing results, that doesn't mean it's not working. You just have to keep going, basically, and be very very patient with yourself.

    It's important to remember that having any of these disorders is not a crime! Acting on it is. Some of these can even be ok if done with full consent between adults (voyeurisme, exhibitionist, even "non consensual" roleplay) I remember an interview with a psychiatrist on the topic of pedophilia, telling how people would not seek help out of fear of being reported, and those seeking help would often be turned away, especially by private clinics, because the risk for the therapists if something happens and they didn't report the patient is too much to bear.

      It's not just a moral dilemma for the provider; where I come from at least, a mandatory reporter can be charged as well if it is found that they did not report concerns of abuse and it led to a crime being committed. I don't really know how you could make an airtight case for that, because it seems like it would just be a lot of hearsay, but either way the provider's reputation would be tarnished to some degree.
