in case of melanoma or cancerous skin condition even after seeing the picture in exam do we have to say best case or worst case scenario or can we just say we are suspecting its a e.g melanoma a type of skin cancer? and for confirmation and further assessing referral to Dermatology?
and in case of BCC if its on back of head do we need to refer urgently or routinely? and in case of urgent referral all the investigations including blood will be done where we are referring or some done by gp clinic?

You need to say you are suspecting melanoma, don't say best or worst scenario.

Thank you dr. In basal cell carcinoma, if the lesion is at the top of the head, do we refer urgently within 2 weeks?

Nice guidelines says refer BCC urgently only if it's causing problem because of its location or size.so for scalp I would say refer urgently to be on the safe side.because scalp is also a highly vascular area.
