Just CZcams some of the work environments in India 😮 One of my pet peeves is the forklift blades pointing upward when parked.

great talk Tim. Australia is still on the higher side than the US. Every monthly tool box since I can remember there is always an incident (non fatal), mostly due to not following process or cutting corners short or leniency. following process (not blindly) and taking a step back and thinking is mostly what's needed to prevent a good amount of incidents.
Sharing one almost incident was when the sparkies isloated a 480V panel by turning off the CB and tested the circuit to be live 😳 came to find that the breaker is faulty. would have been a total different situation if they haven't followed the process to test the isolation.
Do OSHA classifies these accidents with the root cause, Tim?

When they do an investigation I believe they do.

@@TimWilborne I wonder how many would be due to the reasons I mentioned and could have been avoided.
Just want to add to the CB almost incident. The CB was replaced with a new (on the shelf for years) one and the sparkies tested the circuit and found it to be live before turning on the CB. As per the CB OEM that was a known issue with those models. 😳

Most accidents are avoidable but it isn't always cut and dry whether they were or not.
