Proximal Humerus Fracture Weeks 5-6 | Active Light Shoulder Strengthening & Mobilization | Phase IV

- Welcome to phase IV of your shoulder rehabilitation. You are approximately 5 to 6 weeks post shoulder fracture and, hopefully, you have been following this physical therapy series since the beginning. Besides active and passive range of motion of the shoulder, you are now doing active range of motion with exercises like scapular movements, tricep kickbacks, active pendulum exercises and even moving the hand of your injured shoulder across your body.
I wouldn't say these are as exciting as the videos that you will be doing in coming weeks. However, mobility is crucial. This is not a time to just rest. You must continue to move your shoulder girdle. If you don't what mobility you can safely go into, you may lose it and it may be difficult to regain. So, put a smile on your face and cautiously go for it. Embrace the shoulder rolls, pendullum exercises and what your limited range of motion will allow you to do. This is not a no-pain-no-gain situation. Listen to your body and ride the line between shoulder rehab and shoulder burn-out. You got this!
A non displaced proximal humerus fracture is a break in the bone near your shoulder joint or fracture of the upper arm where the pieces of bone at the fracture site are separated, but not out of position.
A displaced fracture is when the pieces of bone at the fracture site are separated and shifted out of position. This type of fracture typically requires surgery. A break more towards the elbow would be a humerus shaft fracture.
Most likely, you had a fall on your hand and the force went up to the head of your humerus and it broke. If there is no separation of the bones, and only a break, then surgery is usually not indicated and this is where this shoulder rehabilitation series will be great for you. Welcome to your virtual physical therapy for your shoulder complex.
These shoulder rehab videos are not an excuse to avoid your physical therapist, medical doctor or chiropractor. Please check in with your healthcare providers, gain their knowledge and make sure you are reaching your healing goals and that you are ready for each step. If the statistics are correct, you may very well have osteoporosis and, along with the supplements, I like a more alkaline diet, some sunlight and, eventually, some weight bearing exercise. With the over age 60 population, the most common fractures are of the hips, then the wrist and the third most common fracture is the proximal humerus. Reached 5 weeks and started with this plan. At 5-1/2 weeks met with Orthopedic doctor. X-rays showed significant healing. I showed him what we were doing and he stated okay but do not push it or allow your minor displaced proximal humerus to go beyond pain thresholds...the bone is only partially healed. I have just moved to 5-6 weeks workout and am getting stronger although my arm is still relying on tendons rather than muscle. I start physical therapy at week 7 and thanks to Adam Fields I am going to blow the therapists away. I am 71 and am going to have full range of motion and my weight lifting strength thanks to this gentleman who I will never forget.
I wish i had discovered this sooner as its been 9 weeks since my injury yet i can only do parts of weeks 5-6 and 6-7. But better late than never! One issue i am having is my bicep tendon keeps snapping and sending shooting pain whenever i do the pizza circles and try and move my arm either to the front or side (when arm is straight). Im afraid this will lead to frozen shoulder if i have to wait for it to heal. Anyone else with a proximal bicep tendon issue?
Just at the end of 5 weeks, I an active 81 year old English artist. So very pleased that I have found your videos, they are so encouraging an full of compassion, thank you so much. Nothing like this with our national health service! When I went for a check up last week they were SO impressed with what I could do, not quite back to painting yet, but a tiny bit of gardening. My family are impressed too. One day I was aching and down, but then having to tell you “I am brave” made it bearable. So a big thank you. 15:49
My heart is full, Sue! Isn't it wonderful that you are doing your art into your 80s. How fulfilling that must be. I send you a big hug and thanks for your story.
@@dradamfields thanks, I’m not back to painting yet, but I will be. I have quite a lot of exhibitions booked in ( see website Sue wales artist) so I must get going. I broke the other shoulder 5 years ago and remember the tedium of the exercises. That was before I discovered you! 🙏🙏🥰
Beautiful work Sue : )
I’m sure any of us watching this video don’t want be here. But here I am. This video saving me on cost of physical therapy, have high deductible health insurance. Thanks so much!
I’m battling too with insurance, however; I’m not using my arm to brush my teeth yet. I almost have it to my face#BattleTime
@@Dani-mj2lh I go back orthopedic today. I’ll be 12 weeks post injury. Hoping hear I’m healed today. I’m back using arm five just working on full range motion. I bit bullet went physical therapy and doing these videos.
@@kellyw5304praying for your total healing. I lost my insurance first then my job due to my arm being broken and in a cast 4 months. I am seeing so much improvement and we just have to keep battling for total healing, extension, strength and stamina to get through this trying time. I’ve never hurt myself or had to take leave because of an injury. We are #Bravehearts
@@Dani-mj2lh I’m healed now but still working on stiffness and sone range motion. You will get brushing your teeth again. My accident happened August 4th. I’m sorry you having go though this as well.
Thank you, Dr. Fields, for helping me! You are a gem! 💎 Your knowledge, expertise, soothing voice, and compassion for the injured are so appreciated! May the Lord bless you!🙏🏼
Thank you so much for these videos! My range of motion has improved daily. The stiffness has improved significantly!
Bless you for your work!
Wonderful! You are what these videos are about. Keep on keepin' on!
Dr. Fields, Your videos have been instrumental in helping me gain my range of motion back after a proximal humeral fracture which then turned into a frozen shoulder. I can't say enough about your diverse series of excellent exercises/stretches and ROM activities! Your encouraging motivation with use of quotes and your own inspiration comments make me smile while doing the exercises. I'm still working on my ROM but without your videos, I wouldn't be where I am. I have a follow up with my ortho soon and I plan to give him your name/links if he wants to help future patients improve faster using your videos. I believe your videos did more for me than the physical therapy. Thank you for all the amazing videos that are helping me regain the function of my extremely remarkable and important shoulder!!
I’m at exactly 5 weeks post injury. I saw the NHS physio yesterday and he was amazed how much progress I am making. I told him that I’ve been doing the rehab exercises of yours right from Week 1. I showed him the videos and he was very impressed. Every day I am seeing and feeling improvements with the shoulder and aiming to get back cycling on the road again by mid June. In the meantime since two weeks ago I’ve been doing hour sessions on the turbo trainer. All heading in the right direction and am seeing the consultant for x-rays at the end of the month - hopefully it will continue to show the fracture heals very well and onto hard bone tissue formation 😊 Once again gratitude for these videos as I continue on the recovery and rehab journey 🙏🏼
You've been rocking this from the beginning. Enjoy the bicycle!!!
Thank you so much, Dr. Fields! Although I’m at week 9 post injury, this is where I’m at with my rehab and I appreciate your cues of positivity and reminders to celebrate the little victories immensely! Grateful for your compassion 🙏💫
We're doing this, Lee! Keep the feedback coming and go at your own pace. Sorry for your challenge but so nice to have you on-board.
I've been following your videos since day 3 of my shoulder break. I'm at the end of week 5, and my doctor is totally impressed. This video scared me a little. I'm feeling it, but I am improving every day. I can pretty much take care of myself again, even though my shoulder bone cracked in half. Thank you for your compassion and generosity. I'm going to keep going twice daily. You're a blessing. I just want to add that I am nearing 69, so I really needed this. Thanks, again!
Thanks so much for bringing the blessing right back to me. Let the weak man say, "I am strong"!
Thank you very much for your work. May God bless you
Thank you from The Netherlands. Your video's made all the difference! Thank you!
You have become a part of my day. I feel like we are great friends.
Thank you so much for these videos. I am coming up on 5 weeks since my fall. While my orthopedist is very kind, he has not given me any exercises. So grateful that you created these!
I have to stop and say THANK YOU for your videos. I found you as i was searching some exos to do to supplement my weekly physio appts following a surgery on a comminuted humerus fracture.
Your exos are easy to follow. And your gentleness, tone of voice, and attitude are a big bonus of encouragement. Thank you so much. God bless you
Thank you SO much for getting me through this. I have an awesome PT, but the snow prevented me from going. I am sooo appreciative of your exercises and encouraging words.
What a kind gift to we who want to help our body heal, these are exactly what I was hoping for. Much appreciated …Lark
Thank you so much…it’s been almost a year for me, range is good but mild pain daily. I believe my body can heal and with your guidance and my perseverance, I will be pain free one day. I am grateful to have found this, you are a blessing and I look forward to these valuable sessions. Thank you Dr. Fields:
I am so thankful that this world has compassionate care providers like you. I was frustrated as I was not given much direction after the two week visit and basically see you in 4 weeks and physio will start. As I am stubborn and determined person I was not satisfied with this direction as my lower arm and bicep muscles were killing me. I found your videos and I felt comfort and relief knowing that there was hope. I am just starting 6 week recovery and I feel stronger and more confident that I will recover. I have been progressing along for the weeks and I watch the next one which inspires me as to what will be next. I believe in you and thank you so much. Not gonna lie but the broom exercise scared me in this one. I will be a little better tomorrow. Thank you again.
Such a heartfelt comment, Thank you!
Yes, the broomstick can be a little aggressive at this stage. Go lightly the first few times through and make sure you have confirmation that you are mended before pushing it hard.
It's been 8 weeks for me but I may stay at this stage for a week or two. The NHS physio I saw last week in the UK gave me an exercise with 0.5kg weights (about 1lb) and it just killed my biceps tendon.
I was amazed how hard I found the hand from hip to hip movement. Biceps, triceps and forearm all screaming at different pitches. The broomstick ones are a joy! Thank you.
And it's also wonderful to be in the company of a kind and compassionate healthcare professional.
Thank you for the encouragement and guidance; I have hope!
Thank you SO much. If i hadn't found your videos i dont know what i would be doing as just been told to work on moving my arm. No mention of how to. Been 7 weeks now. It's great i can follow along and i love the positivity!
So happy to help you at this critical time. I encourage you to go for 100% strength and flexibility
Had my 5 week X-rays and started PT this week. Your stretching and videos helped me so much while I was waiting! Thank you!!
Thank you for your work! Your videos are very helpful.
Thank you so much!!! I am just out of ortez that I was have to wear for 6 weeks of immobilization after combined shoulder dislocation and fracture. Thank you so much for the detailed step-wise series of the exercises and very encouraging supportive comments! I now feel that I am on the way of recovery! Спасибо!!!
Make me so happy to give you some direction. You got this!!!
Thanks so much Dr. Fields. For your easy to follow exercises and encouraging words, I was afraid I'd get behind on my shoulder fx. recovery but I am determined to do the work every day! You are terrific!! Bless you! Dorothy😘
You're on fire, Dorothy!!!!
I tied my shoe today since May 2023, after 3 of your videos. Thanks Dr. Adam💕
That is awesome!
I had a minimally displaced humerus proximal fracture and I love your videos. My x-ray with my doctor is not until week 10 so I am having to pause at Phase IV but I can’t wait to resume Phase V in a couple of weeks. Thanks again for your videos!
I am about 9 mo. post surgery, and started hurting again. These stretches are so amazingly helpful! Thank you so much for providing these videos! 🥰
Thank you so much! This is very helpful and encouraging!

I found your videos a little less than 3 weeks after my proximal humerus fractures and have faithfully followed your exercises twice each day. It is now a little over 5 weeks since my break and I am doing phase 4. (Tied my own shoes today!) You have given me the confidence to keep pressing on. My physical therapist is very impressed with my progress. My goal is to be back doing my passion-swimming in the ocean. BTW, I am 72 years old and have Osteoporosis. Thank You so much, Adam!❤
Thank you so much, dr Adam Fields for these amazing, encouraging videos. I have a fractured left humerus, proximal. Was pushed of my bike, and crashed to the ground just over 4 weeks ago. So so grateful for my helmet, and despite osteoporosis no broken hip !
The physio I visited gave me an app with 6 exercises for the next 2 weeks, and I was not to worry about my neck, it would come good after I get out of the sling. Your exercises have made all the difference to my neck and swollen hand. Again a big thank you.
Doing week 3-4 atm.
thank you
You're welcome
My favorite serie so far! The exercises sequence is a really good foundation for further developments: the abs and shoulder blades engagement creates really reliable and safe frame. Light shoulder stranghtening exercises for triceps and biceps are great, othewise, just after being reliezed from the brace, heavy and weak arm is hanging on the healng shoulder. The comment "chest up" goes exactly on time! Thank you so much!!!
Thanks for sharing!! The "chest up" is my advice for walking also. : )
@@dradamfields Thank you!
THANK YOU FOR MAKING THIS SERIES

Had a bad fall last Nov
Fractured my proximal humerus
Had 2 metal screws put in top of humerus
Off the brace now
But left arm & shoulder is still very sore n limited movement
Cant afford physiotherapy sessions which are so expensive here
So......THANK YOU Dr
Hope you can make videos on how to strengthen our bones

As my surgeon said, my bone is brittle hence the fracture from such a minor fall...
With both arms having broken humeruses/humeri?! from a skiing accident, I’m so glad I found these videos even though unable to do the self massage. 😍 Glad I started at week 2, and will follow them to the end. Thanks again, and this 72 year old is healing really well so far, I want to blow away the PT guy at my first appointment next week with range of motion etc. Very grateful thanks again.
And way to be skiing at 72! I think you get a free pass when you're 80. Something to shoot for : )
You got this!

Thanks so much, and I did surprise my ortho and PT with the healing speed/ mobility, so stay as positive as possible and I know you’ll do great! Enjoy your one good arm😂💪🏻👍🏻, and research nutrition that helps to heal bones etc
#Braveheart after my fall; I was afraid of everything 💪🏽🙏🏽
So glad i have found your videos for my orthopedic didn't told me much just to wear a sling for 6 weeks. I will see him in two weeks and just hope i'm healing well. I'm 72 with a proximal humerus fracture. I've done all exercises and it helped me a lot. I'm on week 5 now but the exercise rolling my shoulders is causing a little bit pain. I hope you will answer my question i don't know if i should continu? Merci beaucoup ❤
Thank u
Thank u
So happy and so grateful to help you, Patricia
I am at 8 weeks and Fracture is still healing. This one was a good one for me except a couple motions where I could feel my SLAP tear screaming at me. I'll have to dial down that motion
Check out some ESWT for the soft tissue.
Thank you so much, Dr Fields! I have no words to express my gratitude here. I am in 5th week of recovery, after a humerus fracture surgery(posterior approach). I have titanium plates attached, which will be remain for ever. By humerus broke to two, diagonally. However, there are no joint fractures or any nerve damages, luckily. I am doing all your videos from week 1 till week 5 every day, along my regular physio session.
I am able to do almost every exercise, except that I cant bend elbow more than 130degrees. Any attempt to flex beyond, is hell. Is this normal in 5-6 weeks. When I can expect to get 145 deg flexion of elbow? Can you please suggest any exercises to improve elbow bending?
Once again, Thank you for all the motivation and positivity! You are God Sent and definitely my confidence booster🙏🙏🌟
Do these exercises apply displaced humerus fracture? Or are there different ones?
If you had a displaced fracture you, probably, had surgery and are good to do these.
@@dradamfields Yes Doc. Two fractures at the same time. Humerus and clavicle. Implanted 2 plates along with 23 screws, been one month since surgery. I am doing the elbow training to avoid stiffness. My surgeon will give PT exercises soon. Thanks for the reply. Highly appreciated.
Thank you so much Dr Fields although im on my 7 weeks.
Out of the woods and seeing the light. Enjoy the progression and don't slow down until your injured side is as strong as your un-injured side.

Dear Doctor. Thanks very much for this week to week wonderful exercises to heal proximal humerus fractures. Actually I had a fracture in my left shoulder in the middle of September and now, it is nearly 6 weeks after that. It seemed that it was not a serious fracture because two physicians agreed at the same time no need for any surgery; just resting and immobilization for a few weeks. Last week, I did a follow up X-ray and the doctor said my bone is healed already. However, I have been trying to follow your exact exercises from the beginning and now at the end of week 5-6, it is quite impossible for me to raise my injured hand above my shoulder level without getting help and support from my other hand. Doing theses exercises especially the ones for opening and raising the injured arm is super painful for me at this stage. Is it normal in your opinion or there might be something seriously wrong about my health condition ? Thanks.
Dr fields. I had a four part fracture and my biceps muscle is ripped away from my humerus bone and now located near my shoulder. No mri at all has been ordered. My arm is very deformed now what should I do. He had told me with or without surgery to put plate and screws in. It would not matter as nothing with fix this. He offered to do a replacement then I went home to think about what I should do. I called his office told them I’m coming back to get the replacement then he said it’s no longer offered. What!!! No pain meds but the pain has been so unreal I. My bucket area with blood still pooling. A sharp pain ran right thru my lower humerus and the pain was like a stabbing and then it just hurt so back near my enter elbow. What do I do. Definitely have popyee arm
Must admit pendulum exercise is ohe of the best.. as soon as i get stiff etc- i do some
Soon it will be 360 degrees : )
Thank you for these videos. I am now doing week 5-6. It is a hairline fracture from a fall. I still have a burning sensation on my arm. Is that normal at this stage. I have great range of motion.
I'm at the end of week seven and can't do 1/2 of these exercises. I'm having a lot a pain. Doc says I'm healing good on xrays. Should I be worried? My bicep area is swollen, throbbing pain, and was very bruised after fracture. Bicep does not seem to be healing.
❤❤❤
I'm at almost 5 week since I fell and broke my humerus. These videos are helpful. My biggest question is often to do them. Some places say four to six times a day others say to only do them three days a week. I have a call into my doctor about this as well but I haven't heard back as of yet. What do you recommend?/
Hi Cindy, I recommend 2 videos/day. Do one that is easy for you and one that is more challenging. Make sure and send the playlist to your doctor/PT:
czcams.com/play/PLx10M_1d-mu1fR1ucNbX57EyClU3aLiab.html
@@dradamfields I have done that and thank you. Since doing your videos (only for a couple days) I'm seeing more progress and moving my arm better than I was with another video I was trying to follow.
Hi Dr. Fields, so I broke my leg same time I broke my arm and i notice most of these exercises are done standing. Can I skip those?
Oh boy! Yes, you can try them sitting or skip the exercises that require standing.
ADAPT | IMPROVISE | OVERCOME
You got this, Gibbs!
Can we have something for distal humerus, with ORIF perhaps?
Hi Dr Fields, thanks so much for all your videos, so helpful, I’m 5 weeks post surgery mid-shaft humerus fracture also dislocated my shoulder. I’m doing PT & your videos. I find my shoulder so stiff/sore arm feels tight, my PT said I can use ice after exercising, is it ok to also use heat pad?
Hi Kate, Ice can be a great way to get through the rehab process and avoid a flare up. With the stiffness, there is also a very good case for using heat. An infrared heating pad would probably feel great and promote healing. See what your body is telling you after both. I would find someone that does ESWT to promote a more flexible fibrosis of repair.
It's a long haul and ice may work today and heat may be your saving grace tomorrow.
Thanks for being here and following the path of strength
Thanks so much for your reply, I really appreciate it.
❤
I am at week 6 today of proximal humerus fracture. From week 3 to 5 my arm skyrocket improved. But now I feel like since a week it's just getting more painful. I alternate daily between step 3 and 4. Is that normal that those movements hurt a bit do I go too far? Seeing my doctor this week for the final x ray...
Hlo
Exactly the same is happening with me. Was this pain normal for you?
Hello sir i broke my left humerus before 4 month and surgery also done .. can i do this exsercise now ?
I say, "yes" but go with gentleness and caution.
Sir after how many days sling bag be used for greater tuberosity fracture (undisplaced).
Do the videos. It's all in them and will take you on the journey back to strength

Hang in there, Jaya! You have a long road ahead (could be a year to full strength) but I am with you the whole way. Even after the Proximal Humeral Fracture videos there are strengthening videos. Start from the level you are able and make your shoulder a lifestyle of rehab. What you do in the next 6-12 months is key to the rest of your life. If you can, get some ESWT for mobility/scar tissue that would be a good adjunct. You got this!
What is ESWT? Thanks for your reply!
I'm on week 5 and I can barely lift my elbow it hurts when I lift it. I guess that's normal? I've been watching all your videos
Yes, you have a long road ahead but press on!
Make sure you get an x-ray confirmation that the two pieces have joined at this point.

Do you do if your bicep muscle is ripped away from your bone and all the way up by your shoulder and you’re still in dying pain all the time
Week five and six with no surgery and no MRI
And I have a four part fracture in my humerus shoulder
Sounds like a possibility for reattachment surgery before these but you could try doing this series very lightly
My arm is straightening out, just really sore and the swelling reducing, I bought Bone Up…
Sir I had a distal humerus fracture...can you do this exercise..I am post surgery..had plate fixed in my arm ...my hand is not properly openings due to imbolization for 20 days please tell
Do you have any videos on distal humerus fracture...?

Broke my left Humerus 🦴 luckily I’m into week 5 of recovery so the timing is perfect! These exercises are amazing. Will do these several times a day. My tricep hurts most and my shoulder pops often. The shoulder rolls help a lot.
Antonio! I'm so happy you found this series and the follow up videos. My goal is that this time next year you will have some nice toned muscles and be stronger than even before the injury. Thanks for checking in.
Has anyone been able to lift their arm fully ? I hope I am able to again 😢
do these exercises apply for shaft humerus fracture? please Doctor help me, it is been 10 days out of surgery please Doctor help me
I think that would be a good pairing. Make sure you get your doctor's clearance.
What to do for pain reduce?
I like ESWT number one. Other than that a heating pad, topical pain relievers or a massage gun after 6 weeks.
Thanks for the video. I’m with you from week one and now on a week six. At the beginning everything was ok, but on week five pain in the shoulder and wrist became unbearable. My wrist was fine before, but now I can’t move my fingers, I have pain in my palm and I can’t make a fist. My hand is badly swollen now and the exercise with the stick I am not able to do also. What is going on with my shoulder….? Regards from Europe 😊
Just put on deodorant with my healing broken arm❤ thanks so much Dr Adam🙏🏽
