There actually is such a system. It's called a rear view mirror. Look into it regularly and you'll see when things are not as they should be.

There is a warning system. It's called Eyes, Brain, Common Sense. There is no way in hell any driver worth their weight in salt is not going to know the bed is still up, you see it in the mirrors for Christs sake ! If you don't got eyes on mirrors at least once every 10 seconds you got no place on the road in the city let alone the highway.

What happened was he didn’t turn the PTO off and it built over time as he was driving but he wasn’t checking his mirrors to se what was behind him . He could have caught it if he was scanning his mirrors

I am an engineer and I don't understand why this still happens. We have the technology to make sure that this doesn't happen. Maybe they should make the fine for this particular thing better. Perhaps force a company that this happens to, to buy the technology. The bridge that was struck will have to be checked to see if it's safe to pass over and under, and of course it will have to be repaired.
Stop looking at the problem and start fixing it
This would require a review from a structural engineer before moving it to avoid the risk of the bridge collapsing when it is removed

The real tragedy is nobody alerting the truck even though they identify the problem
It happens a lot. Dump truck operators need to have these pto drive units cleaned and checked daily so they are not corroded or covered in dirt so they don’t engage accidentally. Also the electrical connections inside the cab need to be regular check spots to reduce the issue. Those trucks go up and down several times a day it is only a matter of time when the parts wear out or be corroded.
How about they quit giving licenses to new Canadians like it's candy on Halloween. You can feel a difference in ride when the box is 1 inch off the frame let alone completely in the air.

Here is an example of how accurate news headlines are. Wonder if the driver noticed the dump box just sheared off the back of his truck? Truck is no where to be seen, dont think it was towed away with 3 wreckers on location still.

I think that's the 3rd time in the last 10 years this has happened. Do the drivers not know that the box was not down? Was he / she (driver) checked for alcohol or substance usage?
