    It's not too late as long as they are mentally capable if say they can't use their hand they can have somebody help them hold the pen and sign it as long as they are mentally aware of what is going on

    If the Med directive is notorized, does it still require an additional witness signature?

      Hi! I think every state has different rules about that, but the form for your state should state the requirement. In California, it’s either 2 witnesses or notarized.

    Good morning, Sophia. Your advice has been TREMENDOUS HELP for me. Thank you for sharing this information. I am confused about something -- to initial the complex medical choices in Part 3 "Treatment Preferences" might not be a good decision 10 years from now. I want my agent to make those decisions based on my condition at the time. Is there a way on the form to omit initialing, but authorize my agent to make all the decisions that he/she knows would be best for me?

      Yikes! I read the entire document and it’s the most confusing form I’ve seen! Here is the form I reviewed. aging.georgia.gov/document/document/georgia-advance-director-health-care/download
      It seems that you can write in additional preferences in the “additional statements” section, for instance, you could write that after a certain age or year you want something else, and write it down. There is also a section called “effectiveness and signatures” and you could put a start and end date. Maybe you could make a second one with an effective date after the end of the first one. I’m not sure if that’s allowed, so please don’t take my advice on that suggestion. In 10 years you could also reassess your condition and see if you’d like to complete a new form….if you can. Did I confuse you enough? You may want to discuss this with an elder law attorney who is familiar with your states forms, or call your local area agency on aging to see if they can help. I don’t think I helped much, but I do hope you find your answer somewhere! Best wishes!
