An interrogator from Veritas scrutinized my citizenship certificate very carefully when I was first applied for a US security clearance by my employer (DHS - homeland security). I was then asked a series of questions to determine my loyalty to this country. I passed, of course. For the top secret & higher clearances, I believe one is subjected to a polygraph test and again every 6 months thereafter to maintain the clearance.

they don't call you, they send you a letter, and will let you know when the ceremony will be.

You need to pass at least 6 questions. 5 is failing.
