    Normally the liver and adrenal glands work together to maintain blood sugar between meals, but in the hypoglycemic person this mechanism does not work fully.
    Keto reactive hypoglycemia is simply when a person has unstable blood sugar and reactive hypoglycemia to start with, and the withdrawal of carbohydrate is too quick for the body too adapt.
    In a fully keto adapted and fat burning person, the fat and the ketones has replaced much of the energy previously derived from carbohydrate. In the early stages of a ketogenic diet when the carbs are reduced, the body may not yet have learned fat burning and so is left with out adequate fuel.

      I had blood sugars of 2.7 in the night 6 months after being on a keto diet. Never had hypoglycaemia before. It’s only at night though

    when you get yourself healthier using keto , paleo or carnivore diet and some fasting, the weight does come down and stay down. once you get fat adapted you don't tend to suffer the effects of hypoglycemia anymore. great video dr sten

      What u mean I'm on carnivore but I'm having nocturnal panic attacks because of hard hypoglycemia what can I do? I don't want to stop the diet but I don't want to die from hypoglycemia either

    I’m an all in kind of guy and what you are saying here is such a blessing! When I was told that I had become A T2D I went crazy! I came home and filled the dumpster with every carb in the house! May God forgive me for what I put my family through! My wife and kids suffered terribly! I have done great but they don’t share my discipline

      ​@mattg7977 long term you cut the caebs completely. Adding carbs or tapering slowly is a short term up to 2 months thing so you can taper slower and not have rebound hypotension of feelings of carb withdrawal as well. Gl!

      Not sure what are you on about guys. For last 30 days my only carbs came from cucumbers and olives. Rest was butter, meat and eggs. I will be surprised if I get 20 carbs per day. Based on what you say I should be dead from hypoglycemia already

    I know this video is old, but it switched on the light for me. Explains so much.
    I was going so well transitioning to keto, then went full keto and began to feel terrible. Hypoglycaemia makes sense along with low BP. 🇩đŸ‡ș

    I'm on week 4 of extreme keto and am suffering with major hypoglycemia. I went straight from high carb to almost no carbs in a few days. I don't want to eat more carbs but I suspect my adrenaline system is broken from massive overuse. I'm 42 and spent about 28 of those years in a state of high anxiety and depression. Keto has helped me so much, I want to stick with it but may need to re introduce a few things I previously cut out like dairy, tomatoes and peppers.

      Yes Alex Morgan, it sounds like you shocked your system. Like you said it is easier on your body if you slowly transition especially if you ate a very high carb diet.

      I've noticed far more energy since then without changing anything. It just took me a while to adjust. I still get sluggish around lunchtime in work but that might be related to being bored and stressed as I do not suffer from it when I'm at home on weekends at all! I've finally weighed myself and have lost a lot of weight so perhaps losing too much in less than 1 month contributed to feeling so weak on occasion.

    So now I understand what is happening to me! I never had my blood sugar tank before I went on keto, and I've been at a loss as to what to do about it. Thank you so much, Dr. Sten! I didn't know what to do, and it is ever so easy to lose weight on keto, since it takes all of my cravings away, so I didn't want to go off of keto. But you explained it so well to me. I will eat more low glycemic cooked carbs and hope by eating more carbs this blood sugar tanking (after I eat a meal) will go away. I sure do appreciate your taking the time to make this video.       @mattg7977 It works! I also had similar problem on my 4th month of keto. I did a lot of endurance training and that was wrong while on keto! Felt like having a heart attack. Went to ER and got all checks. My heart tested with ecg and result was perfect heart health! I got hypoglycemia , I got vitamins and medicines also add carbs to my keto (banana, raw oat and papaya also some root veggies). I am feeling good again now. Still continue Keto and will adjust accordingly when feeling not right!

    Another top notch episode, especially as a Type 1 Diabetic. I started Keto about 4 weeks ago and great results so far. Insulin down from 75 to 20, ketone level always between 3.5 and 4.0, 36 hour fast was great, and lost 11 pounds and 2 inches from stomach. Your videos are informative and inspirational!       ​@DaryaRyanPhotography no there isn't.. not yet. They are type 1 diabetic so the insulin they reduced is their INJECTED insulin, not blood insulin levels. 🙏

    Hello Dr. Ekberg, thank you so much for all this infos!
    I have adrenal fatigue... so I think it would be safer to start moderate/low carb, about 50g carbs from veggies and some starch and to lower very very slow and give the body the time to learn burning fat for fuel, and then slowly go keto

      I've had it since I was a child. Most doctors I've ever seen think it doesn't exist. . . I eat keto/atkins style and it has made a tremendous difference in how my mind works, my digestion, my hunger hormones, weight gain, my energy levels, and overall mood. I have found for myself that its better to get a lot of non-starchy green vegetables in the diet [mostly for fiber, additional minerals/vitamins, and to slow down digestion] and monitor my hydration and electrolytes like a hawk in addition to eating fat and protein with every meal. I DO NOT feel good if I fast, and will be off-kilter most of the day after skipping meals. I stick to three small meals a day and snack as needed.

    Thank you for this. I'm not diabetic but I feel pretty sure I'm hypoglycemic. I was in the store just now & my heart started racing & I broke out in a cold sweat. I thought I was going to pass out. I struggled to get to my car. I was living on a very high carb processed food diet. I went cold turkey & I have been faithfully following keto & intermittent fasting & have lost 20# for about 6 weeks. I have had episodes like this even while eating processed foods if i didnt eat often enough. I was planning on fasting today & and tomorrow, but after watching your video, I see that's not a good idea. I need to find a dr who will listen to me. My dr is basically a pill pusher. Because my stats are all good, he just tells me to keep up the good work.

    Excellent presentation as always Dr Ekberg. I think it would be very beneficial to provide video specifically on the strategy to become fat-adapted. We hear all the time how keto will result in becoming fat-adapted, but it would be great to understand the specific strategy.

    I am so thankful for your lectures. I run full 3day fasting successfully, but before that i suffered from hypoglycemia. all this time i hardly believed that i will not be able to skip certain eating times, but now i see that connection much clearly. it is already 2nd week of OMPD with Keto, and i feel much more energetic, also weight is going down what i couldn't move for even a kg for many years.
    Before that i was already familiar with water meanings so this part was easy for me. It was risky, but i felt i have to try such combination and i feel free. Thank you!

    Doctor, I really loved this video. I am type 2 diabetic and I have been slowly going keto for the last year. I still have sweet potato once awhile. The other day I was working out with my trainer and I noticed I was really shaky and had to drink a random grapefruit juice my husband had on his side of the fridge. I have never felt myself crash so hard. Could you possibly do a video or give any advice on how to avoid hypoglycemia and the dreaded crash. You touched on it a bit in this ☝ should I be glucose monitoring. My last bloodwork panel showed my type 2 was finally stabilized but my cholesterol was still high. Could this be a factor for my crash? Thanks for the advice!

    Love your videos, you are so clear, a great teacher! I was diagnosed with type 2 in September A1c 17!! Went on low carb and meds. Made excellent progress. A1c in Jan 6.3. Came off all meds in early December. Bloods in 5s - 7s IF 16:8 going well. Today had low carb lunch as usual, this evening BG 3.9. What is happening? why so low when used to low carb and not on meds? Can I trust what I'm doing? Blood went up after I ate. Would love your opinion, thanks

    I was diagnosed hypoglycemic in 2000 weighing 175 pounds and 5’10”. August 2021 now 50Y/O I weighed 255 and started the keto diet. Clean and strict. Now 4 months later December 2021 I have lost 47 pounds. 2 days ago I started fainting. I have fainted 3 times in the last two days. 3 months ago. September. I had a physical. All was good but insulin was a touch high and blood pressure was a touch high. Yeah I know. I’m calling my dr. And getting my blood work rechecked. Just want to let people know if you don’t do this by the numbers stuff can happen. I’m learning more everyday. We’ll see what happens from here.

    Yeah, thats my case. On keto for 3 weeks now and experienced ketosis twice so far, each time for around 10h. But mostly feeling like crap and barely can function normally. Yeah, but I have liver issues so maybe that is why. If I do just a small amount of carbs - everything goes back to normal. But still trying not to going back to carbs - ketosis is mind blowing.

    Hi Sten, I have a question. I have been clean Keto for 5 months as all going well. My blood Glucose has been coming down incrementally - at the start it was regularly 110-130 fasting so pre diabetic / diabetic levels although I was never diagnosed. Fasting levels are now generally 90-100 so much better. Last night however I decided to check my glucose 1 hour after eating and it came back as 76. I am not sure if this is something I need to worry about or whether it is just my body taking time to return to normality. It seems that I am still getting an insulin spike after eating to deal with carbs that I am no longer eating? Does that sound feasible to you? My meal last night was 1.7g of carb and 14.9g total carbs for the day. I would appreciate your thoughts on this - Thank you 🙏

    Good thing I chanced upon your video bec just this morning that i was surprised to see my blood sugar dropped to 3.5mmol. Was in high carbs and went to clean keto right away. Been Doing this for 3 wks now and trimmed my weight fr 60kg to 51kg but felt dizzy as days go by. Now i know to add little carbs and perhaps do paleo diet instead.

      @@Wisethinker527 yeh lol thats how it is. Anxiety plays a part in it. Try increasing your protein by a lot. Seriously 4 eggs for breakfast and get protein shakes (low carb) in your diet. Hopefully help your liver produce glucose

      1. Liver not producing glucose, do you have fatty liver? get tested. try my protein thing first
      2. You have high insulin, eat lots of low GI foods for your carbs and plenty eggs and high quality whey powder. That should reduce insulin in a few days but do it for at least a week.
      buy Choline and Milk Thistle.
      Im assuming you have non-diabetic hypoglycaemia.
      if youre diabetic then listen to Jason Fung on YT
      Either way, it can be fixed, its just finding what the cause is

    I was on clean keto diet with IF , on 4th month my body system crashed because after I passed 3rd month I started doing morning walk for 2 hrs and swim for 1-2 hrs. I made a mistake for not fueling enough before and after endurance workout! Also I forgot the rule of keto for not doing endurance exercise while long hrs dry fast! I was rushed to ER with heart palpitations, very weak body almost faint, whole body I felt like dying from heart attack. Doctor did ECG for my heart with excellent result! My heart health is perfect! They put needles and have two bottles of medicines/vitamins into my body! After 2 hrs treatments in the hospital. I am now recovering with vitamins, some digestion pills, introducing more carbs to my keto ( fruits like banana, papaya, raw oat and green vegetables). Still I am continuing my keto but if I am into my workout I prepare my body with fuels also post WO! I feel much better now.

    I agree with you Dr Ekberg., I do okay on lower carbs like 65 gram total carbs, I do not do well on a ketogenic diet. My HBA1c is now at 5.4% from 7.6.% Three years ago. I have lost 85 lb,yes I hope to 20 more lb, but more to lower my blood sugar and improve my sleep apnea.

      Yes please, I have an Hashimoto's an autoimmune disease and i was doing keto for about 8 months then i been getting Hypoglycemia and was hospitalized 2 times. I'm terrified of the symptoms. DR. Eric berg says to fix it you need to Up youf Protein but keep carbs low. A nutritionist told me to control it by having 10 to 15 carbs with Protein and healthy fat to keep it stable. I'm finding that i have to eat more to do this so fasting is not helping. Please someone...does anyone know what to do about Reactive Hypoglycemia do to Hashimoto's and how much carbs, and how much should i be eating? This is life saving information...đŸ˜ąâ€

    I have been doing low carb, then keto for five months, and suddenly, I am having hypoglycemia - nothing has changed in five months (except I lost alot of weight). I wonder what's going on? 10 years ago, I used to have hypoglycemia, and I got rid of it by following the paleo diet. It worked for weight loss and "cured" my hypoglycemia. It was great to be free of that, and not have to carry snacks everywhere. However, I did alot of recreational eating since the pandemic, gained weight, ate poorly - went back to low carb, then keto, lost weight and it was great, but now, for the first time in years, I have that hypoglycemia back! I would sure like to know why.

    Never checked my blood glucose levels before, but I recently got a meter to check ketone levels and since it can check BG levels as well I thought I might as well check both. I freaked out when my BG reading was 73! As I understand it that is on the verge of hypoglycemia, and that wasn't even fast. I ate dinner about 5 hours previously though to be fair I haven't really had more than about 2g carbs all day. If that. I'm going to need to keep a close eye on it.

    Thank you for this!!! Exactly what I needed to know. I started keto last March and 1week into it a all of a sudden got instantly dizzy and hyperglycemia and had bad panic attacks and ended up in the hospital! I was out on anti anxiety meds on and off the last year and the last one ( mirtazapine ) I just got off of and I’ve been having hormonal issues and mainly sweaty feet on and off along with hungry every two hours or so the past few weeks. đŸ€ŠđŸ»â€â™€ïž Hope to be able to figure out what I need to do to reestablish hormonal / gut balance. How long does it usually take to rebalance your system if you’ve had such a crash like this? Any tips?? Thanks

My story is a bit complex because after my keto crash I also got covid which added more stress to my already stressed out body but I’m happy to say much better now đŸ™đŸŒđŸ˜Œ when I tried the keto diet i basically went carnivore with it. I think because i went extreme with the diet along with yeeeears of being a high carb eater and junk food eater prior it was just too much for my system. đŸ€Ș and then when i got covid and that just made things complicated for me. Of course everyone’s needs and journeys are different but that was mine and super grateful I’m much better đŸ™đŸŒđŸ˜Œ i believe everything is figureoutable. 🙂 not sure what your story is but i hope you feel better soon!!

      @@mattg7977 a few things I did that I feel helped me heal were lowering stress where I could. Being out in nature, grounding, meditation were a few helpful tools i used, pacing myself especially with exercise ( so slowly increasing and starting with things like gentle walks, stretching or qigong) and eating more Whole Foods like fruits, veggies, rice, potatoes, meats and natural fats like avocado. And of course patience. 🙂

    Dr. E., your example of hypoglycemia depicted in the video was a blood glucose of 15. Diagnostically, where does hypoglycemia start? I'm also aware that someone used to higher blood sugars can be symptomatic of hypoglycemia without actually being hypoglycemic - could you comment on that either here or in another video?

    What about while breastfeeding and/or pregnant? I was keto (mostly carnivore) before my first pregnancy (for around 6 months) and had no issues with hypoglycemia, even after abruptly cutting out carbs, but I added carbs back in when morning sickness struck, and I was averse to fat and protein. Since that pregnancy, I've tried countless times to get back on keto (through two more pregnancies and breastfeeding each baby without stop), but each time I get down to around 20g carbs (even after slowly easing out of carbs), I end up with severe hypoglycemia (with vomiting, extremely blurry vision, extreme weakness, almost passing out, etc.). Is it possible for breastfeeding to make 20g simply too low? Or is it more likely that I'm still not easing out of carbs slowly enough? I really want to be keto for my health, but I feel like I'm failing when I can't seem to stick to 20g carbs or fewer.

    Hi, (I have a question about weight and blood sugar levels) I started doing the keto diet while weighing 74 kg as a supporting person for someone else also on the diet... in the first 5 months I lost 12 KGs, and am worried that I'm now under weight. I'm trying to eat a bunch, and use resistance training to try to build muscle. I feel great most of the time. But there are times where I have low blood sugar, between 3 and 4.9 mmol/l. I then eat or drink a bit of sugar and take about an hour or two to get to a functioning level. I don't want to take sugar though. Amy ideas?

    As I have some worries about future Alzheimer development, I have with great interest studied Professor Dale Bredesen's protocol which seems to give some hope about slowing down and even reverse the dementia. As a part of the protocol he recommends a mild keto-diet, underlining its importance to counteracting the insulin resistance. To start my fight against Alzheimer I have therefore the last month changed to a keto-diet and I have only seen very positive consequences and I feel great. My weight has also decreased from 82 kg to 77 kg - (I am 72 years old and 175 cm tall), so I think that is OK - even if it was not my motivation for changing to keton fuel for my cells.
    Now one question: When I try to push hard - mostly when I swim - I feel weaker and my head can start turning around a bit. For short term efforts can it be better to give the body a glucose shock before you start? Is sugar a more instant energy source, which can help you in the startup? On the other hand you must explain to me why, for really long term high effort sports - like 90 km skiing in the Swedish Vasaloppet - the serve sweet blueberry soup to the runners?
    Excuse me for this long and tedious question.

    Great information! Thank you! I needed this information but I still struggle with hypoglycemia. I was raised on real food my entire life. I'm 62 and weigh 125 lbs. Never more than 130 in my life. I want to continue to eat this way (Keto) but my blood sugar drops so low, get the shakes, sweat profusely, can't stand up. I throw some carb down and I come out of it. What's up? Feels like I'm forced to eat food that I don't want. Any ideas?

      I'm a nurse and I should have seen the signs. I've been on keto for 6 months and I'm faithful to the regime. I've lost over 50 pounds. I've been doing Intermittent fasting. I was feeling a bit woozy last night. It occurred to me to take my blood glucose level. It was 52. The number freaked me out a bit. I asked myself "What would you do if you were your patient"? I knew I needed carbs but I didn't want to slam sugar.( the nursing protocol is orange juice or this sugar goo in a packet). I ate some lentils with tomato sauce. Good enough. My BGL was 87 in a few hours. I'll be more careful in the future.

    Hello Dr. Ekberg,
    Thank you for the wonderful nutritional education. I am 52 and eating a keto diet/IF and just started measuring my blood glucose and ketone levels, ketones were .09 - 1.5 and 81-86 glucose in the mornings. I decided to do a prolonged fast ( I have done a 10 day about a year ago, and a 5 day a few months ago without a problem, but no meter) At 34 hours into fast (morning) readings were 67 glucose, 2.0 ketones; 48 hours into the fast at 10:00 p.m. my glucose reading was 52, with a ketone of 4.8. My question is - is this okay? I realize this is low blood sugar, but since my ketones went up - providing an alternate fuel source - I am hoping this is not dangerous. Feeling tired, but that is usual for me on day 2 of a fast. My mom had hypoglycemia so, just a little worried, but would like to continue with fast if I can safely. Any help would be greatly appreciated.

      I get the exact same as you. my glucose was 3.8mmol/l (68.4mg/dl), ketones 1.8 It seems that when my ketone are very high, my glucose drops. A few weeks ago my Ketones were above 8 but I dont know how far above because the Keto Mojo only goes up to 8.0. I didn't think to check my glucose at the time.

    Hello i need to know last year i done proper keto for a few months and i experienced no fainting whatsoever, this year this month or two i’ve had 4 random experiences of near collapsing, my doctor said it may be low blood sugar, can u please tell me then is it a possibility that did i develop hypoglycaemia ? Another thing i should mention is that this year on low carb diet i have cut down my daily calories more than when i dieted last year, so could this be another reason for these faint experiences?

    This might be me after almost one year of low carb I have severe chronic hypoglycemia symptoms that no doctor could ever explain to me. I need to sit all day and get exhausted after very short walks. I have heavy chronic disbyosis, dead teeth’s, knee pain, my liver might be so weak due to the toxins from the bacterias that the liver can’t regulate the sugar levels and I get stressed out due to too much cortisol, I have literally all the symptoms of hypoglycemia, just a few steps before going to the hospital. Ate two potatoes this evening and already feel better and have less cravings. I hope adding starches will ease my hypoglycemia long term and that I can one day solve my dental and digestion issues. Wish me luck. Would be so nice to have a doctor like him where I live.

    So...my situation is that I once had low blood sugar, and many episodes indicating such (shakiness, weakness, brain fog, etc.), but NOW I have been struggling with elevated blood pressure (was high, but only reaches elevated last two weeks), and it's the worst feeling I have ever felt. I often feel like I could check out of here at any time, yet here I am. Haha. I have been fasting and such and doing Keto, but my research led me to discover that low blood sugar could actually cause HPB, which I thought was weird to switch from one to the other. Now I'm confused about what would be safe for me to do at this point. What should someone in my case do ?

      I'm currently three weeks in and my heart beat is up but at night it's worse, beats heavy. I've been doing 12g of salt a day. I had to eat some potatoes and toast the other day and felt much better. Next day I was up around the house doing DIY. Lots of people talk about electrolytes, but I'm not convinced as how comes as soon as I had carbs I was fine. I think some people can't keto adapt or have metabolic issues, I'm one of the unlucky ones. I think I'll keep trying but up my carbs a bit just enough to get me through then during day eat no carbs until I feel funny. This normally takes me out of Ketosis but by the evening I'm peeing ketones again, which is when I feel.weird, but it might help my body transition in time. There's not many Keto videos on this and this is the only video I've seen which relates to our issues.

    Could you also do a video on reactive hypoglycemia? When intermittent fasting 16-8 I'm often experiencing lowered blood glucose after eating in the afternoon, and it makes me feel fatigued and sluggish after eating. Have I become too insulin sensitive? Am I eating food that causes the insulin to spike without increasing blood glucose at all? I have never been overweight or insulin resistant, I have just been eating gradually more and more low carb over the years as a preventative measure. Lately I have also started intermittent fasting and eating two meals a day. I typically eat lunch around 2-3 in the afternoon and dinner around 7-8 in the evening. Now I'm considering extending my fast to a 20-4 pattern and eating nothing before dinner, just to avoid that feeling of low energy level after lunch....
    Update: I was still hanging on to a few of the old carbs when I had this problem. Now I have been completely keto for the last 6 months or so, and I have gradually had less problems with reactive hypoglycemia during this time. I guess it just takes a while to get properly adapted...

      If hypoglycemic after eating, insulin is high. The only explanation is insulin is high all the time, when you say it goes low without eating. That is hyperinsulinemia if not insulin resistance. This video is what you explined đŸ€—

      Yes, the quality of the carbs could be the problem. If it's processed bread, pasta, high glycemic index foods/fruits or smoothies, it's too much to be digested in a short period of time and pancreas releases too much insulin... makes us RH'ers too tired. Eat a lot of veggies, 7-10 cups spread over the 2 meals, eggs, low fat meats, healthy fats such as nuts, seeds and Avocados, low glycemic fruit like berries. I find when I do those things, and if I have whole wheat bread, minimize my bread carbs to one piece, I have a better time with it (up to 20 net carbs/ meal), it helps.

    I watched because I am insulin resistant type 2 diabetics, and working to lower blood sugar levels as well as cholesterol. Wondering what is a bad or dangerously low blood sugar number. I feel fine but my blood sugars are 49. What is unsafe?

    Fasting for 6 days. Tonight my Glucose 36 ketones 5.6. It’s the lowest I’ve been so far. I wasn’t feeling badly, just some minor cramps in my calf muscles. Decided to switch from fasting to time restricted eating once a day.

    My Hba1c was ideal at 4.8 , but I had bad arthritis pain , tried a low carb no carb mostly meat . My pain was gone ! I was using stevia in my coffee everyday and found out it causes hypoglycemia if youre doing a diet like that . Dang it ! Just cant win â˜č

    Thank you for the info, I'm trying to go slow. I have reactive hypoglycemia, and its getting better. I can fast longer and I'm more stable. But when If I try to workout my blood sugar can crash from even a short workout. And a fasting workout is better, workouts after a meal are more likely to make my blood sugar drop. Any info for me? Anything helps, been dealing with this for a while

    Do you know anything about keto and severe Hypoglycemia from Rue-en-y Weightloss surgery?

      Only that after weight loss surgery you change a lot of factors so you have to use a lot more of trial and error. The same principles of fat adaptation and blood sugar stabilization still apply though.

    I’m 22 and definitely suffering with hypo. I keep trying keto and I just can’t. It’s definitely hard

      I'm on the same boat. I'm 42 though and I can't seem to function well after 10am if I don't put something in my stomach. I'm from Nigeria and here, our foods are very heavy on carbs. This is hard for me too.
