Hello, I am a former RN and I worked in surgery and I believe the humeral replacement explanation at 22:00 is incorrect which would make the correct code 23616 and not 615. The prosthetic replacement is replacing the original humeral bone and not a previous implant. Just like when a knee replacement is done, it is to replace the diseased knee joint. When they replace a previous implant it is called a revision such as code: 23473

with the question of a 55yr old smoker 2nd to the last list of questions if we use the 2022 CPT code book this question will be coded differently.
Yes. It should be 32408-RT.

For the example with the 55 year old smoker, I used knowledge of the guidelines about signs and symptoms to eliminate a automatically be cause we have a diagnosis and you don’t code for those when DX is confirmed. I know the DX so that automatically eliminates C as well. To quickly decide between b and d I used the knowledge of conventions that a 4th character of 9 is usually pertaining to unspecified and a 1 or 2 as 5th character is usually going to indicate laterality, a 0 is usually unspecified laterality. That lead to the quick elimination of b. So I kind of worked backwards from the ICD codes to rule out quickly before looking at the CPT codes. I shocked myself cause this was the first time that all made sense to me so quickly. This is fun now that I’m grasping the coding concepts. I was able to think it through and not even touch my book til the end to make sure C34.11 was the neoplasm/etiology/laterality I needed to code the neoplasm to the highest specificity.

For the trans bronchial biopsy question, since the lung has 3 lobes, if we did 1 biopsy from 1 lobe of RT lung and 2 biopsies on lobe 1 of LT lung and 1 biopsy of lobe 2 on LT lung, would we then report 31632 twice since we are taking biopsies of 2 additional lobes? The language of the code is confusing me when it says ‘each additional lobe’. Does that mean cumulatively or do you use it for every additional lobe after 1 that’s biopsied?

This is from 2020 or 2021. The description says re-released so these are not new, but I hope you've found the instruction useful.

23616 internal fixation ( implant placed )
Last question 1.44 : patient returns now... so secondary eliminate primary
