If you don't want mushrooms, don't bring them up when ordering at all. They only put on the toppings you say, so if there's any background noise, the "no" part in no mushrooms could be completely unheard. Employees there are used to people only stating what they DO want on their burger. This is also why you should always check your receipt as soon as they hand it to you, so you can check to make sure your order was properly rung up.
