6. Two (2) Recommendation letters by either of the applicant’s former professor/adviser/employer/supervisor and others in a position to evaluate the applicant for graduate study.
7. Photocopy of Professional Regulations Commission (PRC) identification card (for MAN, MAEd, and MSSW Student applicants).
8. Duly-certified working experience of at least six (6) months (for MAN and MM Student applicants); at least one (1) year working/teaching experience (for MAEd, MSSW Student applicants).
