The landscape of athlete recovery has evolved, and one method in particular has gained a considerable amount of attention.
Welcome to episode TWO of "No Stone Unturned," and today, we're delving into the practical applications of IV therapy and supplementation as a whole.
We'll explore the nuances and answer the burning question: Could this be the missing link for athletes like you and me?
At the end of each video, I'm going to provide a score based on three crucial aspects: Personal Enjoyment, The Scientific Evidence, and The Practicality for the Average Athlete. You'll be able to compare it to the other dimensions of this series.
I hope you enjoy and please share your thoughts in the comments.

Glycerin/Glycerol can help with hydration issues. You have to take a lot (~30+g) but if you have a hot environment or doing hard work, this can keep some water in your body.

Hey Mitchell, I love this series and am looking forward to the future episodes! Big fan and love what you do.
I just want to point something out regarding the last study you talked about:
That study was specifically referring to contrast-induced kidney injury, it wasn't about hydration as a whole. "Contrast" is a dye given in medical settings prior to scans/xrays to help the radiologists see more clearly. When using contrast, there is a small risk of kidney injury and proper hydration is shown to help lower the risk by "diluting" the dye.
The study does talk about oral hydration vs intravenous hydration, although it is purely in the context of preventing contrast induced kidney injury. It is not simply about "oral hydration vs IV hydration for kidney function" as you mentioned.
Just wanted to clear this up!

Fluid balance is so critical for performance. Many years ago I tried to go completely fat adapted as an endurance runner, and I managed to do so, but it took quite a while. I couldn't figure out why I could not get consistency in terms of how my runs felt. Some days would be awesome, other days would be like I was starting from scratch. Until I figured out it had to do with fluid volume/hydration. On a strict ketogenic diet, I tended to wake up with lower blood pressure than usual. And then when I went running, I felt like shit. I tried adding a little salt water before my run and immediately changed my performance and my consistency in performing. Since then I have often noticed, that when my eating habits (now doing more of a body builder style low fat higher carb diet) were poor and I didn't get enough to eat the day prior, I have that same feeling. My endurance and overall energy is just gone.
I put a lot of that down to hydration and fluid volume actually. My working hypothesis is that if you are slightly dehydrated, and you start exercising, that you require a little extra blood volume on account of your muscles and expanding lungs etc. When there isn't enough blood volume, and by implication, you are dehydrated on a relative basis, your performance simply suffers.

I remember the Stoltmans getting their IV hydration prior to a big comp (ESM maybe?). Luke cramped up really badly and underperformed.
Not sure what they received but throwing a bunch of unbalanced electrolyte solution (theirs nothing "normal" about normal saline)!seems like a great way to throw off your lytes before a show. Even Plasmalyte A has missing electrolytes and some supraphysiological levels. Depending on the molecule, your kidney has to do something to excrete the extra (Na/K pump, dealing with hyperchloremic acidosis ie: H/K exchange etc).
Drink water before an event. Maybe IV mid event but the plasmalyte A. Also, that amino acid lasting one month thing is garbage...

So does that mean that we normals getting scammed with the other supliments we take or does Mitch know something we dont? Would be fun to know what Mitch thinks about other over the counter suppliments we normally use. Great video.

It's not ethical for him to discuss it. He is a medical professional, a licensed clinician so that is a no no. In general I know I can't answer why nobody talks about it, but it's safe to assume they're part of the training protocols.

Okay real question, if Creatine is used for the first 5 seconds of energy production. Say you're a baseball pitcher, how would creatine
Effect/behave/respond over repeated bouts of short explosive exercise.
How much creatine can one store in the body, and how quickly can one bring creatine into the muscle? Is it 6 hours from ingestion to being in the muscle?
It seems to me like a MLB pitcher would have either an increased ability to store creatine and or ability to absorb it assuming they are regularly exposed to large doses of many years of training.
It's one thing for people who lift to see an advantage, but what about those who just like to be explosive and helpful?

I find it a bit odd that he said completely different things about NAD in the first visit than in the second. Because everything he said in the second visit is correct and most of what he said in the first visit is debatable. Not saying that's anything to point at because people can make mistakes, especially on camera. Same thing with the Vitamin C, he's right that it's water soluble. It's very dubious that it'll last a month. You'll probably pee most of it out in an hour if it's in the blood. And whatever's in the cells is unlikely to make it to the end of the month.
Also wanted to comment that even though exercise physiology is part of the curriculum doctors go through, it's really only to find specific diseases because there's some background biochemistry that has application to diseases and a bit for atypical environments (altitude, diving, starvation). Otherwise, basically not touched at all. So defo if you are a competitive athlete go to someone that specialized in exercise physiology. And know that for 99%+ of people the benefits from additional supplementation are probably negligible unless you have a known deficiency of something. That's pretty well researched from what I've read. But for athletes who need that extra 2-3%, one can argue it's worth trying given that the side effects aren't too bad (although not zero).
On water replacement one important thing to remember is a lot of the food you eat has water in it. Most shouldn't be replacing tons of water loss one-to-one with water. That's also not great because you sweat out important electrolytes and if you don't replace those you can get side effects. Usually cramping and fatigue but depends on the degree. You have to eat as well.
If I had to guess number one for most effective tool for strength it has to be sleep. Good sleep and sleep optimization do wonders for general energy and recovery. It's also something often missed because people are always looking for stuff they can actively do to be better rather than letting their body take care of it. Plus - and I'm guilty of this - phones are awful for sleep and I always have my phone with me.

Coming from a country with socialised medicine (which I wouldn’t give up) I must say many things are a little more “conservative”. There is no financial incentive to push anything and things that are tried have an expectation that they actually work. I respect the freedom of countries that allow “take my money” medicine where for the exchange of money anything goes but I do feel it opens the door to things that either don’t work at all or actually do more harm than good.
In this care it sounds very interesting. As you say human trials on sports performance are limited so the only way for there to be sufficient data is for people to be the Guinea Pig.
