What is the reason of decreasing the dose of ICS from 400mg (TWO puff BD of BDP) per day in step 1 and 2 to 200mg per day (combination inhaler of ICS and LABA 100/6 1Puff BD) if the patient is uncontrolled at first place? Shouldn’t the dose of ICS remain the same if it’s combined with LABA?

Good spot -- this is to do with "Fostair" being extrafine particles than typicle Clenil - this makes Fostairs more potent - have a look at www.medicines.org.uk/emc/product/5076/smpc/print#gref look under posology it is very useful - Best Wishes
