    Here's a good Brain Story for you Mike. My Grandmother at the age of 70 had a Brain Aneurysm, after being operated on by one of the best Brain Surgeons in the country at the time (Nicknamed "Christ" by fellow doctors and nurses for the miracle like surgeries he could perform). She ended up in the hospital for 5 months, nearly dying a few times. "Christ" said she would never recover and be herself that her brain must have been damaged. She made a full recovery, her personality was exactly the same and she lived to be 90. Even at an older age the Brain can fully recover.

      @@faybelle2991 I understand there is a reason to hate your mom, and she likely deserves the comments laid upon her, but you are also your own person, and you have control over your own behaviours. I know there is a spectrum of shit and you might have a massive pile of shit, but always remember, if you want change, YOU must change. If you decide to let your feelings over your mom affect your own personal behaviour, that's on you. I grew up with a sister with a severe, untreated mental health disorder and she was used as punishment for me. She was severely abusive and I was an emotional mess and no one defended me. My mom and sister picked on my weight to manipulate me once I hit early puberty and my weight skyrocketed. It was a cold, isolating home. Ironically, I became a nurse and saw the suffering, and I mean really, really realized; there are a shit ton of people who suffer. Most people will never see the severity of the suffrage and nor would they want to. I wouldn't wish it upon my worse enemies-- not even my own sister who I hate to my very core. Alas everyone has their own huge pile of shit and a level as to which they can control. My suffering didnt stop when my chronic pain issues started a few short years after university. I'm no longer able to work full time and I'm a financial disaster in the vicious cycle that I can't control. But I'm still trying and not giving up on me. Most people just make their pile worse because of their own behaviours and they believe their own BS coming out of their own mouth, to justify their destructive behaviours. Mental health is so important but it is sacrificed. "it won't happen to me", up until it IS you. So you get to decide to burn in the flames like everyone else, or will you rise above and take ownership of the life that is yours? I really hope you will choose to fight and die trying your very best. I am really sorry that whatever she did, it hurt you so much. But just know there are people rooting for you. There are people that will take the time to write this because they care. And when you put the effort into the right place, you very well might be able to have a little control over something in your life, even if it's merely your attitude. Be well.

I am very close to how you were but a lil smaller but I am so sick people making comments to me like I'm huge or like you said having shorts that used to be so cute on me and now I can't even find a pair to fit me. I used to love shopping now I don't even want to go to the store and if I do I pick the biggest size don't even try it on and deal with it. I went from size 5 to size idk because I don't even want to know the real number. I hate looking at pics of myself now or me before. Thank you for your video it's time to make a change.

    Am 66 years and have been exercising on and off since my teens. I hit on IM after reading about it and am now on 5/19 routine for a year now. All my relatives,including those 15 years younger than me are diabetic and hypertensive while I'm not.

    It is downright scary the state of medicine is in. The fact is, we can't trust conventional medicine. A lot of doctors are just not trained in nutrition. The pharmaceutical companies are in charge of medical text books.
    The only doctors I trust are those who went to med school before 1973, but most are now retired.
    The best approach is to be educated and stay up to date on the latest science and find a doctor who is willing to listen to you and will even take a look into the latest published research.
    I have one child with mosaic Turner syndrome and another with autoimmune issues. I found doctors who were willing to listen and take an integrative approach that went a long way to optimizing their treatments. I don't dismiss conventional medicine, but I also do not fully embrace it when there are other less dangerous approaches that not only treat disease, but restores health.

      Late to the party here but I will tell you this having recently gone through postgrad (engineering, not medicine, although I knew medical students and this was a top 100 university in the world)
      Many med students today are dumb as bricks, engineering students too. Sure they can regurgitate textbooks and crunch numbers but they completely lack critical thinking, and if you ask to many questions they just go "you don't understand cause you don't study it".
      We are deeply in trouble in the future. BUT remember this. Your health is in your hands, the best way to stay healthy is through PREVENTATIVE (think eating well, exercise, proper rest, etc. etc.) care and actions, versus REACTIVE (modern medicine).

      My goal is to fast at least for 72 hours in a row then do OMAD on all other days. I want to give my body time to really heal and repair. I recently tried the 72 hour fast and only made it to 63 hours. This week is a new start. I started my fast last night at midnight. I am a nurse and work crazy hours. I am so happy to see a woman at the age of 73 doing this. So many women just give up and WE are not going to be one of them :)

    My husband and I have been doing IM fasting for 14 hours for last 8 months,and from 30 units of insulin he took before he is now on only 4 unitsperday.its really working.and we walk 8kms daily too.

    Some good insights:
    Autophagy is always occurring; it’s a sliding scale of intensity and efficacy.
    Intermittent fasting (compressing the feeding window) alone favorably affects autophagy.
    As you get older, autophagy lessens so you need to fast longer.
    Autophagy is intracellular. The cleanup occurs within the cell. It is not a process of eating whole cells.
    At 36 hours and 72 hours of fasting, autophagy intensifies in muscle cells.

    I fast for 72 hours from Monday to Wednesday and 24 hours fast from Thursday to Sunday then repeat that for almost 4 months.. Also on carnivore diet and doing tabata workout daily.. and feeling super active everyday...

    I am using bio-kinesiology (muscle testing) to determine my optimum fasting/restricted eating regimen. Best practices are often less severe than we think. I am getting consistent positive results with routines that are not difficult to maintain... daily 6 to 8 hour eating window, monthly 36-45 hour fast. Weight loss, energy levels and autophagy are all at optimum levels. It is worth noting that I am 69, have no major health issues, eat mostly paleo (I do love my daily dark chocolate fix), walk regularly, take supplements, and use herbs and essential oils to maintain my health. But I've been 20 to 60 pounds overweight my whole adult life, and tried all kinds of diets, including a brief fling with prescription diet pills. Intermittent fasting is working for me like nothing else. I feel great, and for the first time, in control of my weight management. I love how naturally flexible and compatible with all my lifestyle choices this practice is. Finding our own best routines is a personal journey, and using muscle testing can be an invaluable tool

      I am a neophyte at all this. Recently dumped massive amount of fat when I inadvertently triggered a spontageous great rendering of lipid xenobiotic toxins...specifically obsenegens. I went from a size 22 pants to a size 4 within less then a week...
      A WEEK! I am in need of autograghy, even though it has like I unzipped a Sumo Wrestler's fat suit custome and step out the tiny me I was when I started bare handling (sans masks or any known precautions-- flourides decades ago)

    I am 65 years old and have been exercising all my life. I have found that the 8 16 mid fast during the week works well for me maintaining my weight. One day of the week I have one meal for the day (soon I will do two days week). I also run 3 miles 3 to 4 times a week. I find or feel that i hit autophagy on the days I run. Now after seeing this video I will need to increase fasting but remain hydrated. Thank You

    You mentioned having a cold. I have not actually come down with one single cold or sore throat in 5 years, since I began taking simple acidophilus. It's a miracle.

      I'm turning 75. Until a month ago I hadn't experienced a cold in more than 20 years. When I recently experienced a mild cold, it took me a day to understand what it was. I don't do anything special to avoid colds.

    Camera tips, don't have auto focus on. You don't need it and your slight moving back and forward won't be noticed. I've seen so many videos where you are out of focus. Try it, set to manual and focus yourself at the start and you will have a nice clear shot!

    I'm a T2 and fasted 11 days to begin; I went from 100 u before to 30 u after with better control. My second fast was 4 days and I went down to 20 u. Third fast was 3 days and went to O. Am now doing 3 days a week, OMAD the other 4 days; no insulin.
    The whole thing took me under 2 months so far. I've been a T2 for over 3 decades; on insulin over 12 years.

      My mom: Same, basically. (Although, she didn't go over a few days at a time.) She went from being poorly managed on insulin+ to healthy numbers in a few months. Neither of us will ever be diabetic again. Sure, my mom and Jackie are bad asses, but y'all can all be healthy bad asses, too.

    Autophagy is an intracellular process. I've been laboring under the false impression that lymphocytes are going around eating old and damaged cells, but that isn't true, based on this information. How fun to learn something new!

      Best way to think of it is autophagy is like demolishing an house and moving the remnants into a steel waste bin. The lymph system is the truck that fetches the steel bin and transports it to the dump for disposal (bowls and bladder)...

      If the bin is empty when the truck comes...not much gets done by the lymphatic system. If the truck doesn't come regularly...you get a lot of debris spilling over...and maybe autophagy slows.
      So ideally a healthy system of both is best.

    I am 64 and doing variant fasting . My body and mind is returning to when I was 17 yrs of age . Feel love very in my clothes & all the tiny spots on my arm and upper legs all cleared up and I am now 7 stone 6 pounds ; I was 8 stone 5 lbs . First couple of weeks was truly tough , however by understanding Autophagy I managed to keep going - cheated occasionally when I felt really low ( ate an odd date or too ). This is my 4th week and truly , I can’t eat back the way I did 4 weeks and beyond . Please try it - simply cut down about 25% of your daily meals and then skip a meal and then slowly move into a little more restrictions with the amounts abd frequency of eating .
    I keep asking why am I shoving so much food down my oesophagus and you not my tiny stomach .

    I just stumbled upon this video! Wow!! Mind blowing. I did not know autophagy was an intracellular process! The majority of medical or health gurus perpetuate the idea that it is a “packman” effect, kind of like phagocytosis, and that after a certain amount of time the whole body undergoes this process. Anyways, thank you for the clarification. I appreciate all the info you put out on here. I know you said that autophagy occurs at different rates in different tissues for different people, but I was wondering if there was a “general” guideline that indicates at approximately what time tissue cells undergo autophagy. For instance, bone and muscle start at around the 17 hour mark, liver at 18... and so on. (I just made up those numbers). Thank you 🙏🏻💕

    Intermittent fasting is not just good for weight loss. I'm currently bodybuilding on a 18/6 fast and I'm in a caloric surplus, slowly putting on weight. I do intermittent fasting for all the other benefits that come with it (increase HGH, increase testosterone, insulin sensitivity, anti-inflammation benefits, improved energy levels, staying lean and maintaining lean muscle,...). Feels great and it's convenient for me personally. At first it wasn't eating getting enough calories in with a 6 hour eating window, but giving it some time I have become accustomed to it and have no problem reaching my caloric surplus now.

    Been doing hardcore IF while losing weight. Monday to Thursday I eat at 1PM and 6PM. Friday and Sunday I eat at 3PM. I was astonished how fast I adapted to this.
    3 months I lost 6 inches off my waistline.

      @@nicola4594 nothing on Saturday. I'm going IF for another round of this for a few months this winter. I like doing diet sprints where you go as hard as you can for 1 to 3 months and then take a break to maintain and learn how to eat for your new size. I needed to get from 370 to 200 and didn't have the willpower to do it all in one go. Right now I am about 240 so I am getting close.

    Autophagy starts around 18h-20h I do omad 23/1 and I start weightlifting around 18h doing total body workouts x3 a week to help increase autophagy.

      I can tell you that high levels can happen when fasted. About a year ago I did a T and insulin test. I was about 18 hours fasted when blood was drawn.

      I'd say that is a symptom of insulin resistance. It has the reverse effect in women. Too much insulin in the blood will lower estrogen and increase testosterone.
      Go low carb and fast. Get insulin down.

    Re your dad's hospital experience: emerg doctors are great a acute care; they are not so good at chronic care. They will save your life but you need to care for it afterwards if you want to be healthy.

    A trick I can advise with your camera focus issue: use autofocus to begin with as you would normally do. Once you are in focused, TURN OFF autofocus, so your camera wouldn't find some other area in the frame to focus on. Assuming you are not going to move back towards your bookshelf or move your head towards the camera.

      Probably then need to talk about how it is the most satiating of the macros, how low-fat and low-carb diets result in equal weight loss when protein is equated, how people need MUCH more protein as they age, and the effect of specific gluconeogenic and ketogenic amino acids.

      Joda Gordon, not disputing anything you have written. Protein can definitely be very satiating, but that is part of the problem... it’s easy to over consume. Therefore, raising insulin and glucose. On a low carb or ketogenic diet, it's not just the carbohydrates that need to be monitored.

      Folks are way over estimating their daily protein intake. It is causing a myriad of health issues as people get older and they tend to treat each symptom independently instead of looking at the root cause which is spiking insulin too frequently of a daily basis. All chronic diseases lead back to insulin.

    Thank you for these insights into these autophagy studies Mike, really enjoyed this stream! :)
    I became more flexible with my fasting and I achieved a better balance for myself.
    Monday I do two meals a day, morning and afternoon with no snacking between, Tuesday-Wednesday-Thursday I do 22/23 hour fasts, Friday I do a 18/6 or 20/4 fast and after that I go into a 36-48 hour fast until Sunday when I break my fast with a small meal high in protein, 0 carbs and low in fat and go back again to fasting.
    And at the end of every month I do a 72 hours fast.
    I don't advise others to do it like this, I just wanted to give an example of what works for me, but I encourage people to experiment for themselves and make their own schedule. I train 6 times/week gym+ cardio and feel great.

    What I've read is that ketosis starts in a healthy immune system at 14 to 17 hour of fasting. However, when the immune system is impacted by metabolic syndrome/Type 2 diabetes then ketosis may take up to 3 days of fasting to trigger. ... Ketosis is required to trigger autophagy ... another method is to test for ketosis is to measure the ketone levels with a ketone meter - the mildly annoying aspect of this is that ketone level may vary over time as the body adjust to fasting levels so ketone levels drop over multiple attempts of fasting.

    I knew alpha fetoprotein sounded familiar! I had a child with neural tube defect years ago:
    “Your doctor will most likely check for spina bifida by first performing these tests: Maternal serum alpha-fetoprotein (MSAFP) test. ... But abnormally high levels of AFP suggest that the baby has a neural tube defect, such as spina bifida, though some spina bifida cases don't produce high levels of AFP.”

    I am 73 in very good health, No meds, on Carnivore 3 months 4 hour eating windo., lost 24 pounds need to lose about 25 more yet.Walk 4 miles 5 days a week, thinking of adding 36 hour fast once a month just because i was a yo-yo dieter for about 50 years, so I thought that might be a good plan for me????

      If you are fat adapted and comfortably OMAD I am envious. I started keto with a 48 hour fast and man was it hard. After 5 weeks I am at 20 gr, carb , 18/6, and walking everyday. My sleep is good too . I need to lose 30 lbs. I am 67 and on no meds. My diet was always organic but way too many carbs and snacking. I am not doing a longer fast for a few more months.

    I never get colds. When you feel a cold coming on take some good quality eucalyptus oil and put a drop on a Q-tip. Then run the q-tip on the inside of each nostril. Do this morning and night. I haven’t had a cold since I started doing this years ago.

    Autophagy does not necessarily kill a cell but the removal of cellular waste material. High insulin hormone levels decreases autophagy and low insulin hormone can increase higher levels of autophagy. The only way to decrease the level of insulin is to avoid eating protein and carbohydrates - essential one needs to fast.

    Mike - firstly, I like you have a MSc in Nutrition - although I think the USA abbreviate it as MS (?) - I really enjoy your presentations they are very professional and more importantly they are not dumbed down and you are not afraid to hit the Biochemistry etc head on - or more importantly treat your audience like 5 yr olds ...thank you for that ...
    I would like to ask when you present the studies could you either link or reference them in the show notes below and possibly annotate if they behind a paywall please ?
    Also, Like me when you get revved up / excited about a subject you tend to fire off at 90 miles an hour - I am the same and often leave my friends staring at me glassy eyed wondering what the hell I am talking about lol...so could you please try and consciously slow down a little as sometimes it becomes too overwhelming and important things are missed so need to be replayed several times ...not a criticism but just a gentle request ... don’t get me wrong I couldn’t present info like you even if I wanted too...lol
    Please keep going with your show it’s deffo one of the best and although in some areas I have drawn some slightly different perspectives to you, I do feel that you and I could comfortably sit in the same room reading from the same hymn sheet ...

    I've been mostly 16:8 IF-Keto and had good success with it, but have been wanting to supercharge it somehow and also settled on 36 ish hours once a week, 3-4 days straight once per quarter. Very happy to hear this format validated by someone else!

    For those asking how long do you need to fast the answer is: Are you just looking to increase autophagy or are you looking to cure your diseases? Because if you're just looking to increase it, do what he said. If you're looking to cure your diseases then you need to fast until your disease is gone.
    For me personally I did a 2 week fast, which made my hyperthyroidism and myasthenia gravis disappear, but it did not make my type 1 diabetes disappear. However, after about a few years they reappeared again, showing that I didnt actually fast for long enough to completely cure myself. Id say probably a month to cure yourself, but after all my research 3 months seems to be the perfect amount of time to completely cure everything. It all comes down to you and what you'e willing to do.
    However do not fast as a type 1 diabetic without medical supervision. I tried to fast many times as a type 1 diabetic without medical supervision and ended up going into DKA and nearly dying every time except for 2 times, so its very dangerous. If you are a type 1 diabetic test your sugars regularly dont just assume you're fine because DKA can happen out of no where. Also test your ketones regularly, test before your fast to see what your normal ketone levels are and then check your ketones around every 4-6 hours while fasting. If you do end up going into DKA, sodium bicarbonate (baking soda) its great for lowering your acidity but putting a half teaspoon into half a glass or 4 ounces of water(read the back of the box / bag of baking soda, it gives you directions for homeopathic use. Its actually what they use in hospitals to treat acidosis, they put DKA patients in the ICU on a sodium bicarbonate saline solution via IV drip. They also put you on an insulin drip, so to simulate that you can use long acting insulin. Using these two things while going into DKA are how I was able to stay out of the hospital and remove the acidosis at home. I would however not try any of this yourself and do this with medical supervision, but if you go against my suggestion there's something you can do to keep you out of the hospital.
    Everyone else should be fine as long as you dont have some form of acidosis that could kill you like with diabetes.

    Re: 24:46 Hi Mike. Alex did not specify whether he or she was type 1 or type 2, but from my own experience as a type 1 who fasts without difficulty, the answer is yes. Of course, do not just dive staright in, and do discuss this with your physician. However, Dr Richard K. Bernstein (who has been type 1 since developing the condition at age 12) mentions in his book "Diabetes Solution" how when he was training in medicine, he'd sometimes go as long as 36 hours without eating with no problem. His CZcams series "Dr Bernstein's Diabetes University" provides a lot of useful insights into many different aspects of (primarily, but not exclusively, type 1) diabetes. As well as Dr Bernstein's book (and his shorter version of it "Diabetes Diet"), I can recommend Volek and Phinney's "The Art and Science of Low Carbohydrate Living" and Davis and Runyan's "The Ketogeneic Diet for Type 1 Diabetes".

      Just don't be afraid of calories keep them up so you don't slow down your matabolism.
      I'm in the middle of a 48 hour fast:
      I normally do OMAD for 23 with a one hour feeding window.

    I can fast forever, but damn what do i hate the muscle cramps that comes together with it. Even on magnesium for 2months every day and on L carnitine and allot of sea salt with iodine in it on my food i'll still get it

    Okay, so mediocre forms of autophagy are occurring concurrently all over body. So the question now changes to: When does the most ""meaningful"" autophagy start? Or, we need to now shift our focus on differentiating b/t ""qualitative'' vs ""quantitative"" perspectives on autophagy....and their respective efficacies. Maybe mitophagy is king.

      I had been comatosed for 11days, lost sight and hearing on left eye and ear after being run down by a truck. Brain was haemorrhaged despite an expensive Arai helmet and it was shattered, held together by the stickers and glue. Cut long story short, are our lives shortened by going through this kind of head injury? Wish your dad speedy full recovery
