Hooks angle what you have told is wrong its 45 degrees only not 135 degrees. 135 degrees is on the other side. Between the bend, it should be 45 degrees. please check and update.
Effective depth calculation is wrong b/c to calculate it we use this formula D-conc cover - dia of stirrup - dia of main reinforcement /2 there fore
It becomes 450-40-8-20/2=392mm
You said wrong in curtailments of bottom bar. After 0.8 L distance from the column curtailed bars are used in the tensile stress zone. But u said 0.8 l of curtailed bars from column.
