  • Dr Choi goes over a case where a patient has periodontal disease and is going to lose his current implants and his teeth. He also discusses whether it makes sense to place All on 4 dental implants on the same patient.

    Great info!! I am 66 and when I was 46 I had a total hysterectomy as treatment following being diagnosed with endometrial cancer. Since this was and estrogen feeding cancer post op I could not take estrogen replacement therapy to reduce my risks for recurrence. Within 6 months post op I began to develop periodontal disease and my dentist suggested I see a periodontist. At my periodontal consultation due to deep pockets the periodonist said that ruled out deep cleaning as a treatment option, and he suggested periodontal laser surgery which was relatively new at that time. I went forward with the laser surgery over the course of a year because I chose to have one quadrant done at a time and my periodontist wanted full healing of that quadrant before he did the next quadrant. Sadly year 4 post op my periodontist took ill and had to give up his practice. The periodontist that took over his practice I did not care for, so I had all my follow up with my general dentist 4 x year. Over the next nearly 20 years my general dentist extracted my 3 remaining wisdom teeth that became very loose along with a heavily filled upper molar that caused me frequent pain that was next to a crowed tooth which did not require a root canal. Since that extracted tooth was in the rear and gap not visible when I smilled as I have a low hanging upper lip, I choose not to have an implant or bridge to replace that extracted tooth. Additionally, my periodontist had planned to place a splint on my two front lower teeth to stabilize some mobility that had developed, but he stopped practicing before he could apply the splint which was subsequently done by my dentist. From then until now we all did the best we could to save my teeth. Last year my husband and I relocated from R.I. to Florida which meant I had to get a new dentist. Due to developing what my new dentist describes as an ongoing root infection of a non root canaled crowned upper molar and increased mobility of several other teeth she suggested I see a prosthodontic affiliate of hers that has a periodontist in his practice to see what possible restoration he recommends with my now significant bone loss. In all honesty, I am very discouraged since it seems so much of my life has centered around putting in so much of my time and effort in an attempt to save my teeth and those efforts failed me. And I think back to when I was a thumb sucking child which my parents could not break me of that habit, and if they could and I did not refuse to have braces to correct the damage I had done with my thumb sucking, I would not have had to go through my long adult dental journey in trying to save my teeth. Over the years every time I see a child sucking their thumb which I only did in my sleep, I wanted to tell their parents to do whatever it takes to break that habit or their child will likely suffer lifelong dental issues like I have had. I appreciate your educational videos especially this one which has given me some encouragement to continue my dental journey which will hopefully lead me to a successful restoration and not have to spend so much of the rest of my life in a dentist's chair.

    Thank you so very much for the information. To date, I have not been able to get any information on what my situation was, re gum disease and bone loss, from one of the “top” periodontists in Miami..He did a through exam and then told me he would speak with my dentist to see how much my dentist was willing to do. A couple of months later (!), when they finally spoke, all my dentist said was the periodontist wanted to remove all of my veneers (65%+- of my top teeth) treat the gum disease and then implants. That was virtually all I was told. I’ve learned more watching just a few of your videos than I have during three appointments with my dentist towards obtains info. Thank you again.

    Hi great video as usual. I have been following you as I have been diagnosed with severe collapsed bite due to very excessive grinding of teeth. How does having surgical posts installed deal with collapsed bite and excessive grinding. How do you reset collapsed bite with implants.

    • @northtexasdentalsurgery
      @northtexasdentalsurgery  Před 2 lety

      we have seen quite a few patients like you. having a collapsed bite that occurred due to excessive grinding doesnt affect me when i place your dental implants. however when it comes to restoring your teeth with the actual prosthesis, we have to re-establish your vertical dimension of your bite (what it looked like decades ago before you ground your teeth down). without going into details there's a way to re-establish a correct bite even though you're currently in a collapsed bite. its done with patients who get regular dentures all the time.

    I have PD, and I’ve recently seen two dentists for consults.
    They both want to first do a deep cleaning, then extract several slightly loose teeth, and one very loose tooth, and do bone grafts for those, then after healing..place implants. I’m talking 6-10 teeth/implants.😭
    I’m so concerned about the astronomical cost, and putting implants in my mouth that’s got PD. I’m so scared I’ll wipe out my savings and end up needing full dentures in 5-10 years anyway!
    They both really advise that I keep my other teeth, even though I have PD and lots of fillings and crowns. Sigh… I feel so lost and anxious 😬

      I don’t have a way to send my scan, unfortunately. It’s at their office.
      I’ve since consulted with the oral surgeon and he felt I should do a full mouth extraction. I’m extremely scared to place the implants for dentures in my diseased gums.
      This is so overwhelming and scary. I’m 59.

    • @kellyschultz485

      That happened to me. The most affordable answer for me was to have the bad teeth extracted, and then get a partial. I got a prepaid dental plan which also gave me 30 per cent off the partial. Partial was 700. I chose an Oral surgeon with IV Sedation and the cost was 1200. If you can just have Novocaine, the price is cut in half. I want the all on 4 on my bottoms now, because more teeth had to be pulled. I had to choose Dentures 10 years after I had the partials. Bottom Denture doesn’t fit, so I am looking for a solution. This was my Complete Teeth Journey. Of course I would have loved to have all on 4, but it is not in my budget.

    I do have PD. I did see a few oral surgeon. I live in small town hard to find a experience oral surgeon. They said my case is complicated. One of them will do for me upper jaw for 25k PMMA. I told them I want zirconia but they said heavy and make a noise. I'm so worry in few yr later will web out my money. I have a copy of x-ray. Please help me what I can do. 25k for pmma is way too much. i've put many patients with perio disease in zirconia in the past with no bone loss for years. i'm not saying pmma is bad. it can just stain and its hard to keep its esthetics up for years. if you get pmma, it should be priced below 20k. if you have any questions you can email me at contact@northtexasdentalsurgery.com

      @@northtexasdentalsurgery that doesn't really explain what it is - i had an, unknown at the time, an accidental tbi when i was 2 - this put all my skull bones out, some were fractured and i had little craniofacial extension (flat profile) and most adult teeth taken out at about 7yrs by dentists who didn't know any better (was a long time ago) then found a dentist 15yrs ago who knew about the temperomandibular dysfunction and systemic distress ( article in private dentistry 2010 by justin glaister) but didn't know about the early damage that put my tmj out.
      as the repair of my head and body is happening now (very odd at 72), muscles relaxing downwards into the body (read the article - blew my mind) but now i have gum loss at the very places where the growth would have happened to allow room for my adult teeth. that's why i am asking what it is - and how i can help the gum to grow? see if you have any answers for that hahahaha how nuts is that - incidentally - i never became feminised - had miserable painful menstruation and eventually ovarian cancer - presumably from a compromised/ twisted/ obscured posterior pituitary
      please read the article cos it is very important for a dentist to know that and as you are on the internet trying to get info to people, thought you might like some more info about our head and teeth
