Wow I don't often see a demonstration with a patient that is actually presenting with symptoms/issues! I hope things went okay for him. Great professionalism and demeanour from the doc.

He has feeling in his legs but the lack of response to the reflex tests indicates a possible tendon or muscle problem . Poor mate must have a real hard time walkin

I don't know if that's the case with this video here, but several old exam videos had information removed from the description after they realized the video got a lot of views from people outside the niche. Some channels even blocked comments and others even blocked videos.
He most likely used/ uses a wheelchair the majority of the time. Since he probably has difficulty/or very little mobility with most likely standing and walking he's not putting the leg muscles to use.
Assuming some sort of paralysis of lower legs or neurodegenetative problem like MS or MNS. Not sure though but it's usually somthint with the nervous system.

This a case of charcot Mary syndrome in which there is a bilateral symmetrical proximally more than distal paresis with no sensory loss

Clenching teeth potentiates reflexes is a patient is hyporeflexic. Helps to differentiate between reduced and absent reflexes
