This toilet has a great flush but the performance isn't that good. I imagine it would be perfectly fine on normal use though.

These are the only good Cato toilets that I know of so far, besides the St. Thomas Creations Marathon style ones.

It looks exactly like a mid to late 1990s American Standard Cadet to me. It's not a bad flush for it being a Cato.

I agree with Tom, that 99% surely looks like a Maxwell bowl
Maxwells have a 7" distance from the tank bolts and this has a 5.5" distance so those two together would be pretty much impossible. It couldn't be a Mansfield either because those use three bolts and it can't be an Orion because those have a raised spud.
This is definitely a Cato bowl, Maxwells have a larger trapway and a different shape at the back of the base

I'm pretty sure that's a Gerber bowl. Tank is mismatched.

It needs a new handle more than anything. The flapper works fine.
You've got a Cato tank on a Gerber Aquasaver/Maxwell bowl. And the handle is not compatible with that tank.

I imagine if stays unflushed a lot.
