This song is about being on the road away from family and loved ones.
The blonde in the bed is his guitar while the mistress is the stage and the music industry.
The spotlight refer to the media and he's not alone because he's with the band living his dream but he's still lonely because he's away from the family.
"But tonight I sleep on a bed of nails" means that he's sleeping alone in another hotel room on the tour instead of at home.
