    Toponin is the cardiac contraction regulatory protein . Levels are increased in the presence of infarction. Absence of cardiac troponin will rule out myocardial infarction.
    There are two types of troponin .Troponin I and Troponin T
    Levels increase with in 3 hrs of infarction , However High sensitive Troponin levels are increased with in 90 mins.
    Diagnosis of myocardial infarction needs to things. Presence of ischemia which is seen as the presence of chest pain and EKG changes.

    Hi Dr, i had Trop T= 50ng/l, no chest or resp symptoms, with normal EKG, normal MIBI + normal effort test. i m runner 30km every week . what do you think about the Troponin result? thanks

    I was in the ER on Tuesday with severe chest pains. EKG showed Left bundle branch block with left axis deviation. My troponin level was at 9.8 and normal value says 13.0 and below. One hour later my levels were below 6.0. They said test was normal. The next day back in ER for continuing same chest pains. Troponin level was below 6.0. One hour later troponin was below 6.0 again. So my question is, is it concerning that the level was 9.8 then lowered later. Why was level higher initially with the heart block and is that a indicator of heart injury at that time. I know the level was still in normal range though. Thank you.

      Initial cardiac biomarkers developed are basically enzymes like CK MB. Spectrophotometric assays are available for CKMB, as they were able to measure the rate of reaction (by measuring the difference in light absorption)
      Measuring range of analytes which use spectrophotometry is usually high when compared to immunoassay
      Larger molecules like troponins ( present in extremely small quantities) are highly immune reactive( antigenic). Their ability to efficiently bind to antigens makes it highly sensitive and makes it ideal to estimate even at lowest possible concentration. Hence immunoassays are the tests of choice for troponin estimation.
      For you to use any test for quantitative estimation it should be feasible for standardisation (ability to detect levels at various concentrations)
      As far as estimation in mouse ELSIA is the best available standardised method, i believe.

    I got chest pain and it was first time then dr gave e a test of toponin i and the result is 1.0pg/ml . So what about this??? Please explain. I'm 27 yrs old and I'm on tension now. But doctor told me i am safe no attack on heart. But still i want to know details??

    Hi,
    Just had 2 high troponin I levels of 3. Chest pains up jaw and into left arm.
    Ecg normal.
    I have antiphosholipid syndrome. Would you be concerned re this level please?
    The doctor thinks its angina, at the moment but I've been given isosorbide mononitrate to help the pain, which doesn't go away and comes on at rest.

      The test strips consists of embedded Troponin antibodies . Blood of the patient with myocardial infarction has Troponin antigens. When antigens bind to antibodies they form a complex which is indicated by the presence of a positive line .
      Add venous blood sample is the slot provided to add sample and wait for 15 to 20 mins for the lines to appear .One line is the control line (C) . The other line is test (T) line . Presence of 2 lines in both C& T indicates a positive test. Presence of only one line indicates a negative or invalid. For a test to be valid the line at the " C" region should be positive.

      $$$$$$$$$$ That's what its about. They told me a number 1.3 and then 2.3 and it is from this number that they wanted me to stay 2 days 2 run more test. This visit alone was 2 grand. They also did an ECG and x-ray and refused to discuss that information.

      Cardiac Troponin levels are highly specific and won't be increased unless there is an injury cardiac muscle . However the cause for the cardiac muscle injury needs to be evaluated.

      CK MB was used previously but not any more . CK MB is present in all the muscle and in the brain too. Cardiac Troponin are present in the cardiac muscle .Hence troponin testing replaces CKMB test.

      Troponin's cannot be measured by spectrophotometry . Immunoassays are the test of choice.

      Negative test rules out the presence of heart muscle injury at that point of time . If there is a strong clinical suspicion serial tests are to be done over next 24 hrs

    My husband had a level of .006. Cardiologist told us there was no reason for concern, although his surgeon suggested his Troponin level suggested a mild heart attack. He had an ablation 3 years ago and says he has no more symptoms. He was admitted into hospital due to flu like symptoms and a fever. Even after admission into hospital his fever lasted for a week. They were unable to find infection or reason for elevated Troponin. Can you offer any thoughts on this?

      I believe that's a Troponin I value you r talking about . Normal cut of range will be < O.04 ng/mL for Trop I .
      Interpretation will be based on cut off values provided by ur laboratory . Providing the units and the cut off values of the test will be helpful in providing feedback

      Troponin levels are not used for determining the severity. Its primary use is only in diagnosis.
      Severity of the disease can be determined by the findings on ECG, Echocardiogram, angiogram and the current clinical findings only . Echo determines ejection fraction which indicates the pumping ability of the heart , ECG shows the conduction abnormalities and angiogram detects the actual number of blocks, if present .

      Levels seems to be elevated . Saying that you need to cross-check with the normal reference ranges provided by ur lab as they vary from method to method

    Last month I got slight chest pain and a pain in my left hand. I was suspecting it as a MI and admitted. Initial proponin test shows as 0.2 and they took me to an angiogram. But final conclusion was no MI or not block found in the vains. They didn't have any explanation. Is that a possible situation and what are the next level of actions I must take?

      Angiogram will be a definitive test for diagnosis of MI . Serial levels of troponin needs to be increased for MI .

      O.012 ng/ml translates to 12pg/mL . The levels seem to be less than the normal range of 14 pg/ml .However mentioning the following could be helpful
      1. Presence of symptoms and EKG changes .
      2 Cut off values / reference ranges given by ur lab.

      Echo says it's suggestive of ischemic heart disease ( Heart attack) . How ever only your cardiologist can give more insights . As far as Trop T is concerned , if a repeat testing is done , a 50 Percent increase from the initial value of 12 will be clearly suggestive of heart attack

      The levels seems to be increased which indicates some injury to the heart muscle. Increased troponin levels are not be used by themselves to diagnose or rule out a heart attack. Clinical history of chest pain, physical exam and ECG are also important. As far as BP is concerned , Systolic is on the lower end however diastolic seems to be stabilized . Pulse rate and rhythm are also significant in assessment .Your doctor may also need to see whether the troponin levels from a series of tests are stably elevated or show a rise and/or fall over several hours.
