Full form of CA 125 is Cancer Antigen 125.It is a tumor marker for ovarian cancer. Normal levels are less than 35 U/L. Increased levels are seen in ovarian cancer.
CA 125 is a mucinous protein present on the epithelial surfaces of the eye, respiratory tract, ovaries
CA 125 is specifically seen increased in epithelial cell cancers. More over it has higher chances of being positive only in advanced stages of cancer .It is NOT USED for screening of ovarian cancer as it is not sensitive and specific. It needs to correlated clinically and always should be correlated with other diagnostic tests.
Role in Endometriosis:
It is not a sensitive marker for endometriosis. Levels are greater than 35 are especially seen in stage 3&4 of endometriosis. Levels greater than 100 U/mL indicate extensive pelvic adhesion's. However the most accurate test for diagnosis of endometriosis is Laproscopy(Direct Visualisation).
NEW MARKER
HE4 - Human Epididymis Protein 4 (HE4) is a relatively new marker for Ovarian malignancy and indicates gene over expression.
It is used to monitor recurrence & disease progression in patients with Epithelial ovarian malignancy.
HE4 when used in conjunction with CA 125 helps in estimating the Risk Of Ovarian Malignancy (ROMA) in premenopausal /
postmenopausal females presenting with an adnexal mass.

i had chemo again and continue continue no stop
Had one done yesterday and came back 12.7 so normal. All my blood came back normal. Not having a diagnosis sucks but at least the chance my "massive" cyst filled ovaries are not cancerous is about zero which is good. I wish there was just one frickin answer though since pcos is unlikely by the types and size. Blah.. Hoping surgery soon will help the pain. Peace and love my beautiful slightly malfunctioning sisters.

Hi, My mum diagnose with stage 3 ovarian cancer with BRCA2 positive, On End of July 2019 we found the CA125 is 900 over
after surgery it went down to 130, then 50, 30, 19, 16 now is CA125 is 10 and it is before the 5th chemo. My wife’s period came one week earlier and she had mild fever so we went to doctor.
We made transvaginal ultrasound but the the doctor said he see nothing wrong. Ovary were not big ( left 2.46-1.45 cm / right 3.82-3.21 cm)
We made also blood test and the results were:
Ca125 - 94 iu/ml -higher than normal
HE4 - 49.29 pmol/l - each is normal
We are in vacation and he said he believes is ok and we don’t have to be worried but when we arrive to our country to make further test.
The chances of being cancer are low ? Knowing that he found nothing wrong in ultrasound and HE4 is normal. Only ca125 is 94

Fibroids can increase the levels of CA125 . Please don't get too much stressed about the levels of CA125 . The levels needs to be correlated with clinical symptoms, findings and other investigations . CA125 alone won't be used to diagnose cancer as it's raised in many other conditions which are benign .Speak with ur doctor why the test was ordered at the various possibilities.

CA-125 has found to be elevated (70.58U/ml) in case of my mother's report. She is in premenopausal stage (Age-48) and also suffering with irregular menses. She has also detected with a adnexal mass in ovary.

The increased levels should be due to benign ( non malignant) cause. Causes like endometriosis will have increased levels and respond goood with treatemnt. The benign causes usually respond to treatment. Speak with your doctor regarding the most likely cause of the levels in your case

Levels of CA125 can be mildly elevated in patients with granulosa cell tumor in certain clinical conditions like the presence of fluid in abdomen (ascites) . Most likely this is not a cause of worry, but the treating surgeon will be in a better position to explain in your mother's scenario . Please follow his advice .

Cancer is the cause of Ovarian cysts in only one percent, that too in elderly female who is in menopause. Non cancerous causes can also result in increased CA 125 like in endometriosis and needs to be rules out. However nothing can be said and done with out adequate clinical history and examination. What ever it may be Ur levels are not indicative of any cancer . There are several non cancer causes which can increase the levels. Hence there is nothing to worry . Please update the ultrasound findings

I'm extremely sorry for ur mother. Many research studies have shown Levels of CA 125 can help in the identification of the relapse. Usually Serum CA 125 levels will increase first followed by positive findings on scan( CT/ PET CT . The time between these two will vary usually 2 months( as observed in several studies). Your Oncologist will be the most knowledgeable person and deciding the further course of action . Sorry again
