    helpful , as a COTA I love OT's who write notes like this . I try to return the favor but sometimes get in a rut or bogged down. good to brush up , lighten up from time to time
    sometimes when the going is tough we forget the skilled services we provide due to being so used to our skills we think they are common sense, then a patient or CG says thanks I had no idea or never thought of that :)

    Been an OT for 30 years. In college was taught the right way and I currently follow. Make it measurable, note time duration and how they did compared to the previous session or progress from initial eval.
    I’m specifically looking for how to navigate Med B using HCHB program.
    Also add in functional outcome measurements. The SOAP note definitely is a great guide. Include the plan and that will set up the outline for your next visit. This would help the COTA to folllow a plan set up by the OT. Also add in what the patient is interested in , note their goals. Include caregiver training as we know visits are being reduced. Write if you taught family or a facility caregiver to promote carryover.
