Idea: Rusty keys. act like keys but they can be used multiple times (about 2-4 times) they have a 25% chance to remove the key whenever you use it, they are rarer than normal keys. the challenge to unlock them. name: Keys!!!. how to unlock the challenge: use about 20+ keys during a run. challenge: you start with slightly normal stats (slightly reduced stats), you have a key type itemact (an enemy I made) that will shoot keys at you that then split into 2 small pieces of keys that then move in the opposite directions, the enemy can be killed but respawns the next room, the enemy will also become a crowned champion (that have 6x more health i think) and you have to fight it to complete the challenge. When it ends: mom's heart/it lives, when this boss dies you have to fight the crown champion Itemact (which will have 120hp since it haves 20hp when it is not a champion) to actually complete the challenge, if you have already killed it during the boss (mom's heart / it lives) then it will respawn and then become the crown chapmion enemy

So fistula heart=pseudo holy mantle that creates an army when broken, with the challenge to unlock it looking piss easy. And you can hold multiple. Hm
