    Great episode, but someone's been drinking the GSK Kool aid. Proper comprehension that historically untreated depression did not lead to higher risk of suicide. Careful consideration of the evidence suggests that akathisia and mania are the best explanation for the change in the description of melancholic depression in the last 100 years. Most modern psychiatrists wouldn't recognise akathisia if the patient tore their own skin off, I think we can absolutely safely assume it's going unnoticed.
