For Capgras syndrome: this one fascinates me. I wonder if extraordinarily long times between visits with family and close friends can cause some of these delusions, or the doctors/helpers aren't hearing the whole story because it's a long-term trauma or dramatic change impacting the person coming for the help.
Under Cotard syndrome: TBI patients can suffer from this due to seizures as a symptom, if the seizures can't be controlled or altogether removed by medicine. I have not ever been told I had it, but I have known a lot of peers who suffer from it.

Does Capgras's Syndrome ever cause the suffer to think he has been replaced rather than family and friends being replaced?

Yes, that is just called "Reverse Capgras Syndrome" :) It's even rarer than typical Capgras.

Yes. I’m experiencing that now. Nothing seems real. No one seems real, myself included. I believe my symptoms were caused by a stroke. It’s not getting better.

Bipolar disorder type 2. Dysthymia. You are so sleepy even without medication that you just can't wake up.

I don’t know how long you’ll get a response but I hope you get help from a doctor in your area asap!! Please do and just practice grounding techniques and don’t try to self diagnose yourself either too. There are urgent cares also and nurse advice hotlines. Please take care of your mental wellness. Peace and love
