Master Solomon thanks for watching. Mostly according to the NMC approach to writing care study, we only write 4 problems and 4 strengths and work with 3 diagnosis. I can not tell you how they go about but I can only make assumptions. One is to save time because its an exams. But if it were to be for your own ward as an In Charge, then you have to follow through to the 4th diagnosis. I hope this answers your question. Solo

Yh because the problems were noticed on that same day. Nowadays if you go to the hospital, they write careplan (Nurse/Mid) daily...that's how it's supposed to be... Aside that, your licensure exams will be written on a certain day, and you can't write a different date and put solve the patient problems... It has to be the same day... That's why I advise you minimize the time you spent when writing your objectives... preferably 50 minutes, 60minutes ....to help you solve all three problems of the patient
Please kindly get the NANDA to gain assess to a lot of the diagnosis... But these are some eg. Impaired swallowing Ineffective child eating dynamics Ineffective infant eating dynamics Readiness for enhanced nutrition Imbalanced nutrition:less than body requirements
Patient's will be less anxious after 2 hours of nursing intervention as evidenced by; 1. Nurse teaching patient how to manage anxiety 2. Patient verbalizing he/she is less anxious
Client's risk of infection will be minimized after one hour of nursing intervention as evidenced by 1. Nurse performing wound dressing (if it's a wound) Or. Nurse performing catheter care(if it's a catheter) 2. Patient observing wound being dressed/catheter being cared for
